GTA: Vice City - The Definitive Edition - The Rub Out Duplication Glitch 

Basically, i got stuck in the mission titled "Rub Out" just because i died and my checkpoint got saved on a corrupted place, which led to everything being duplicated and then Lance dying if i succeed at the mission, i eventually figured out the fix for this.
