Home Blood Pressure Testing Consultation - Dr James Gill
A crucial part of treating raised blood pressure is actually determining a patient's true blood pressure. The easiest way to do that is with effective home Blood pressure monitoring.
A patient is asked to test their blood pressure in the morning and in the evening three times, selecting the lowest readings. This is done for five days, and the average is calculated.
Some people will be lucky enough to have a family member, or a partner, who is able to use a manual sigma monitor in order to check the blood pressure. However, even in this situation, many people find using an automated blood pressure monitor, makes the process much easier.

High blood pressure runs in my family, and both of my parents who take medication for theirs frequently monitor their high blood pressure at home for any changes that might need to be made by their doctors so I really appreciate this video! As someone who works retail unloading trucks and restocking the sales floor I have often wondered if I would eventually end up falling victim to my family history as a result of working in a fast paced environment but thankfully so far I have yet to do so. Additionally I’ve been tracking my pulse and blood pressure etc. from every blood donation appointment over the last 19 years as a backup extra measure on top of everything else just to keep consistent and that’s been a huge comfort for my anxiety.

Excellent video there. Hope that guy will be okay, his blood pressure surprised me, you really can't tell just from looking at a person it seems. As you've said in other videos, high blood pressure can be a silent killer, so it's a good idea to check it occasionally.

@@DrJamesGill I foolishly check my BP most mornings and enter it into the Garmin connect app, unfortunately it doesn't seem to give an average, though does give a weekly range, which for me this week has been between 99-116 systolic and 63-76 diastolic, so that's pretty good. Once a week I also check my blood sugar. My father has been diagnosed with diabetes, though I don't have it as far as I know. During the pandemic though I had one instance though where I fainted multiple times after feeling miserable and staying in bed all day, not eating anything and when I eventually got up and went out to get some food I felt dizzy and the next moment I was being picked up by two people, then I fainted again two or three times after that, and experienced sudden temporary blindness. I assume that I was hypoglycemic and that's what caused it, but it was a bit worrying, still is slightly worrying not knowing what the cause was. I'm still not even sure if it was hypoglycemia since it was only for about 24 hours that I didn't eat...

I had postpartum hypertension just after my baby was born this year, my BP was much higher than this. It was terrifying, especially having a newborn. I really wish I’d been given this much advice & information when I was being treated (as an outpatient & inpatient).

My mom had mostly normal blood pressure but spikes if high blood pressure when she gets very upset. Idk if she should be on meds for this she doesn’t communicate much with her doctors.

I had an home set for a few weeks, but that was an automatic reader. And than it was a two in the morning and two in the afternoon. I must say that it was for diabetes.

I’ve recently been offered blood pressure medication from my gcp and was given one of those at home kits! Any advice on ways to keep blood pressure down? I’ve adjusted my diet to watch my sodium intake but are there any other suggestions? Thanks for the videos!

Exercise if there's nothing stopping you. You might not want to because exercise is hard, exhausting and can even be painful. But the way I look at it is you can either pay that price of sweat and effort now, and get what you paid back later with better health, or you can choose the easy route of avoiding the effort and pay for it later.

One of the simple gains for patients with slightly raised blood pressure is adding beetroot into the diet. There are several, good studies, showing about 5% reduction in BP due to the nitrates in beetroot Personally I’m skeptical about recommended salt reduction. We USED to recommended it, but last evidence hasn’t suggested it is really impactful, and some some patients might cause harm

All my local health shops carry the home tests for blood pressure. I get the feeling that, for some people, this might only serve to increase anxiety --- especially if they aren't operating it properly?

GP practices now encourage you to do your BP at home and submit readings because people's levels seem elevated more than normal when done in the practice due to the anxiety/pressure of being in the GP surgery to begin with. As Dr Gill mentions, White Coat Syndrome.

There will always be people with health anxiety, if I felt that was an active part of high reasons we can try to I’ve come it by using ambulatory home blood pressure minoring - the patient wears a monitor for 24hrs, much less intrusive and it inflates randomly

Home blood pressure monitoring is more specific, ie is more “true” than practice monitoring when it comes to high BP

I've had a sphygmomanometer for years, but the readings have been vastly different from what they read at the doctor's office.... Couldn't figure out why. Apparently it wasn't calibrated correctly 🤦🏻‍♀️

At least you've got that sorted! How long did that take to sort? Often Miscalibrated equipment is the most difficult to address

@@DrJamesGill Fortunately not long. I had started nursing school so had to practice in labs with classmates with school supplied equipment. My instructor asked me if I felt ok because it wasin the area of 170/90s. oopsie. 🙈

It is possible, it’s curable though. A cardiologist can give blood pressure meds if it’s high and give you salt tablets if low

Actually yes. I worked in O&G during my training. There is a lovely balance between surgery and medicine in the job. However it is a very emotional job, and quite prone to complaints

The evidence suggests less accuracy. We are advised to use arm ones - keeping in mind the issue with a BP cuff needing to be correctly sized anyway
