This car came to us with some problems, and we've found the culprit!
Came to us for diag, drained oil, found pins in the oil from the timing chain. Popped valve cover and found exploded cam phaser. Told him motor needs replaced. Got new G3 Long block from Ford. Pulled old unit today. Noticed an unnecessary amount of oil residue on the lower half of the timing cover. Kept moving. Pulled crank pulley bolt. Saw it mushroomed. Immediately realized that they did not reinstall the 1/2" washer on the crank bolt - which distributes the Torque to the Damper and seals up to the crank Sprocket, etc.. open the timing cover and find total chaos!!!
Our thoughts: the crank pulley bolt was not actually able to be torqued due to the washer missing, thus mushrooms the bolt, then the bolt head slides right thru the center of the Damper, let's the crank Sprocket weaken over time at the keyway, snaps at keyway eventually, and the cam phaser exploded, causing the rest of the chain reaction destruction!
Not all installs are done with the same care or competency!
