Funny things you hear in restaurants (Your burgers come with meat?) 

not crazy at all. people have certain taste for things together and apart. they may take left overs to other people who also likes it together or apart so it's a very reasonable request.

A pizza with a mix of pepperoni and sausage is different than only pepperoni or only sausage.

Yeah I usually get pepperoni and sausage but sometimes I just feel like only one or the other. And sometimes I want just half of one and half of the other.

If I heard (for example) someone calling a bacon lettuce and tomato sandwich a burger, I would think they were mentally deficient and from a group home for the mentally disabled.

It's not "any sandwich" that they call a burger, it's anything served on a burger bun. So, fish on a burger bun would be a fish burger. Here in the US, a burger is generally only a ground beef patty. Sometimes maybe a turkey burger, but only if it's ground turkey in a patty.

Ya, the world burger is actually short for Hamburg-er (hamburger), as the dish originated from Germany. Cheeseburger is just a short hand for hamburger with cheese that doesn't make sense unless you know that both a burger is short for a hamburger, and the cheese is just a prefix adjective. It makes perfect sense in English grammar, not so much in foreign languages.

Depends on where you're from. In certain places you would get breaded cheese that's been fried or you could get a hard cheese that's been grilled.

If the burger has "vegan meat" or "meat substitutes" then you could be accused of lying if you tell a customer that it has meat.

veg burgers where i live are just potato patties not meat substitute

In the Netherlands you literally get what they list on the menu, especially in regards to sandwiches. When I first moved there I asked what a “broodje kaas” was and they said it was a cheese sandwich. I ordered it thinking it would come with some lettuce or maybe a salad on the side… it was literally unbuttered sandwich bread with a slice of Gouda cheese on a plate. I then asked if the Netherlands was ever a communist state. Never. I then asked if they had indeed been liberated from the nazis. They didn’t answer that question. 😊
