Yes, I was wanting a medical doctor who knew what she was doing. One doctor recommended mixing the Retinol with moisturizer and putting it on together, but this method gets the Retinol into the skin.

I heard that there’s no such thing as spf 100 :0 isn’t that crazy?! That you just pay more for something that does the same job with a lower spf! It’s crazy! I use a spf 25 i believe ☺️ I love spf

@@samanthacruz6135 it's true. Anything above spf 30 is pretty much the same lol. Spf 30 protects 97% of uv rays. 40=97.5% spf 50=98% spf 70=98.5% Also, the higher the spf has shown false since of security and people are known to have longer exposure times the higher spf they use. Just get 30 and be reasonable with the Sun. She giveth and taketh

6 months after posting this, my skin definitely looks different. Been using Paula choice 1%, slowly worked up to a point where I can use it 5 times a week, saving 1 day for no product at all and 1 for exfoliating. Never had an issue with acne or spots, frequency of pimples and normal spots are very low, can't remember the last time I had any spot, that wasn't shaving related. Skin texture and overall look, health has significantly improved. Used sunscreen daily apart from when I was in doors in the winter. I'm 22 years old.

Hi dr. Bomer I still figure out how to moisturize well when using retin-a since it's so easily come off when use moisturizer. So, can I mix retin-a with face oil on my palm? Or should I use retin-a first then wait about 30 minutes and then dab face oil on my face? I'm using rice bran oil.

I'm happy my video has been helpful for you. Moisturizer can go on first before a strong retinol if you have dry skin or you have easily irritated skin. The moisturizer before retinol will create a barrier to retinol penetration, so the retinol will penetrate the skin more slowly over time and overall not as deep. This effect depends on the moisturizer however. If the moisturizer is a liquid, it actually will increase the penetration of retinol into the skin and potentially make it more irritating. On the other side, if the moisturizer is thick and has a lot of barrier function in it (like shea butter and/or contains silicone ingredients for example) then the retinol will penetrate less and be less irritating. Bringing retinol on to the neck works for some people very well. These people usually are also able to tolerate application on the chest also. I am not one of those people, but I have many patients in this category. It is best to start with the face because it is more resilient and heals faster than the neck and chest. To start, put the retinol below the jawline under the chin first before the whole neck. Do this for one week. If all went well, go a little lower down, but not all the way and do this every other day or even less. Again wait another week and judge your response. The reaction most people get if they don't tolerate it well is itchy skin. When it is itchy, most will scratch it. After scratching is when red bumps appear - important to stop scratching and let it heal. It will take about 1-2 weeks to heal completely. A 1% hydrocortizone cream over the counter can help ease the itching; but don't overuse because it can make the bumps more red. The best method for advancing to the neck and chest is to advance slowly and remember where you applied it. If that area become itchy - don't scratch it to avoid getting the red bumps. The low concentration cortizone can be very helpful for itchy skin and if there are no red bumps the irritation will be gone in a couple days. Once you understand your reaction to retinol on the face, it will be a predictor for your neck and chest. But always need to remember the strength of retinol is very important. If the retinol is not medical grade, the concentration will be so low that most people will be able to use this low concentration retinol everywhere without any irritation. This also means there will be little rejuvenation effect. We provide an assortment of medical grade retinol products on our website rejuventskincare.com. This is fascinating and yes I was told to apply over a thicker moisturizer - my go to for winter, I am using my tried and true good ol Cerave moisturizer in the tub (the blue and white thick one). I will be applying prescription Tretinoin Cream 0.025% over top. I had explained to my Derm that I struggle with already dryer skin type even during hormone fluctuations that naturally occur. This is a wonderful explanation scientifically why she recommended a thicker cream and starting 2 days a week now I see. Hey listen slow progress is still progress in my book! I am not one for a quick fix for anything and I just love the glow retinol gives it's a beautiful finish to the skin very kich like you have here. Thank you for taking the time to reply in such detail. As far as the neck/jaw that is a great starter you gave and I will follow up with my Derm on how to move forward. Id never slather on such delicate skin like my moisturizer but I was curious if it were doable. Its gunna be a slow ride for me but eyes on the prize right? @@rejuvent-skincare amazing. Okay I'm gunna lay it out since you seem to get the game. I use good old cetaphil thick lotion the tub at night.i KNEW to layer I knew it! Slow progress is effective progress right? So layering. I actually stopped wearing makeup for now (maybe eyeliner that's all) to allow the skin to breathe. I will say. I do live in NYC. Despite Covid regulations Doc youd be surprised how many salons/ clinics are readily available now! So - layer the moisturizer. Apply retinol delicate and with focus. Apply moisturizer again down the neck How are you not more promoted via YT? I thank you so much for our in depth replies. I can see you truly enjoy what you do and have studied to excel in! Hello Dr. Bomer and thanks a lot for this video. That's so clear and thorough. I would like to start slowly with Retin-a at the minimum percentage and see how it goes. Indeed, my skin is fair, really sensitive and somehow reactive to some ingredients (alcohol, perfumes and some natural extracts are the main culprits) . Moreover I suffer from seborrheic dermatitis-rosacea in certain periods, and some products does not work so well for this condition. I wonder if a am a good candidate to start with tetrinoin, and if I have to apply it all over my face (a pea size), or just where I see wrinkles e.g. forehead, avoidind the beard and neck area. Hello Gabber from Rome, seborrheic dermatitis- rosacea can be helped with retin-A in a slow to advance regimen. Start with the small pea every other day or every 3 days and follow with moisturizer. With these skin conditions, it is also very helpful to reduce or remove sugar from the diet as well as all dairy. Good luck - Dr. Bomer

Thanks for the video! I was just wondering if you have any tips for applying retinal with stubble on the face? I've noticed it's difficult to spread out the pea sized amount evenly and feel like the hairs absorb the product rather than my skin... if I shave, I usually break out, so I try to avoid shaving when at all possible. Thanks :)

Josh, you will need to use a little more retinol because of your beard, maybe twice the amount you are using. If you are consistent with using retinol, it will keep your skin exfoliated and this should decrease the breakouts that occur with shaving. Using a 5 or 10% benzoyl peroxide (Jan Marini is my favorite BP) after shaving for a couple days helps with breakouts as well. Sincerely, Kelly Bomer, MD

Dr. Bomer, thanks for the timely info! I've been using .1 differin gel for about 6 months with little to no change in my complexion. The .1 used to be enough to clear me up about 5 years ago, but doesn't seem to be the answer for me anymore. I used to shave daily while using .1 differin with absolutely no problems breaking out afterward. I've tried using .3 adapalene but it was way too drying and made my skin red... I also have very oily skin and not sure if applying moisturizer would be appropriate. I seem to get mixed answers whenever I read up on this topic. I've currently been taking an oral antibiotic for about 2 months. And I've taken two 5-month courses of isotretinoin within the last 4 years, which both worked wonders but only proved to be a temporary solution before my oily skin returned and acne soon after. I've tried tweaking my lifestyle habits and currently am doing cardio 3-4x/week, eating a healthy diet, no added sugars, limiting caffeine, taking a multivitamin and fish oil daily, and practice good hygiene. Any advice? I've been struggling with this for a few years now and it's really been holding me back in life. :(

Hi Dr. Bomer, I am wondering what is the process of increasing the percentage of the retinol serum? I have been using the 0.25 one for almost two months and am considering to use the same percentage for the coming four months. That is six months in total. When would you suggest me using the 0.5? Hi Jo Liu, It is best to keep increasing retinol strength as time passes. As long as your skin is tolerating the lower strength with daily application for several months, it is time to increase the percent strength to one tier up, so for you that is .5% retinol. I think you are ready! Remember, our skin is changing all the time, so our tolerance can change and that is okay. We doctor our own skin by monitoring it daily. If absolutely no irritation, no redness and no dryness is happening from the current retinol you are using, then a higher concentration is going to provide a more rejuvenating result. Remember, increase retinol or start retinol preferably when you have a stable environment without excess sun, water and wind... Have a great day - Dr. Bomer.

I apply retinol 2-3 hours after I’ve moisturized. Retinol should be used at night just before one sleeps. My good friend is a dermatologist & advised applying retinol on dry skin.

You explained everything so well!!! But isn't it better if we apply moisturizer on damp skin? So should I slightly dampen my skin after applying the retinol in order for the moisturizer to absorb better?

Moisturizer does perform better when your face is moist before application. BUT with retinol it is different because water makes all products penetrate deeper into the skin. This is not helpful with retinol, most of the time, because it gets too irritating. The water or watery products will promote uneven penetration resulting in uneven irritation that is often worst in the creases like corners of your eyes, nose and mouth. So it is best to not wet your face after retinol application and it is also important to start with dry skin (not moist) before applying retinol. Therefore, not watery moisturizers are best after retinol application. This is true for most people who are using a medical strength retinol. Occasionally, some people's skin tolerates retinol so well that watery products help it to penetrate more so they continue to get a stimulating response, like a little dry, pink and flaky. Thanks for the video. I read where I need to apply retinol (.5 at this point) to a dry face. However, after a couple of weeks of doing this, with no adverse affects, I started applying to a clean and just dried face, so probably slightly damp. I did this because I find it hard to distribute the cream on such a dry face. I'm just wondering why we should do it on a dry face and what happens when I apply to a slightly damp face?

Sorry for the delay in response. When the skin is moist, it is more permeable to products applied topically. For some skin care products like Vitamin C serums or glycolic acids (which are both great products to include in most skin care regimens) better penetration is beneficial. However with retinoids (Retin-A and Retinol), the moist skin can allow for a deeper, and possibly uneven penetration which can increase the chance of irritation and flaking. Once one becomes acclimated to retinoids, applying to a damp face is fine, as long as there is not excessive irritation. It's actually a way to make your retinoid stronger as long as you tolerate it.

The damp face does allow for the small amount of retinol to be moved around on the skin more easily. But the moisture can lead to irritation because the moisture increases retinol penetration into the skin. It is important to understand this, but if the damp face is working for you that's okay. If you don't want to have sudden surprises with irritated skin, you can blend the retinol with your favorite creamy moisturizer to help it spread and enhance more even retinol penetration into the skin.

Absolutely. Just start slowly while getting acclimated to a Retinol. A good quality moisturizer applied after Retinol can help if you are dry.

It is best to apply retinol on dry skin. On dry skin the retinol will penetrate the skin more evenly. If is applied on wet skin after a serum, the retinol will penetrate more deeply into the skin and be inclined to cause exaggerated irritation. The serum can be used before retinol, but let the serum dry before the retinol is applied. Happy Skin Care Days - Dr. Bomer

hi,i want to ask something-i saw on youtube, people who use retin A and bee venom in the same time,mixed both..or apply bee venom imediatly after put retin A..they say had a great result and they make a routine of this..i want your opinion..is good to apply in the same time or at few minute this 2 products?..i have retin A 0,05 and i buy recently bee venom with honey and i want to apply too,but id liked to hear your proffesional opinion..thank you..i wait for u reply..

Shazii Zammr I started using 2.5% retinol straight away and it worked wonders for my skin, didn't irritate it at all. My skin is oily and it has helped control the oil and help with acne and texture

Lindsay Connally What product are you using if you don't mind sharing? I ordered a 2.5% cream from Eva Naturals and they have very positive reviews. Although, I did see on the commentary where someone asked about 2.5% and the Doctor stated it's probably not 2.5% but something alot lower...

I was wondering if you could answer something for me. I had the paula's choice recovery serum with retinol and the intense resist cream with retinol, but it felt really weird on my skin. It was like something was moving om my face for hours. They told me to stop using it, so I did. But I heared you have to get used to it. And the products are serum or cream with retinol, so I would think the retinol % is low? What do you think? Thanks!

Your reaction does not sound like a typical retinol sensitivity. In most cases, the reaction to retinol is delayed, often 18 to 24 hours. Common symptoms of a retinol reaction are red, flaky, dry and possibly itchy skin. In addition, the concentration of retinol in both those products is extremely low- 0.01% per Paula's Choice website. In comparison, we often start patients new to retinoids on a 0.5% (50 times as concentrated) medical grade retinol such as Jan Marini (better for skin on the normal to dry side) or Skin Medica (better for oily skin) and both are well tolerated when applied as directed. In my experience, 0.01% retinol (equivalent to 0.001% Retin-A) is not strong enough to significantly rejuvenate skin. I feel you are most likely having a reaction to another ingredient(s) in those products and not the retinol. I agree you should stop using those products. To maximize your rejuvenation efforts, I recommend you seek out a Cosmetic Dermatologist, Plastic Surgeon or Facial Plastic Surgeon and work with their aesthetician. I’m glad you found the video helpful. Both Retinol and Retin-A (Retinoic Acid) are effective at rejuvenating the skin. Specifically they both increase collagen and elastin in the dermis, while exfoliating the epidermis. Fine lines, texture, enlarged pores and acne can be improved with the regular use of either Retinol or Retin-A. They are similar to each other in molecular structure; the main difference is that Retinol is converted into Retinoic Acid in the skin itself, at a conversion rate of 10%. So the concentration of a Retinol should be 10 x higher than a Retinoic Acid to have an equivalent effect. For example, a 1% Retinol has the equivalent strength and effect of a 0.1% Retinoic Acid. While Retinol is effective, it must be in a high enough concentration (0.5-1%) to work. Many brands sold over the counter in drug stores and department stores do not have enough Retinol to actually improve your skin. Medical Grade Retinols (Jan Marini, Obagi, Revision, SkinCeuticals, Skin Medica, ZO etc.) available in Physicians’ offices and Medical Spas are of sufficient strength to be effective. Another difference is in side effects. Until one’s skin is acclimated, use of Retinols or Retinoic Acid can lead to red, flakey irritated skin. This can last several weeks, especially in patients with sensitive skin who have never used them before. Retinoic Acid does not have any additional ingredients to limit the irritation. Medical Grade Retinols have additional ingredients (often plant based) that reduce inflammation and make acclimation to a Retinol product easier. In addition, some Retinol’s are slightly more suited for dry skin, some are better for oily skin, some have lightening agents and some have ingredients for reducing redness. Now, I'm looking for a serious product to acclimate my 34 light and in an overall good shape skin. I'm living in Paris and I have access to skinceutical for example in medical drugstore. I was planning to consult a specialist to ask for a prescription products but in afraid this would be too strong for a start. That should be fine for daily use without a lot of sun exposure. When you are spending more time outside in the sun you should add a separate sunscreen for maximum protection. Ideally one that is at least SPF 30 and has both UVA and UVB protection.

Will Differin 1% cream give me the same results as Retin-A? I can get Differin over the counter. Others on RU-vid have said it is a retinoid same as Retin-A. Life According to Maria Hi, I have just been prescribed Differin (I'm over in the uk) Differin is a weaker topical retinoid then Retin-A and has not been proven to be effective specifically as an anti wrinkle or anti ageing treatment (due to lack of research) thus is typically only used as an acne treatment, however due to the effects Differin has on the skin it is likely to have some effectiveness against wrinkles and improve skin tone. you need to wait for 30 minutes after applying a topical retinoid such as Retin-a or Differin before you can apply any other skin products, it is generally advised that you do not use any other products that could dry out the skin such as alcohol etc as this may worsen the side effects of topical retinoids.

Differin is similar to Retin-A, however it is better for those with moderate to severe acne. But if that were the only retinoid that I could find, I would use it for rejuvenation. The medical grade retinols available today are wonderful because the formulations have various ingredients to help with brightening, reduce irritation, enhance tissue hydration etc. After a retinoid or Retin-A is applied to the skin, moisturizer that is creamy may be applied directly after the Retin-A and can even be mixed with the retin-A to make a retinoid that is a lower concentration if the Retin-A is too strong for your skin at this time. But remember, if you use the retin-A regularly, your tolerance will increase as your skin becomes younger. Avoid water moisturizers with retinoids because there will be more irritation.

If you're using a real retinoid (Tretinoin, Retin-A, Adalphlem, etc.), the order does not matter. Most people find it best to moisturizer first, then apply retinoid a few minutes after. This helps to prevent more irritation. But either way works. Some want the moisturizer on top.

What's your moisturizer while using retin-a products? If i apply moisturizer before using retin-a, will it still works? Because when i'm moisturize after retin-a, it seems my retin-a come off 😑

I currently use Jan Marini Transformation cream as my moisturizer. I apply it after washing my face in the evening after work, and again before I go to bed after putting on my retinol. There are several options you can try If you feel your moisturizer is making the Retin-A come off. I recommend letting the Retin-A soak in more completely before applying your moisturizer (how long depends on how humid or dry your climate is). Alternatively you can actually mix the moisturizer with the Retin-A and apply them together. Another option is to apply the moisturizer before using the Retin-A. It's possible applying the moisturizer with or immediately before the Retin-A may limit it's absorption to some degree, but it will still work.

Hi I've bought 1% but never used before. My plan was to just dilute it at first but I've just read that it shouldn't be diluted with water based products. Could I dilute it with rosehip oil? I'm just a bit worried about putting it on face undiluted as never used retinol before. Thanks :-)

I recommend applying Retinol or Retin-A after application of toner and serums. But you should wait a few minutes after serum application for your skin to dry, otherwise the moisture of the serum will make the Retinol or Retin-A penetrate more deeply and possibly not evenly which can increase the chance of irritation. - Dr. Bomer

I used to confuse in the same point . For me after retinol I go with moisturizer and face oil. And I have the time gab around 30s each step. (sometime I mix cream with face oil) I do this for a couple weeks my skin looks ok and well moisturize in the morning. I hope this help.

As long as the niacinamide isn't making you red or flushed, it's OK to use it first. If the niacinamide makes you flushed, you should not use them at the same time- consider using the niacinamide in the AM and the retinol in the evening.

You leave it on until your next facial cleansing whenever that might be. Leaving it on overnight offers the easiest and best results, but it can also be applied in the morning if your skin is not very sensitive. The retinol soaks into the skin and does not need to be removed. Just resume cleansing as your normal routine. Cleansing the skin morning and night does give the skin the best change for looking refreshed and healthy. K Bomer

It is by the brand SminMedica. The one she mentioned here is called SkinMedica TNS Recovery complex which only has the growth factors in the serum. The brand also offers others that have more complex ingredients like antioxidants and peptides. Those ingredients are in the TNS Essential Serum and TNS Advanced. The TNS Advanced is new to the market. SkinMedica was the first brand to use human growth factors which greatly helps aging skin

If you do not get a response from a retinol product, it is not strong enough. I recommend you visit rejuventskincare.com and invest in a new medical grade retinol. If you have dry skin the Jan Marini or Revision retinols are your best option. If you have oily or T-Zone skin, while all retinols may work well, the Skin Medica and ZO SkinHealth might be better formulations for you. We have to remember that each person's skin is different and will respond to certain medical brands better than others. So if you haven't found a good fit, it's important to keep looking.
