Last week I had one of those situations where a patient couldn’t get off the bed by themselves due to their back “locking up”.
Ryan here had a acute back spasm episode from an aggravated lumbar spine and previous disc bulge herniation. His McKenzie extensions were fine and relieving, but being on his front meant his back went into protective muscle spasm.
Sound familIar? Does this happen to you or have you had a patient like this?
So I had to do a step by step guide to get him get from lying to standing. This was really helpful for him as he needed to do this at home for a few days until it fully settled AND he had to learn to do it by himself (unaided) in case he was by himself and this happened again.
It was so helpful we thought we would share this process with you all to help anyone out there who has a back that locks up.
Good news is he is now good again and back doing his core stability and strength work as well as the mobility drills.

Day 1. I was trimming some bushes in the backyard and felt a sharp twinge in my back and instantly went down. I crawled into my house through the living room back door, just barely inside, stayed between the back of the couch and the wall from 1 something p.m. till after 5 something p.m. Every adjustment, movement and reposition took forever to do, and some positions were very bad positions that I got stuck in and took forever to get unstuck. I dared to try to climb up the back of the couch to try to stand a couple of times to a horrible no avail. Later that night, made it around the couch to the front of the couch and spent the rest of the night in the floor. Heating pad, Icy Hot patches and back pain pills did not seem to have any effect whatsoever. Day 2. Spent most of the day in the floor but was able to get up with a walker chair on wheels meant for elderly people. Managed to get awkwardly slowly and painfully to the bathroom and didn't have to pee in a bottle anymore. Had a little bit more mobility in the floor to try to loosen up. Day 3. Very tired, still sore with no sleep lying on the floor with folded up blankets and pillows. From morning time until evening time, spent the day watching RU-vid videos on "getting up and down with lower back pain", or "getting in and out of bed with lower back pain", and targeted stretches to relieve lower back pain, from all different types of physical therapist RU-vid content creators. Finally able too slowly carefully cautiously timidly stand up and walk but still keeping the walker close by and still accidentally triggering sharp pains. Trying a lot of little things and taking a lot of breaths and rest in between gaining a little bit more mobility every hour. I was athletically active in my younger days. I was an infantry Marine in my twenties. I've always had a physically active but not physically breaking job. This is my first experience with true back pain and it was completely debilitating painful and torturous. Especially sneezing. I have made more strange breathing and grunting noises and growls and groans and sharp hissing sounds then I ever have before my entire life. I am glad these channels exist. Some things I would not have thought of and some things not to do I would have kept doing. Some things actually gave me measurable relief in a pretty short amount of time.

Awww I hear you right now is that I’m afraid of even move because it hurts so bad but I need to get up today is Sunday, September 8, 2024 I live in California and we’re cooking in California right now. I live on the third floor I need to get up get pillows to put them on my back, but I can’t move because I need to turn on my central air. I can’t close the doors I can’t. 😢😢😢😢😢

My back isn't in spasm right now, but watching this gives me sympathy spams! Horrible to be prone and try to get back up while in that much pain! Thanks for the techniques!

This is the worst pain Ive ever experienced. Id rather give birth or get bit by a gator.

Just found a way to prevent back muscle spasm when standing up from a seated position. Simply place each palms on your laps as near as possible to your hips and press against the laps as you stand up. In this manner it somehow deactivate back muscles from firing and pulling to strongly which in turn helps reducing or even stopping pain during your healing period. Try this to see if it works for you.

Poor bugger, I go through exact same thing, worked out the same routine out of necessity it seems. My only variation is when I drive to stand up position using legs I focus on force going through heels or back half of foot rather than toes or ball as it's more controllable for me and weight/momentum doesnt over overshoot and then have to settle back which can trigger spasms and then I'm back on my back side if not the floor writhing in pain. Great vid Tim, you really know the details!

helped move a sofa upstairs that was a struggle. regret it. cant even walk now. have to brush teeth on my knees. that nerve is letting me know it aint allowing me to do shit

This is a good one Tim. Good I managed to click on this. I immediately stand up and start walking and the pain is unbearable sometimes.

in addition to back problems, your deep thigh/hip adductors can stiffen and prevent you from standing up straight. its your thighs, not your back that is causing you to be bend over when you walk.

How did you cope on the flight? Did you take any medicines? I have a long flight coming too and with acute LBP Im nervous too. Hope you're doing better.

Excellent video very clearly demonstrated. I sympathize with your patient there as I am in the same boat. I can see him grimace through some pain. Its been 2 years I write this, is he doing better? At night my back goes into hell spasm giving pain level of 9/10 and the other night I was thinking I would have to crawl to the bathroom while biting a small rolled up towel. I succumbed and used 400mg ibuprofen which works really well for me but I dont like it as im aware of the risks of NSAID. Getting out of bed is a hell for me but after seeing this video I have great knowledge on what to do. Specially the core bit. its the getting up on elbow and hauling to the upright that im scared off most. Thanks again and I subbed!!

Currently having back spasm from injury training legs. The further issue is that I can get up because i have no strength in my legs or glutes because of the heavy squatting at the gym. Looks like it's going to be a few days on the sofa peeing in a bucket. trying painkillers and muscle relaxants to get me moving

3rd day after hurting my back I can’t stand up at all when I try to stand it feels like when I hurt it

Well, I can’t barely move. The pain hurts a lot just to move and I’m sitting on the couch but I need to get up. I had an ice pack on my back for 2 1/2 hours and I want to get up but I can’t because it’s spasms too much.😢😢😢
