Very interesting hypothesis, I was aware of the migration of adaptive immune cells to the marrow during prolonged fasting but I was not aware that macrophages could be helping lipolysis like this. It seems like a good theory. The macrophage behavior might merely serve the function of cleaning up debris or it may be to assist with lipid distribution as you posited, seeing the macrophages collect small lipid droplets from the adipocytes themselves would seem to rule out mere cleaning activities. My own current hypothesis is that the macrophage switches from a sugar burning (M1) phenotype to a fatty acid oxidating phenotype (M2) and that they receive a special delivery of fatty acids cleaved with a different enzyme so that they do not run the risk of fuel exhaustion during fasting when the regular HSL system is often overloaded as you noted in the video. Also, macrophages are the only white blood cell with any real ability to run exclusively on fat as fuel, monocytes only have a limited ability to use fat for fuel. As most mobile immune cells return to the marrow during fasting it would also support the idea that macrophages are the only immune cells which can enter ketosis along with the body and keep active during long periods of glucose deprivation. Anyway, I love forward thinking like this so you just earned a new subscriber. :)

4.5 years ago I water fasted for 21 days with a 7 day re-feed; emphasis on sipping water all day long and rest. As an active person in my 40's i'm used to growing amounts of inflammation, fatigue, tightness. Inflammation and pain was one of the first things that subsided and disappeared in the first few days - post day 11 I felt nothing but the expected weakness. After th re-feed and returning to more regular eating and movement/exercise, I noticed the total absence of inflammation. And that lasted for 18 months.

@@walid7885 After 18 months, I gave in to some of the old habits, cravings, etc. But far less than before. It did create lasting change and shifted my eating. I would definitely do another...

@@x51DonGoddard Wow! 45! I would like to do another one but maybe not that much. After day 11 I just felt fine. No energy fluctuations, no symptoms, nothing. The "theory" of the practitioner who supervised mine was that it takes 11 days to get into the deepest level of Autophagy. So according to this theory, it's about spending as much time after that in that physiological state. But as maybe we both experienced (?) there may be a point of diminishing returns (?). The prescribed post-fast protocol was 6 weeks of raw vegan diet. I didn't accomplish this because it didn't feel satiating and I wanted warm food. But, my experience is that giving the body a rest (water only fasting) and periods of high water content foods (fruit) are very beneficial...

I appreciate your willingness to take speculative, educated guesses. This is a very important step in the scientific method that many people seem to forget about. If you don't take any guesses, you can't design an experiment and you limit yourself to the current scientific consensus. Until we have a magic pill, we need to keep taking educated guesses. We can't trust the science if the problems are getting worse. That means we haven't found the original catalyst yet....

Yes, at the same time, it's also very important to be clear that it is speculation and interpretation at this point. Many "experts" online seem to forget that and state things as facts when there's still a lot of nuance and uncertainties to take into account.

Absolutely. Simply short term stress (such as delayed gratification..) builds strength/discipline while chronic stress causes disease/dysfunction. Our metabolic system seems to work like this as well.

I think inflammation is beneficial most (if not all) of the time. The damage to the body is caused by whatever insult is causing the inflammation... but maybe allergies and autoimmunity are exceptions to this.

@@Mario-forall I think you are going to have to make a LOT more exceptions to your statement. Be sure to make room for foreign body retention (that cannot be surgically removed) ... I can't imagine what damage that inflammation causes. .. Although I don't have to imagine the pain...

I mean it's kinda obvious that inflammation isn't in and of itself a bad thing, and it only becomes bad (or good) depending on the context. For example exercise causes inflammation in the short term, but over the long term it reduces inflammation.

I think people are told stuff is bad too often, so they think everything related to it is bad. Inflammation is bad is quite common. Usually, acute inflammation is good as the body is doing its normal function. Chronic inflammation is usually bad because there is an underlying problem the body can't fix. It's like asking people what the bad cholesterol is. People will say ldl, cholesterol, or triglycerides, all of which are normal functions of the system. It's only when there is disfunction that they become problematic. Thanks.

100% Your body is doing it for a reason. You need to understand WHY before you say it needs fixed. Inflammation is your body trying to bring in extra fluid, nutrients, repair, and cleanup mechanisms (possibly transport also as I'd consider a possibility from these studies). If you reduce it, then you're reducing the regenerative ability if your body, but not fixing the root cause of the potential damage which cause it in the first place.

Unfortunately, not all root causes of inflammation are fixable. Thank goodness fasting .. particularly regular extended fasting, is a tool that reduces chronic inflammation.

I agree with your assessment that inflammation is not a good or bad thing but a marker of change. If your change body composition through autophagy (fasting) then that will necessarily increase inflammation but for all the right reasons. The result, after recovery, is a reduction of fat content (triglyceride content of fat molecules) or change in body composition.

Very interesting! My understanding from pathology is that macrophages will engulf excess lipid-carrying LDLs and can become foam cells which are often associated with atherosclerotic plaques in blood vessels. They do release proinflammatory cytokines which will attract other immune cells. Though foam cells are not bad unless they collect all in one spot. If you are fasting and using your fats as fuel, perhaps it will not be an issue and the macrophages are just cleaning up.

Connected to fat loss, is it true that toxins the body can not excrete (through kidneys, sweat and breath) are stored in fat? Then if the fats are broken down during a fast, would this release the toxins into the blood and thus increase inflammation temporarily?

I am in the middle of doing a 7 day water fast and one of the things I noticed on day 2 and 3 was that my body looked inflamed. Even though I am loosing 2 pounds a day. I do understand that what you are saying is that this type of inflamation is part of the process of athophagy in breaking down fat and using it to feed cells. That is awesome!

This makes a lot of sense, since fasting like exercise is a type of stressor. They work particularly well together. The best way to decrease chronic inflammation is to become acclimated to acute inflammation.

Great analogy, using the warehouse, holiday season and change in dynamics (increased volume, workers, etc). Short term inflammation serves a (protective) purpose, allowing the body to go into “hyper-repair” mode, resulting in a stronger, better outcome. Whereas long term long term inflammation can potentially create systemic problems (going back to the warehouse analogy) where delivery channels no longer work/ become less efficient, communications break down, resulting in the wrong amount(s) of “packages” to be sent, returned, exchanged, etc As always - very good information & presented very nicely!

I'd been experimenting with water fasting/interm'nt fsting for the last 5yrs, @ 68 y I want to bal Auto'gy w/ muscle retention. 2 y ago, right after a 5 day WF i noticed a few skin anomalies I'd had for yrs decay and drop off. ( A skin tag, a small wort and a tiny sebaceous cyst). I hypothesized this may have been an immune (inflammatory) reaction facilitated by autophagic brkdown of these structures. The observation caused me settle on three meals/day to maintain lean mass with a once/mth 60 hr WF to allow for system cleanup. Could autophagy activity be related to an inflammatory response?

My explanation goes something like this: lack of gut microbiome biome diversity is the cause of all disease. Where exactly inflammation comes into play after that lack of gut biome diversity doesn't really matter as much if you can solve the original catalyst. We just need to eat a greater variety of whole foods, less processed foods, and put an emphasis on returning the nutrients to our soil and get rid of the profit motives that are destroying our ability to make healthy diet decisions. "You are what you eat," and that includes your food *and* information diet. If your information diet is what's telling you to eat the bad foods, well then I suppose the information diet is the original source of our health problems.

Like most things, there are multiple angles. Consider this perspective. Insulin resistance can be physiologic and a normal response. It happens on a cellular level when we eat certain fats, but is transient. Think of it like the cell saying "no more please". It also happens to muscle cells when a body adapts to ketosis, causing the muscles preferentially use ketones for fuel. It can also be bad, such as in type 2 diabetes. Inflammation can be seen as a bad sign, tissues have been damaged. Many try to stop the inflammation with drugs. In so doing you reduce blood supply and fluid bringing in reparative and cleanup processes. This reduces the bodies ability to heal. I'd argue reducing it alleviates symptoms, but is a net harm in most cases... And ignores the root cause of the damage which lead to the inflammation.

If it reduces inflammation via antioxidant activity, reducing oxidation of fats or reducing other oxidative stresses, or maybe reducing blood sugar, thus glycation damage, then it could be good. If it i stead acts directly on Inflammation, vs the causes which lead to it, then I'd be suspicious it could be bad, without further evidence otherwise.

The obvious question is: What do the macrophages do with the triglycerides? Is there a mechanism for macrophages to deliver (release) triglycerides directly into the blood (or directly to other cells)? I suppose if such a mechanism were known, you would have mentioned it...

Fun fact it’s been used for medical or healing purposes since 5th century BCE. Hippocrates advised ‘abstinence from food or drink’ for patients who showed certain signs of illness. Russia had a large fasting medical clinic for decades then (Germany.) I’m more curious how it will be used 10-20 yrs from now. We’ll see.

I'd also be curious as to the type of fat taken up by the macrophages. Are they preferentially taking up very specific fats va others, such as oxidized fats, which we know they can take up, one of the causes them to turn into foam cells and lead to atherosclerotic plaques. Or maybe this is simply to help shuttle fat away for the body to process into energy, with inflammation being the mechanism to bring in extra blood supply and fluid for the macrophages to do their work.

I have twice done water fasts of over 20 days (5 years apart). On both occasions, long-term chronic aches, pains and annoyances were eliminated by the fast and never returned. I cannot recommend fasting enough. I'm about to go for 40 days.

Isn't inflamation part of the healing process? Like after doing sports there is some inflamation to clean up the damage and an influx of water and nutrients to help build new tissue but from the outside it's vidible as a swelling. I think sportists that cut short inflamation with medication have worse injury and tissue degradation. Could it be that there are toxins within the fat stores that when released the macrophages are cleaning up?

I wonder if this export of fat globules to macrophages is just one more backup process of an orchestrated immune response. During a severe infection, a person is likely fasting, yet still needs energy for vital organs and for the innate immune response to fight the disease. Could the connection to autophagy simply be a second-order effect of the need to keep the immune system operating effectively?

For me inflammation and oxydative stress are the vitals hormetic stressors that build up a strong immune system and strong metabolism. Just like suffering/damaging by burning and exhausting muscle by exercice build stronger body during the following anabolic response. The problem is constant inflamation without any adaptative response to counter it. What doesn't kill you make you stronger but only if it is followed by rest, digest, calm down and repair.

Inflammation (like bacteria or protein) has been filtered through mainstream/social media incl influential names (brands). A belief w/marketing becomes a billion $ industry w/cult like believers. Objective science is inconvenient and risk to profit. Great work as always Nicolas. PS: @0:48 prob this. They either forget or not aware good science constantly challenges itself and never (or rarely) constant or absolute. I’ve fallen into this thinking too at times. ✌️

Really interesting! I have a question that I've tried to get answers on, but the research seems to be lacking; Is there a significant effect on reducing autophagy activity when consuming very low amounts of calories, basically doing non-perfect water fasts? When reading blog posts about water fasting everyone seems to be really strict on calorie intake, but based on my understanding of the mechanisms involved for inducing autophagy I'm sceptical that intakes of let's say 50-100 calories per day for a couple of days would have a large effect. Do you have more insight or thoughts on this?

Yes - your intuition is 100% correct - a few hundred calories will have negligible effects on reducing autophagy.

If the macrophages were primarily facilitating faster release of fat into the blood, for energy/general use, then we might expect the macrophages to then quickly release the fat that they have 'harvested' from the adipocytes. And/or perhaps the macrophages have a lot more work to do during a fast, including facilitating autophagy for higher rates of protein up cycling, and so primarily they are simply fuelling themselves sufficiently well with fat to do this extra work? The inflammatory markers may just evidence a higher level of coordinated macrophage activity.

Fasting is fascinating! I think that if the inflammatory respons not only shuttles the fat (and if so.. where?), but also takes part in the autophagy of the fat tissue itself, then that could be a reason for keeping the weight off after fasting. Or maybe also during / after keto, which follows a similar pathway? Keep up the good work!

Hello! I have swollen neck lymph nodes after re-feeding post 3.5 days fasting. It’s more swollen in the Left side and it’s subsided now on 3rd day after re-feeding. The swelling isn’t noticeable and does not hurt. No other symptoms. Is the lymphadenopathy due to immune cells killing bad cells?

I just did an enforced 6 day fast on just electrolytes and antibiotics!! On the 6 day when I turned the corner and got out of bed without having to dash to a toilet, I noticed a dramatic change in my body. At 51 an ache and pain free body just does not exist for me. However on that morning I noticed all my joints were tension and pain free! Squatting bending and lunges felt free and easy like I was 20 again. 60 mins after my first food (vegetable and chicken soup with a slice of bread was all I fancied) all the usual aches and pains were returning.

Prof. Verhoeven, I'm in engineering field but have strong interests in nutritional health. Regarding inflammation, I wonder if the old idea that fat cells store lots of poisons and undesired components, and may need more protective/defensive agents to rid of these detrimental compounds as the cells go thru autophagy during release of TG is a reflection of the increased inflammation.

Never mix science and religion. fasting is my science. And so is evolution. And for a fact. we adapted to a world we're food was scarce. That is why we have fat stores. A chimpanzee would die if it didn't eat for a week.

Short term inflammation as a result of a metabolic process is natural and although we may not yet fully understand it, should not be considered bad. The body will reslove it on it's own once the underlying purpose/need has ended.

I think it might be a bad thing: just like macrophages gobble up oxidized LDL in atherosclerotic plaque and become foam cells, I imagine the extruded fat must be oxidized fat. But the macrophages would still be cleaning up this fat, so end result is still positive.

I would realy like to understand inflammation better. When on 3 day fast sometimes i I feel sick. But after fasting I always feel good. It feels like my body is able to solve something with fasting.

I have an inclusive hypothesis which allows for your hypothesis as well as others. Are there multiple genes involved? We know the body stores fat soluble toxins in adipocytes so maybe there's sometk connection there?

Considering most H-RCTs show no effect on inflammatory markers or improvement, this increased temporal inflammation appears to be therapeutic (I.e training the body to handle inflammation better), much like exercise and other stressors. Of note I have see 1 study in which fasting >10 days does increase markers.

There is something known as Keto-Rash, usually appears on some people with fsating ( including me) maned as "prurigo pigmentosa" . Do you think can it be also related with these macrophages? The cause is still know known. When you start eating carbohydrates, it goes away.

People fast because they seek a health benefit and when macrophages become energized by receiving endosomes they certainly become more capable of performing their health related tasks and so contribute to this health benefit. It would seem that endosomes might circulate to distant locations in the blood and so serve as a means of communication between remote components of the metabolism.

I though fasting reduces inflammation since I associate pain with inflammation whenever I eat too much and felt like crap the next day. But surprises me that theres more inflammation taking place when I fast and it reduces most of my pain away.

I have a chronic leukemia (no chemo yet) and LC and when I try (water) fasting I fell very weak and horrible while I'm doing it,very bad afterwards and without any improvements on my blood test results. Maybe keto is better option together with 16:8 fasting.

Sup, currently on my 4/21 day water fast, 3rd time, the benefits were so extreme the first time i just wanna go again, and i will probably do 1 more after as i still have enough body fat 15%ish, dunno what really happens when you do it my guess is a lot more then just "autophagy", after the first one my migraines/headaches during were close to none and after i managed to have far less migraines, and honestly it was closer to just normal 2-4h headaches with clearly less pain then before, this alone made me wanna do it again, after the 2nd one i pretty much had no headaches, but well i still have irritations/upset/fog still at least it didn't go into pain, i hope after this 3rd one even my irritations go away, only issue with 21 day water fasts is the huge loss of strenght/endurance, even with 1 month of eating and training as much as i can squeeze in i can barely go back to my regular strength/endurance

That's my concern about prolonged water fasts. I'm working out every day, I regularly run marathons so I don't think the gains from a long fast would outweigh the potential losses of strength and endurance and simply not being able to work out would probably drive me nuts, too. Also, at 13% body fat, I'm already at the low end of what's healthy for a woman so I don't think I could do more than 3-5 days without compromising my health and fitness.

@@edithgruber2125 if you are already healthy then y its a mistake to do a water fast, but in my case i had huge health issues and prediabetes, and after my first ever 21 day one, a huge amount of those issues subsided by a lot, so in my case the benefits outweigh the negatives by a huge margin, dat 13% body fat you can probably do 31 with medical supervision, you don't really lose more then 150-300g of body fat a day the massive loses are from water weight early on, and you quickly get that back after refeeding.

So you wouldn't recommend a prolonged water fast for people who are already healthy. That's also what I thought. I can't afford to lose any body fat, as for women, body fat under 13-15 % is no longer considered healthy, depending on which source you consult. Women need 10-13% as essential fat which means that their bodies don't function normally if they go below that. Going lower will affect the hormones which can lead to 1) amenorrhea (no more periods which sounds great at first but isn't healthy in the long run) and 2) RED-S, relative energy deficiency in sports which can lead to osteoporosis and stress fractures among other bad side effects. Men can go below 10% and still be healthy as they don't have the same hormonal needs as women as their essential fat is just around 5%. Thank you also for the numbers for daily fat loss. Knowing that I would lose 1-2% of body fat in a 3 day water fast tells me that that's the maximum I can probably try after Christmas when I have gained a little weight. Or I just stick to doing IF 16/8 and cut out the gingerbread and cookies which has worked for me in the past. I can lose 1-1.5 kg or 2-3 lbs in a week or two doing that.

@@edithgruber2125 damn i kind of forgot that for women its even harsher when it comes to hormones, in my view water fasting is mostly an incredible tool for people that essentially become food intolerant, their body just no longer works right after eating, and when you stop eating your body takes a break and heals, you heal normal, if u get a cut or something, hair nails skin all grow normal during, its just alot of fatigue, also u dont lose 2% body fat in 3 days thats just insane, you lose around 150-300g of fat a day, the rest is just "water weight" that you quickly add back on after eating again, i usualy gain like 3-6kgs withing 7 days of refeeding after a 21 day water fast, and if u consider 3 day water fast, just so you know there is little danger as long as u dont break that fast with some insane highly processed carbs with only salt, lets say a big bag of salty biscuits, i doubt you will have any issues, i would say 3-7 days are insanly easy for the body with very quick recovery 7-14 quite alot of strain low recovery 14-21 at least for me personaly is pushing it takes like a month of workouts and cardio to get back, so if u ever consider doing like 3 days just to see what its like, dont worry its easy its only hard becouse your in that "omg is anything bad going to happen" type of mind, i had it as well i only did 1-2 days at first

@@AngelTyraelGM thanks for the detailed reply. At the moment, I don't have any known food intolerances or insulin resistance so I'm doing fine. I keep the information you shared in mind for later in case I ever need it. As you said, starting with 1-3 days to test it doesn't sound so bad. I'm used to not eating for 16 hours when I do IF, so I guess 24 wouldn't be much harder. I do that as a post-Christmas reset for a couple of weeks unless I train for a marathon in April, then I'll lose the weight in training from January to April. Regarding the maths, a bit of context: when you say 150-300g fat loss a day, that would be 450-900g in 3 days. 500g is 1% body fat for me, weighing 50 kg. So even assuming the lower end of that range, that would get close to 1% for me. I try to avoid going below 6.5 kg or 13% body fat when I'm at 50 kg. I once went as low as 10.8% and then I constantly had colds and shivers and lacked energy so I had to regain weight to be healthy again.

It is not a secret that inflammation signaling makes macrophages consume more LDL, for example. This has even been suggested that inflammation causes arteriosclerosis, when macrophages are "overfed". In this case, inflammation would be indeed a bad news.

As, with all inflammation. It's a, precursor to growth. Growth does not mean gaining mass. As with exersize we with purpose cause inflammation so the body or immune system will call the alert to do its best to become stronger or more resistant to inflammation in future possibilities. So fasting is the same, water fasting or complete fasting as in (nothing) not even water, causes the body to inflame to create the need to become resistant. It's very simple to as with most academics they do thier best to make it sound as complicated as possible to confuse the average being. If you exersize the right amount you get more resistant to it which means you can handle more! If you do too much you cause damage not allowing the needed time to recover for instance. So you are correct, tho it very over complicated to cause others to feel dumb, thereby losing most people & then they simply do not realize it's as easy as balancing your intake with your output. In everything! Period. Thank You for your experience & your sharing. I've learned much from you, tho it does not need to be complicated. You must burn to have heat, you must release heat to allow rest. Simple It come down to what you burn & why when you rest for how long. 🙏🙏🙏

It would be interesting to understand what happens to triglycerides stored in macrophages. It seems that the macrophages, in the absence of sugars, being able to move, go to stock up on the only fuel available directly from the source. It appens aslo in very low carb diet? Has it been understood whether they use it for their own livelihood or whether they keep it to release it into circulation or take it elsewhere? Another interesting thing is to know if fasting causes a significant proliferation of macrophages or just a higher concentration in the adipose tissue of macrophages already present in the circulation. What happens to blood cells, inflammation markers and triglycerides levels?

i need your help. since you are my go to for all real numbers, the two questions are smothering me. heartbeats. do all animals have a similar lifetime heartbeat number. is limiting your heart beats to the lowest a longer-term success plan than exercise, especially high intensity???? i was told by my science brother before her died from...a heart attack...that all animals have similar life-time heart beat numbers. so, in fact, having more would limit your life. is a low-stressed yet healthy lifestyle a BETTER long-term plan than the now high intensity popularity? am i better off eating my limited yet healthier foods and sitting on my porch meditating rather than competing to be the more in-shaped high intensity athlete????

It would be interesting to hear how fasting impacts wider immunity. For example, some research shows fasting pre- chemotherapy can "protect" healthy cells whilst weakening cancer. How does fasting impact immunity to viruses and bacteria? I hear it's all about the "refeed" quality to ensure overall benefits. Whilst the fasting period itself seems to have all the usual effects one can expect thanks to autophagy

Given that explanation, could it be expected that a person who does regular fasting might have higher triglycerides than someone who stays well fed? If the body is regularly expecting to ship more of these triglyceride packages then might it be triggered pretty early in the process? In a recent blood test my triglycerides came in pretty high but my LDL was normal but HDL weas below where they want it to be. Obviously, the doctor immediately wanted to put me on statins, which I refused. Are triglycerides normally higher in someone who regularly fasts?

Hi and greetings from Cape Town. I find that you sound rather defensive about your point that fasting triggers inflammation. I may be the living proof of that thesis. I recently tried a 48 hour water fast, after positive experiences from intermittent fasting for five years. Instead of experiencing any benefits (mental clarity and overall’good’ feeling) I noticed a sharp and immediate increase in inflammation in my body, resulting in joint aches everywhere. Not sure how or why this can be favorable to my health. Nor do I understand how you can find it to be a cool discovery. Would be interesting to hear if others have had similar negative experiences from fasting.

A 48 hour fast isn't long enough for the body to adjust, you basically had the worst of it without the benefit of it. In general, it takes 72 hrs to fully adjust and to experience what people call the prolonged water fast benefit. It's really the 4th and 5th day when you start to feel like you can just continue and have that overall good feeling and mental clarity. Do a good deal of reading on water fasting by reputable sources. My personal approach is to go very low carb with plenty of healthy fats and fiber to feed the microbiome (kinda keto style) for the day of meals before beginning a fast. This helps one acclimate sooner. If you choose to go beyond 3 days, definitely understand what minerals to replenish. This can help with any aching feeling. Some may need to have minerals on day two, as it depends on how good your diet was before fasting. Going beyond 4 days really requires one to understand what do post fast do avoid any issues on returning to eating. Also in this video, he isn't talking about triggering general inflammation like you describe, but localized inflammation in the fat tissue.

I think the inflammation tag is a misnomer. I do not necessarily think someone water fasting should take electrolytes. Maybe zinc and d-vitamines if anything, maybe not. Whatever helps the mitochondria might help just not sure if that is helping the person. Complicated.
