Hello! Do I use last name, first name for #10? Then since the name doesn't fit in the boxes do I write on a separate page the whole name or only the other letters that I couldn't fit?

Okay, so I got my passport a few days ago and they screwed up my birthplace. But they still have my birth certificate in their possession, so what should I do since they still have it? Also, do I have to take a new passport photo or send in my original photo? P.S. I also have made six copies of my certificate with the birthplace highlighted and I intend to mail all six of them in so make sure this screw up doesn't happen again. Thank You For Your Help.

For a replacement of an emergency passport, do I need to include my birth certificate and ID if the consulate did not tell me to? Meaning they did not check off the boxes for required documents on a document they gave me

You should probably reprint the form so it’s properly done, they can be pretty stingy about it

Thanks for a quick reply! But I meant my error with my passport is that it was printed crooked and I couldn’t get it scanned in the airport. I was wondering where on the form do I indicate that ? I’ve read people check box #22 and then on the last name part they write crooked printing. All the information is correct just the printing is the error..

Sorry I’m a bit confused. If your picture is bad, you just want to attach it to an application right? So you just need a new picture before applying… is that right?

there is a fillable form online that I found but it says DS 5504 C instead of DS 5504 A which is what the one that the county Clerk gave me ...wtf! its not expired either they are both the same date and updated, but the clerk gave me a form and that letter changes on it why? ugh I hate this why cant they make these forms easy why!

You need to update your information to make sure it matches your new, legal name. Send in your legal documents showing your new name

So if I am filling out this form because of a legal name change, would I put my new name at the very top under item #1? Or would I put the current name in the current passport that I have right now? I was confused because item #21 asks for the new name that I would want on the new passport.

#1 should have your new name, and dont forget to include proof of the legal name change

If it’s a certified copy, yes. A simple photocopy, no.

Most recent passport. Color photograph to the specs they require for photos (I have another video about those requirements) Legal evidence of changes you’re making. Proof of citizenship is sometimes also required (birth certificate, for example)

I have a question. I changed my last name from "Fester" to "Foster." I applied for a new passport within a year of applying. I included my certified court order name change document. However, the passport office made an error and sent me back my passport with the last name still being "Fester." I have the court order document of the name change still. 1. So should I fill out line 22 saying the passport office made a mistake? And then just include the court order name change document with it? It's still within one year of changing my name so everything is still valid. 2. Should I also fill out line 20 and 21 again as well?

Hey Kris, 1. Yes, fill out #22. Be sure to read the note at the bottom of the question box for more info about providing documentation. 2. The section for #20 and #21 are asking if your name has changed since you've gotten the passport you currently hold. If you're holding a passport with incorrect information because it was printed incorrectly but your name has remained the same since it was issued, then you should only fill out #22. If your name changed after the issuance date of the current passport, you should fill out #20 and #21.

You should send in your original and a copy. Keep a copy for yourself, too, to use until you get yours back.

I got my Passport in the mail today and I realized my birthday (month) was wrong. I still haven’t received my birth certificate in the mail either so I’m alittle worried if I might not be able to do this process. Do I just have to wait for my birth certificate to arrive in order for me to fill out the DS 5540 form or could I just send them back my passport and fill out the application?

You have to submit the appropriate documents along with the DS-5504. In this case you need to show your birthday on an official form, such as your birth certificate.
