He was already under a lot of pressure and I doubt he had the temperament in that moment to analyse the situation properly. Too much guesswork in cricket

He did not think it was close enough so he was shaking his head right from the start of the appeal. In hindsight we can say it was an error of judgement. I don't think the umpires are trained to do a permutation & combination on the field, they make decisions in a split second and if they are burdened more they might make more mistakes. Sorry for a hypothetical scenario but Imagine team not having a review left, umpire miscalculates and gives a decision thinking team has a review left, will lead to more problem. Better solution would be for the teams to use the review more wisely or to increase the quota of number of reviews a team can take. I agree though that ultimate aim should be to get every decision right. DRS is evolving rapidly and who knows we might see every decision being made by technology in future.
