In this video we explore ptosis, droopy eyelid, sand options for eyelid surgery, specifically ptosis surgery by using the example of Forest Whitaker’s ptotic left eye to understand ptosis, droopy eye and how we as ophthalmologists and Oculoplastic surgeons are able to fix this type of droopy eye with surgery.

I think a lot of us are so used to his left eye abnormality or it just being a bit droopy as you might say. I am sure most of us, just accept it as being part of who he is. I think the important thing is that it's obviously not causing him any real pain or discomfort, not really impairing his vision in any way. Whether or not he chooses to have something done about it is entirely his own decision as these issues are very personal and quite frankly nobody else's business. But the thing that has always struck me about Forest Whitaker is that he comes across as a very genuine person and as such is very likeable. He's smart and intelligent, more so than many of his acting counterparts in the industry.

Amazing video! I want to know, are there eyedrops you mentioned available in Australia? I got a prescription from an eye doctor but it was a tiiiiny bottle. I wish we had upneeq here! I wonder if there are similar ones being sold under other brand names? Thanks 😊

Thanks for posting this video. I just had surgery for ptosis. They went in under the front of the eyelid. I have stitches under eyelids. I have swelling as well. Both eyes were done. Right eye was much lower than the left eye. It’s my 4th day after surgery. Still have swelling but my right eye still looks lower that the left. The left looks slightly higher than the right. They are not symmetrical, they still look asymmetrical. I have a follow up with surgeon next week and I still have swelling. Once swelling goes away I will see what the true final results are.

@@visionx46 my right eye is much lower that the left. Doctor said I need to do another surgery. There is a big difference between the two eyes. The right eye looks worst than before because the left eye is higher. I will do it again in six months.

@@nini.xo.1 The right eye is still uneven and the left eye some times gets teary and I also experience dry eyes. I saw my eye specialist who stated it takes a good six months for everything to be completely healed. The eye surgeon prescribed upneeq eye drops for the right eye. It helps lift it a bit so it looks even with the left eye. Thanks for asking.

Hi, I have been wearing a scleral shell for many years due to disfigurement from surgery to reattach my retina. Because of this, my top eyelid has shrunk over time . Is there a way to get plastic surgery for this so I can close my eye fully? I had gone to a surgeon for entropion repair for the lower lid and when I mentioned it , he didn't give my any solutions or hope, and this was at a well regarded hospital.

Great great video! Love the explanations. One question though - so if after the Phenylephrine test, the right eye falls, it means that you would operate this one too after the left eye even if it did not have ptosis?

If it falls after then it likely has masked ptosis to begin with, just not as severe as the more ptotic eye. So it usually involves treating both but with different degrees of ptosis repair. Hope that helps.

If your brow has dropped on one side, can it be repaired? I’ve had multiple surgeries and my eyelid and brow are now worse than before. I had the surgery done by my medical provider, and they were horrible! I should’ve never opted to allow them to put a frontal sling on my forehead. I don’t think I needed it at all, just needed the eyelid lifted a bit.

If I email pictures of my eyes post Blepharoplasty/ lacrimal gland surgery could you advise me. I’m a foreigner in an Asian country and I haven’t really gotten post operative checks 🥺 I’m so stressed because I’m sure I have ptosis. 😞

Hello Dr, my right eye = MRD 0MM, PF 5MM, LF 12MM, Normal lid crease. Left eye MRD 4MM, PF 10MM, LF 12MM, Nirmal lid crease, no lag. I wasn't born like this, I ran into a metal fence. 1. Do you think I should fix this and 2 after reading the reviews on Google regarding the doctor that will be doing the surgery has very bad reviews.

Hi Dr. Keenan. I want to ask you why is it when you look at past match data for ophthalmology residency spots there are sometimes a couple spots left open? It just seems weird because isn’t ophthalmology supposed to be a sought out specialty?

I think this is sometimes just a consequence of the Match process but often those spots get filled in the end because they’re always med students who want to do ophtho

This is so interesting and informative. I was born with ptosis and your video has taught me a bit more about my condition. I also have Marcus gun which generally comes with congenital ptosis - could you do a video on Marcus Gunn Jaw Winking syndrome as little is known about this condition and I would like to find out more on it.

@@WyattMoore756 I've had two ptosis surgeries (my ptosis was covering half my eye), Its not perfect but I am happy with the results and also its helped with my self confidence tremendously. I can't speak for everyone as I know that it all depends on the severity of each individual's ptosis. I need to constantly lubricate my eye with drops as I have a small gap in my lid from the surgery uplift but I don't see it as a major inconvenience. For me, I am happy that I had the surgeries. I say, if you have ptosis and thinking about it, speak to a medical specialist and they can tell you if there are any risks. For me, the first surgery I had as a child I think I stayed in the hospital for about a week, the second as an adult, it was a quick process and I was sent home the same day.

I can't speak for everyone else as severity of ptosis will be different for each individual and you also have to take into account any personal risks etc. For me, personally, I am very happy that I went for the surgeries, although not 100% symmetrically perfect, it has helped boost my confidence majorly. The only con is, I cannot fully close my lid when sleeping so this results in dry eyes which I have to lubricate at night with a thick eye ointment and sometimes during the day with drops but I do not find it an inconvenience at all. I say, speak to a specialist and see if you are happy with accepting any side effects which may come with getting it fixed.

​@@WyattMoore756 I've had two ptosis surgeries to make it more symmetrical, not 100% perfect but I am very happy with the results and it has certainly helped with my self confidence. Personally I would recommend if you feel it is affecting you a lot and if you are ok with the side effects that may come with it (dry eye, constantly having to use eyedrops/gels to lubricate the eye).

I love this video. I was diagnosed with aquired ptosis. My neuro ophthalmologist is unsure of the cause. He ran some tests for myasthenia gravis, which came back negative. He also did an MRI with contrast. What are some causes for aquired ptosis in young individuals? Also, how long is the recovery after surgery?

I’m glad you liked the video 🙌🏼. In young people myasthenia certainly can cause it. We will often check for that in young people. The other thing I see is in young people who wear contact lenses.

Sir I am a freelancer. I have floats and flashes aslo light sensitivity problem and focus problem.My eys's can't focus the whole image.In the night flashes are increase i can't see anything in night and in the day light sensitivity 😔.Please guide me.

Fascinating! As I age, my eyelid seems to be heavy. This video explained a lot. Would like to here why the iris has cloudy rim as one ages. I’ve heard hereditary factors and or cholesterol deposits, or diseases if you are younger. Does diabetes cause cloudy rims?

I have a dropping right eyelid and a right eyebrow that my brain subconsciously raised to lift my eye lid. I did have a corrective surgery, not Plastic Surgery i dont think and it raised it by very few millimetres, Dr didn't correct it further because he said that i wouldn't be able to close my eye fully, leading to dry eye, is this true? There was no mention of it in this video. Six years later from that surgery, i hate that its still noticeable and how high that eyebrow is, would you be able to tell me what my next steps would be?

I have a very similar story. I've had 2 surgeries on mine. First one didn't work at all. Second one worked but not as well as expected. I still have blocked vision, my actual vision is getting worse and need glasses, and mine causes pain around the eye muscles and my neck due to tilting my neck. They can't figure out why it's doing it or how to fix it. Been dealing with this for 10 years as it continuously gets worse. I wish I knew what to do next too.

I heard it’s called deceitful eyes. Have you heard of this? It was talked about in “vaughts practical character reader”. And then from a scientific study where they read faces “Researchers have discovered for the first time, five tell-tale muscle groups that control facial expressions, activate differently when we are trying to deceive.” They studied this with Tracie Andrews a woman who had murdered her husband and was lying about it could be read through her facial expressions.

Hi, that’s an interesting question. Because congenital ptosis is a problem with the way the muscle is formed i.e. there is fatty infiltration of the muscle itself and dysgenesis, it’s a good question if there is some role for something like that, or even a genetic target that could help in the future. My suspicion is that all of medicine will shift toward genetic engineering in the next few centuries but who knows?

When I asked my ophthalmologist about this he referred me to a plastic surgeon. Was that correct are ophthalmologist capable of performing this surgery?

Ptosis surgery should be performed by an Oculoplastic surgeon (which is an ophthalmologist with subspecialty training in eye plastic surgery). Regular or facial plastic surgeons can do blepharoplasties but should not be doing ptosis surgery.
