I filed a provisional patent many years ago by simply sending it through the mail registered, with a check. Received a mail delivered notification. Provisional filed. Done. Less then 15 minutes. 30 years later, mail is not an option, and I am on my sixth week without my patent secured provisionally using poorly designed on line programs that do not interconnect, deny accessibility even when the 'steps' are followed, and lock up because of 'inter communication' issues. The patent office is simply another example of how much our crappy American government is! You USPTO people should be really proud of how well you are doing! But as long as those huge pay checks cash everything is good in your books, right? Jerks.
The examiners will charge you for different searches- even if it is on one design with multiple patentable inventions. The utility patent and design patent are two separate patents you might want to claim for one invention. The provisional patent and the non provisional patent are different patents.
