When a healthy heart beats, it makes a “lub-dub” sound. The first heart sound “lub”, also known as S1, is caused by the closing of the AV valves after the atria have pumped blood into the ventricles. The second heart sound “dub”, or S2, originates from the closing of the aortic and pulmonary valves, right after the ventricles have ejected the blood. The time interval between S1 and S2 is when the ventricles contract, called SYSTOLE. The interval between S2 and the NEXT S1 is when the ventricles relax and are filled with blood, called DIASTOLE. Diastole is longer than systole, hence the lub-dub, lub-dub, lub-dub…
Heart sounds are auscultated at 4 different sites on the chest wall which correspond to the location of blood flow as it passes through the aortic, pulmonic, tricuspid, and mitral valves, respectively. This is how SIMILAR defects associated with DIFFERENT valves are differentiated.
Heart murmurs are whooshing sounds produced by turbulent flow of blood. Murmurs are diagnosed based on the TIME they occur in the cardiac cycle, their changes in INTENSITY over time, and the auscultation SITE where they are best heard.
Examples of conditions associated with common systolic murmurs include:
- MITRAL valve regurgitation, when the mitral valve does NOT CLOSE properly and blood surges back to the left atrium during systole. The murmur starts at S1, when the AV valves close, and maintains the same intensity for the entire duration of systole. This holosystolic murmur is best heard at the mitral region -the apex, with radiation to the left axilla. Because the valve closure in mitral regurgitation is INcomplete, S1 is often quieter. On the other side of the heart, a TRICUSPID valve regurgitation has similar timing and shape, but it is loudest in the tricuspid area and the sound radiates up, along the left sternal border.
- AORTIC valve stenosis, when the aortic valve does NOT OPEN properly and blood is forced through a narrow opening. The blood flow starts small, rises to a maximum in mid-systole at the peak of ventricular contraction, then attenuates toward the end of systole. This results in a crescendo-decrescendo, or a diamond-shaped, murmur which starts a short moment after S1. It is often preceded by an ejection click caused by the opening of the STENOTIC valve. Aortic stenosis murmur is loudest in the aortic area and the sound radiates to the carotid arteries in the neck following the direction of blood flow. Again, on the other side of the heart, a PULMONIC stenosis has the same characteristics but is best heard in the pulmonic area and does NOT radiate to the neck.
Other conditions that cause audible systolic murmurs include ventricular septal defect and mitral valve prolapse.
An example of diastolic murmurs is aortic valve regurgitation. This is when the aortic valve does NOT CLOSE properly, resulting in blood flowing back to the left ventricle during diastole- the filling phase. As the blood flows in the REVERSE direction, the murmur is best heard NOT in the aortic area, but rather along the left sternal border. It peaks at the beginning of diastole when the pressure difference is highest, then rapidly decreases as the equilibrium is reached.
Other common diastolic murmurs are associated with pulmonic regurgitation, mitral stenosis and tricuspid stenosis.

I had open heart surgery last Aug 2020 to repair my mitral valve (I had severe mitral regurgitation). Prior to the surgery, my murmur was so loud I can hear it whenever I lay down in bed, "SHHH DBSSH" 😂 After surgery, it was exhilarating to hear the normal LUB DUB heartbeat sound again! I consider it a creative miracle. Medical science is amazing.

I was born with a hole in my heart and still have a murmur. I often have low blood pressure or abnormal readings as the nurses always put the squeeze band multiple times and double and triple check my numbers. My doctor ordered blood tests but said they were normal. I don’t know what to do I am afraid the hole in my heart never closed properly. No doctors are accepting new patients in Canada besides medicentres where you get a different random doctor (normally a student) each time you go

Explained Heart sounds and murmurs better in 4 minutes than hours of lecture and studying did. This would have been REALLY appreciated 30 yrs ago. I was told I have 3 leaky valves by my cardiologist, but he says the problem is mild. I go to my cardiologist every 3 months, have blood test done before each visit. I have EKG test in office, stress test on the treadmill, once a year and the regular physical exam. Next, will have a CT scan. So far, so good. I keep my weight down, walk 3 miles daily, exercise, eat veggies and fruits, fish, chicken...very little red meat.Dont smoke and rarely have a glass of wine. Take my vitamins..I feel great so far. Other people in my family had the same problem. I think it's genetic. I hope my problem won't get worse and affect my quality of life...

You don't normally hear heart sounds, let alone the abnormal ones. Hearing them heart sounds is a reminder of how well this dynamic organ works to keep you alive but also how fragile it is to the forces of life. I didn't like hearing when I started meditating but I got real familiar with it after awhile and actually find it comforting knowing I'm not really alone on this earth, I've always got my body, specifically my heart to remind me.

We had a dog. A Newfoundland I named Chloe. She was such a pleasant and easy puppy to take care of, not a hassle like other puppies. It seemed to good to be true. Not only did she take frequent naps, but she also showed no interest in the pools we set up for her (newfies are supposed to love the water). We soon discovered she had a heart murmur. She was such a gentle puppy because she was tired, and she didn't have much time. We had to let her go. And when we did, all I could think about were the times when I saw her sleep. Her paws would twitch. In her dreams she can have all the energy in the world to run as much as she wanted. It was when she was sleeping that she was the happiest.

I'm 75. I had Aortic valve replacement on March 4 2020.. I had no complications at all from the surgery and seemed to heal quickly. Very minimal pain or discomfort. I received a Model 2800TFX Pericardial tissue valve (Bovine)...which should be good for hopefully at least 10 years.....which would put me to 85.....and at that point depending on how much more advanced the technology will be by then could have a mechanical valve inserted to carry me to end of life. I remember hearing these heart sounds both before and after surgery. Its wonderful that these operations can be performed and extend peoples lives.

how did you find a doctor to monitor your heart murmur? i was born with a hole in my heart and continue to have a murmur (my mom never followed up with doctors/tests and doesn’t believe in modern medicine so I never had any follow ups after being born premature 5 pounds 5 ounces)

I was born with this condition and I had a surgery at the age of 1, my mum realised I was breathing in a strange way and the doctor told her it was a heart murmur. I was the first heart murmur surgery case here in my city and luckily I'm still here.

When I was younger, I had all three of the Innocent Heart Murmurs. I went to the cardiologist and all of his students came and listened to my heart. It was very strange for 3 y/o me having all those people around me 😂

Just found out I have a “pretty loud “ heart murmur. Have an echocardiogram scheduled a month from now.....wish me luck
