Form SSA 3368 walkthrough (Disability Report - Adult)

I have not seen any specific guidance, although you can verify with the SSA office to see what they recommend. If I were in your position, I would probably include all tests that I had the results for, or actually scheduled, whether or not they appeared related to my disability.

I'm not familiar with that form, and I couldn't find it on the SSA website. Do you have any more information on the form?

@@teachmepersonalfinance2169 Ah yes, I had it wrong. I think it is called the 55555 Disability Update Report. It is only 6 questions.My brother wanted to ask about the best way to answer them to not fall into any pitfalls.I could perhaps link to what the top of the document looks like it you want. Perhaps it is only a Utah Doc. (sorry for the late response, only now do I see my first one had an error)

@@teachmepersonalfinance2169 That is very very similar. Mine is a Utah form that basically has all the same questions, but different Formatting. As too why I was asking about it... Well I guess the trouble is kinda passed. My brother was trying to fill it out last week. And was afraid of answering the questions 'incorrectly'. I'm sure you know what I mean. We eventually visited the SSA building in our area. He filled it out with the help of my Father. Who recommended we fill it out in a way that implies we would like to meet with a representative, and that things are still up in the air. Any specific questions Icould ask about that form.... I dunno
