when i first encountered an orc with decoys, i thought maybe Talion had quietly gotten hit with some kind of poison and was hallucinating--it didn't help that that particular orc said something like "You're doomed now that you're facing me!" then the camera shifted to one of the decoys who sinisterly said "Or is it _me_ you're facing?" then another "Or me?" then another "Or _me?"_ (i could easily be misremembering the details and i don't know if i saved the clip of that run-in)
