The tips about retinols are nice, thank you!. However, it is NOT true that "all the aging is just photo aging" that is simply wrong. There is also intrinsic aging like lost of bone density, fat lost and also muscle atrophy. So an affirmation like "all aging is just photoaging" can generate false expectations in people, be careful with that. Greetings from Spain! 😉

There is some misleading/inaccurate information in this video. 1. 1:08 Tretinoin is not the "generic name" for prescription strength "retinol" (more accurate to say RETINOID). It is a TYPE of prescription Retinoid. Strictly speaking it is a retinoic acid. Others types of prescription topical Retinoids include Adapalene, Tazarotene, Trifarotene. 2. 1:20 "just going to call" Tretinoin "retinol" because of difficulty with pronunciation is not appropriate. You can refer to it as "retinoic acid" or "prescription Retinoid". But it is misleading and potentially confusing to viewers and patients if you choose to simplify or dumb it down because it's "easier". 3. 4:48 Retin A is tretinoin. It is strictly topical. You do not take systemic Retin A to treat acne. The acne medication you are referring to is Isotretinoin, commonly known as Accutane. Although the names are similar they are in fact different medications. I appreciate you are an eye surgeon and dermatology is not your field of specialisation but you are making these videos to educate viewers and with that comes a responsibility to provide accurate information. Speaking as a fellow medical professional, I hope you too can appreciate that correct terminology and information is important.

Hey there! Just wanted to let you know that Tretinoin does come in pill form. It's usually only administered this way to promote cell growth in leukemia patients undergoing chemotherapy. Trentinoin (prescription) is not just "higher concentration" then retinol. Prescription trentinoin is retinoic acid. Retinol has to be converted by your skin to retinaldehyde, then to retinoic acid. So it's a multi step process where trentinoin is already retinoic acid so it doesn't have to go through all those steps. It's not just the strength that's different.

I'm 67 years old. I've have had crow's feet my entire life. I didn't wear sunglasses or sunscreen when I was a child growing up in New York. I lived on Long Island for most of my life. I started getting painful dry eyes and blurry eyes after 3 weeks of using accutane and went to my dermatologist where she told me its impossible that accutane is the cause of those problems. I have never had issues with my eyes in my whole life until i used accutane. I then stopped using accutane where the dryness and blurry vision improved a bit but it hasn't resolved and 4 years has passed already. I guess accutane caused permanent damage to my eyes somehow.

Thanks for this video! I have been using tret for acne for quite a while now and I never really made the connection between using it and my dry eye getting so much worse - I saw an eye doctor last week and she diagnosed me with meibomian gland dysfunction. We talked about punctal plugs (she put me on a steroid eye drop and a lipid one for two months to see if it helps) but there's a good chance I'll have to quit using tret altogether - I'm already struggling with dry eye a lot after two crosslinking surgeries for my keratoconus and I'm horrified at the thought that I might have done even more damage to my eyes by using tret. Do you think it'll get better if I drop the tret? I don't apply it around the eyes but it *is* pretty strong stuff. :(
@DrRupaWong
@DrRupaWong 3 года назад
Yes, hopefully it will improve and the tret hasn't caused permanent meibomian gland dropout. It is strong stuff. and you have to be careful because of your keratoconus. Try the plugs and steroid drops as your doctor recommends and see if that improves it.
@aquana
@aquana 3 года назад
@@DrRupaWong Thank you for your reply, I will do so! I have stopped using my tretinoin (I was on the 0.1% version, so literally the strongest dose ever). I'll try 0.025% or a cosmetic grade retinol to see if my eyes are willing to put up with that as acne is no joke either and I've heard you can have results with that, only much slower (and I already have results, I just need to keep them up). If not, well - I'll just have to face the fact that I cannot use Vit A products because of my eyes. Thank you again.

@@TheUltharcats The steroids didn't really help. I had to get silicone punctal plugs and drastically lowered my tret dosage + went for another brand of lipid-based eye drop because I couldn't deal with my horrible acne and I figured it doesn't really matter, my eyes are screwed from keratoconus and crosslinking surgery anyway. I was off it for a few weeks but didn't really notice any improvement. Trying scleral lenses soon.

The bumps are called millias. I've had them for ages (they run in my family) and I've tried tons of products and done lots of research. I'm sad to report, the only thing that really works is having a dermatologist manually extract them. From my experience, even the best skincare products I've used have only made a minimal difference, only visible to me when I'm looking with a magnifying mirror and good lighting.

@@elenapopovic2527 That's unfortunate, I too have tried several products. I haven't found anything that works, including high end eye creams. Hope you find something that works for you!

@@Jesus_is_Lord555 I have all the problems you have, including the bumps, which in my case (and possibly yours) aren't milia but just visible sebaceous glands. Nothing can be done about those. I have also tried everything for my undereye circles, from IPL to laser to microneedling to filler and even carboxytherapy. None of those worked but I finally found something now that made a difference. It's called Sunekos and it's an injectable containing hyaluronic acid and amino acids. It's not a filler, it's more of a booster. My undereye cicles aren't gone, but look a lot better. I think for now it's only available in Europe though (and you need a skilled injector), so it depends on where you live. You might have to wait a while.

Thank you so much for explaining how retinols can affect the lacrimal and meibomian glands! I started getting recurring styes since I started retinol and didn’t make the connection until a year later. My primary care doctor and optometrist also denied that there could’ve been a connection. I’ve seen some dermatologists recommend applying a thick product like Vaseline underneath the eye and on the eyelid prior to applying retinol. Do you think this would be a good solution to preventing gland damage?

Hi Dr Wong, watched with a lot of interest. I've been using tretinoin since July 2023 (1 month) and usually use a pea size on my face once a week. Whatever's leftover on my fingers, i would gently apply over my eye area, a bit below my eyebrows and below my lower eye lashes. When you say retinol is toxic to the meibomian glands and globlet cells, do you mean when it gets INTO your eyes by accident or when it is absorbed by the skin near your eyes? I wasn't clear on this despite watching your video twice. Hope you will be kind to confirm. I do have evaporative dry eyes syndrome. Recent eye check confirmed my meibomian glands is making oil but the oil is not flowing down, it's just hardened under my eye lids. But I have always been suffering from dry eyes since my 20s and now at 47, it feels even more dry. After seeing your video, am not sure if it's because of the use of tretinoin around my eyes but it's only been 1 month of usage. I have been careful not to get any into my eyes but would really like to know if absorption of it via the skin around the eyes can also cause death of the meibomian glands and goblet cells? Thank u so much.

Kind of scared me Hydration and essential fatty acids / omega 3s help dry eye.... alcohol can cause dry eye. I’m 59 and people take me for in my late 30s... yes good but not if I may be damaging my eyes using Retin A under my eyes. I would appreciate your opinion . Thanks for your video !!

@DrRupaWong
@DrRupaWong 4 года назад
It can cause goblet cell death. Goblet cells make the mucin for your tear film, so without it, your tears evaporate too quickly, leading to dry eye syndrome. LASIK patients can get more dry eye after surgery and it can disrupt the refraction, so that's why they want you to be off Accutane before having LASIK. www.ncbi.nlm.nih.gov/pmc/articles/PMC3694789/?fbclid=IwAR10FTO0lsY4UwTb0edufT3iK4tJ7jqJR6Cv36vV5Z3z1hAEYYAAiGbTOWU

So happy to find an ophthalmologist with a take on this!! What about covering the eyelid and lining the lower lash line with aquaphor to stop it from migrating into those areas? Will that protect my eye health but allow me to still get the benefits of the retinol on the skin around my eyes (which is where I show my age the most...I’m 41)? Also, when you say right now users are not advised to get Lasik....Do you mean they should stop the use of Retin-A before their surgery and then they can resume after they have healed from Lasik? Or do they have to give up retin a for good? I was just beginning to look into lasik myself. Thank you for your time!!

@DrRupaWong

It shouldn't b/c it should be far enough from the conjunctiva and lids. But it's why oral tretinoin can cause severe dry eyes and they won't let you have LASIK if you're on it.
