Hi Dr Griggs, how about doubt related to effectiveness of AIs in terms of someone who is 90% ER and PR positive. Appreciate your guidance. Thanks.
@yerbba
@yerbba 15 дней назад
Thanks for your question. We have a great deal of evidence from randomized trials that in people with a high percentage of ER and PR, aromatase inhibitors (AIs) are very effective. Thanks for watching.

These "how to cope with" videos are points of light in a darkness. Thank you! Sorry if I've missed these in your content already, but would you consider doing videos focusing in more detail on coping with the "anniversaries" of diagnosis/treatment and "scanxiety"? I've found other resources I also rely on for those topics, but I would very much like to see your version and suggestions (and I'm probably not alone in that. Please direct me if you've already covered this.) Also curious if you have more detailed thoughts specifically on the usefulness of hypnotherapy, particularly for side effect management. It's an area I'm having trouble finding reliable information on (and it isn't covered by most insurance so experimenting may be out of reach financially, but if there is evidence it might help, it could be worth sacrificing to attempt.) Do you have any content re: medical gaslighting and how to cope with that specifically wrt breast cancer treatment? Or how to cope with co-morbidities and doctors who don't take those into account or even acknowledge them or the challenges faced by a patient dealing with multiple large issues?

We appreciate your suggestions and have put these on our list, so keep an eye out for them soon. Hypnosis involves deep relaxation and is helpful in multiple settings, particularly those that are associated with anxiousness. However, the quality of evidence varies across conditions. While nearly all studies report positive effects, many suffer from limitations like small sample sizes and risk of bias. More high-quality research is needed to establish hypnosis' potential for conditions like depression, sleep disorders, and eating disorders. Hypnosis is not a replacement for established treatments like medication for severe mental disorders. Its efficacy depends on factors like the individual's hypnotizability (suggestibility) and the therapist's skill. Hypnosis carries a risk, though small, of creating false memories through unintended suggestions. Thank you again for your ideas and for being part of the Yerbba community.

@@yerbba Thank you so much for taking those on board. I will definitely be looking out for any you can manage to create. You've filled in the gaps and helped validate and support me so much, I often feel like you're one of the best parts of self-care! WRT hypnosis, it's been suggested to me for help with AI side effect management (motivation to exercise, sleep/hot flashes, tolerance of pain etc) + potentially to help with some sensory issues I have to wearing compression garments. Similar line of thought to massage or acupuncture as an additional source of non-medication support.

How soon is surgery recommended for stage 1 invasive ductal carcinoma, ER and PR+, HER - ? My biopsy results were 12/21, saw surgeon 2/9, completed preop tests and genetic tests in February but still have no surgery date…every time I call the surgeons office they tell me they will call me back but I’m still without a date. I’m 62, and without a surgery date my head tells me it’s not there , like I’m in denial and don’t need to worry about it. Is this length of time letting that cancer grow?

I had surgery within 45 days of diagnosis. The delay was trying to get on the plastic surgeon's schedule. I had a bilateral mastectomy with diep flap revision. You definitely need to get a new doctor. Is there a cancer center near you?

For Stage I (1) invasive ductal carcinoma, surgery is usually scheduled as soon as possible. The characteristics of your cancer make timeliness less critical, but the distress of waiting is understandable. Consider reaching out directly to your primary care physician, your surgeon's office, or seeking a patient advocate for assistance.

After nearly a year of treatment, I recently started feeling a lack of confidence more than ever. I have lobular and ductual cancer, and I want to do cancer antigen testing to watch for activity to know if there might be activity of recurrence. My oncologist says these tests are unnecessary, and are not used in deciding on any kind of treatment. Therefore she refuses to order them. I can pay for them myself through Life Extension. Since chemo typically doesn’t work well for lobular cancer I feel like a ticking time bomb. What do you think about cancer antigen testing?
@yerbba
@yerbba 15 дней назад
Feeling anxious about recurrence is common, and we appreciate your sharing your doubts here as it helps other people feel less alone. Cancer antigen tests are not helpful in people without symptoms. They do not rule out cancer and are elevated in only half of the people with recurrence. The best way to monitor for recurrence is with a history (symptoms) and a physical exam. We hope this is helpful in understanding why doctors don't order these tests.

First stage can save the breast and remove the lump
@yerbba
@yerbba 15 дней назад
In early-stage breast cancer, breast-conserving surgery (lumpectomy) is often possible and aims to remove the cancer while preserving as much of the breast as possible. Discuss the best surgical options with your oncologist. We are wishing you the best, and thank you for watching.

My wife is 37 years old and we live in Pakistan and her left come hard and also ultrasound report shown the 30*15*9 mm suspicious mass what is this lump or what stage is it
@yerbba
@yerbba 15 дней назад
It's difficult to determine the specifics of the mass or the appropriate treatment plan without knowing the unique characteristics of the tumor, any pre-existing conditions, and overall health. Further diagnostic tests, like a biopsy, are essential to provide more clarity and guide the next steps in care.

My higher power is Epona, Celtic goddess of horses. I believe in her as much as I believe in any other higher power. I have found it remarkable how much goddiness I have encountered since beginning my effing rotten detour. (I refuse to call it a "journey," a word which is linked in my mind with prospects of adventure and discovery.) Yes, I find it disturbing. Not all breast cancer victims are superstitious. Some of us are utterly secular.
@yerbba
@yerbba 15 дней назад
It's important to hold on to whatever gives you strength and comfort during challenging times, and your connection with Epona reflects that beautifully. Your perspective shows that everyone's experience with cancer is unique, and not everyone finds comfort in the same ways.
