Some states require them some states do not. I currently only practice in one state that requires a collab so I have one in that state.

You can only see patients in the state you are licensed. You can technically work from anywhere in the US.

It depends how much your services cost and if you have passive income opportunities set up in your practice. I can physically see as little as 5 patients per week and can generate $10k.
