How to Perform a Sewer Air Test and Line Acceptance Test

How would this work on a home with a 4" sewer lateral. What if the city sewer pipe is not reachable how could a city check the sewer laterals from each home to the sewer pipe ten feet under the ground. So the one pressure plug could be at the inside of the home but where would the other plug go if the tester could not access the end of that sewer lateral where it connects to the city's sewer.
