I’m 44 yrs old diagnosed with breast cancer a year ago. After double mastectomy , I started chemo on late August 2023. I had hot flashes starting on chemo no 2 of 16. It was bad , it did wane down for towards the end of my chemo . I had my ovaries removed in January 2024, I experience some bad hot flashes again for few weeks, then it got better , but now it doesn’t come as often .There are days I don’t get hot flashes at all . Will be starting letrozole and Lynparza in April. Hopefully my hot flashes don’t come back in a vengeance . What I been doing , taking magnesium glycinate at night and magnesium oil on my foot and vitamin D. That’s all the supplement I’m on. I swear the magnesium helps with my sleeping. I start exercising , barely drinks coffee, instead I drink more matcha , eat more healthily and I dont stress on things I can’t control…

As you prepare to start your next treatments, it's understandable to have concerns about hot flashes returning. Am on letrozole since November 2022 and experience hot flashes since then. What helps me the most is exercise, especially running and weight lifting and a cooling yoga breathing exercise: roll your tongue into a pipe and slowly breathe in and out through the pipe. Last summer hot flashes were bothering when it was hot outside and my oncologist prescribed me a light anti depressant (venlafaxin), which also helps against hot flashes and is known to prevent BC recurrence. I have a fan with me and use it when needed.

Been on Letrozol all for two months and so far only mild hot flashes. My doc told me to take vitamin E to help. Well see how that goes.

It's assuring to hear Letrozole's side effects have been manageable for you. Some evidence suggests vitamin E may help reduce hot flashes, but the research remains mixed, with many studies showing limited or no benefit. Keep us posted on your experience.

Hot flushes started when I was 55, 5 years before my breast cancer diagnosis. I began taking Letrozole in February 22, with 5 months interruption while I was having chemotherapy. The hot flushes did not change very much over those past two years despite the changes in treatments. They last no more than 2 min, and happen day or night. I can usually anticipate their onset through a mood change: as the bad mood passes the hot flush arrives!! This is teaching me not to take mood changes too seriously. Morning yoga makes me feel good, and as it's weight bearing, hopefully, it also protects my bones.

I started having Hot flushes after I started Tamoxifen. Once I was diagnosed with Male Breast Cancer I felt like a lab rats. I feel some of the medicine combinations are causing side effects that concern me. Maybe you can do a post on side effects of combinations of medication

It sounds like you're not having the best experience overall. While we have much less data in people with male breast cancer, there are "tried and true" treatments. In other words, you are not technically a lab rat. Combinations of treatments give people the side effects of both medications.

Ovarian suppression with medications or removal or radiation to the ovaries are all reasonable options for someone who has functioning ovaries. The benefit is highest in people who have high risk disease and is effective only in people who have functioning ovaries.

Ado-trastuzumab emtansine (brand name: Kadcyla) is a HER2-targeted antibody-drug conjugate used to treat HER2-positive breast cancer. Ado-trastuzumab emtansine is used in people with metastatic HER2-positive breast cancer who previously received trastuzumab and a taxane chemotherapy, either for metastatic disease or disease recurrence within 6 months of adjuvant therapy. It is also used in people with HER2-positive early breast cancer patients with residual invasive disease after neoadjuvant taxane and trastuzumab-based treatment. It is extremely effective compared with trastuzumab alone. Side effects include nausea, fatigue, musculoskeletal pain, thrombocytopenia, increased liver enzymes, headache, peripheral neuropathy. @@yerbba I am a Her2 positive EARLY breast cancer (Stage 1B) patient with 3% residual disease after mastectomy on left breast. I also received chemotherapy before surgery. Start tomorrow (June 5th), I will start my 1st “Kadcyla treatments” for 14 cycles/ 3 weeks. Thank you Dr Griggs for the response on my question

It’s my 10th day on letrozol and kiskali. And I get a monthly shot of enantone for hormones. I use a portable small fan that I can carry around my neck or put in a table. At night, when I get hot flashes, I just turn it on as the sound is very low and I get a relief instantly. I also bought Silk pyjamas and bedsheets. Very very useful. For summer, because I have to protect my arms, I have long sleeves silky shirts. With all this I don’t feel stressed anymore. 😅😊

It’s been almost a week of taking tamoxifen and my mom is having difficulty in breathing. Is it a side effect of it and how can it be treated? Any suggestion

Tamoxifen can extremely rarely cause inflammation of the lungs (pneumonitis), leading to shortness of breath and a dry cough. It would be reasonable to stop the tamoxifen and then to talk with her medical team as soon as possible.

thank you for your information. I started to take Anastrozole for a week now, I have not had any symptoms yet, I was wondering how long I would expect to start feeling the side effects. I’m 64 years woman. New diagnosed ER positive ca. Thank you!

Great question. Side effects start within 1 to 2 weeks and peak (are their worst) at 3 months.

Common medications for hot flashes and anxiety due to estrogen loss include medications that are used for depression and anxiety as well as hot flashes, including SSRIs, SNRIs, as well as many other medications. It's important to always discuss these options with your medical team to find the most suitable one.

Unless someone is iron deficient, however, iron supplements should be avoided outside of a daily vitamin if someone has been advised to take a daily vitamin.

Why do I have cold flashes?? I’m on Enhertu chemo for stage 4 breast cancer and my core sometimes gets so cold I have to use a heat pad.

While not common, this symptom is experienced by some people. If the cold feeling is during your treatment, it's possible that the iv fluid is chilling you. If it is constant no matter where you are, let your primary physician know as there may be other underlying conditions worth investigating.

Mam my mom is 47 and on tamoxifen. Her period stopped during chemo till now. But as she is somewhere in age of menopause.. should she continue tamoxifen for 5 yrs or she should be given letrozole?? Plz guide mam er pr positive stage 3. Treatment just completed.

You should be asking your mom's oncologist about this -- they know her medical history and prescribed the tamoxifen in the first place.

It is likely that your mother's ovaries are no longer making estrogen. With Stage III (3) cancer, she will most likely be offered 10 years of endocrine therapy (5 years of an aromatase inhibitor and 5 years of tamoxifen). Starting with tamoxifen for 5 years is a reasonable option. Hope this is helpful.
