ABOUT DR. BREWER
Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on the run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.
At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. ABOUT THIS VIDEO:
How to test for cardiovascular inflammation: Microalbumin Creatinine Ratio(MACR). This is one of the 2 most important tests for cardiovascular inflammation. The other tests are: LP-Pla2, C-reactive Protein, and MPO (myeloperoxidase). MACR is a urine test. It shows cracks in the intima. The intima is the inner lining of the artery and capillary. It is also called endothelium. Brad Bale and Amy call it "the tennis court" in their book. There is about 6 tennis courts' worth of intima in our body.
Here's 1 key - if too much albumin is spilling through the kidney filters, there are likely cracks in the intima (endothelium).
Here's another key - if the intima is cracked in the kidney, it's cracked in all the other arteries and capillaries in the body.

Thanks. Most people don't understand that kidney disease is usually just arterial disease, like heart disease, stoke, senility, heart failure, and even erectile dysfunction. When they see the diagram of the kidney and glomerulus, it starts to become clearer.

my microalbumine/ crea ratio is 32.39 mg/g (reference interval < 22.12) is it stll reversible i have foamy urine and prediabetic

Dr.. I just had my lab work back and my Microalbumin Creatinine Range id 675. My general doctor sent me to a specialist but I am waiting for insurance to approve referral I am a bit scared. what dopes these results meanand what should I be doing meanwhile? doctor said he never seen them this high before.

Another very informative video - thank you. I had no idea I could just wander on down to Walgreens and buy these test strips (even if they are pretty expensive). I wonder why people would keep these on hand. If there are cracks in the Intima, this says you have cardiovascular inflammation. But, when you see anything below 3.5 (higher is better) or so, what does this mean? I mean, at what point in the breakdown of the arteries are we, when we reach this point? Is there a heart attack arriving within days, or does this suggest a change in lifestyle, and there are still a few years to worry? Or are cracks just a sign of inflammation, and something needs to be done? The blood vessels in the kidneys are just intima, the inner lining of the arteries, and no more? Creatinine is able to pass through these walls, but, unless there is a crack, Microalbumin cannot. A crack? A hole? What if there is plaque? Does plaque grow inside the kidneys? That might be what we call "kidney disease," I suppose.

+Robert Mertens Yes. To most of the questions. But the test strips only tell if you are spilling protein. The MACR is a ratio of protein (albumin) to creatinine. I have not seen a strip that measures or calculates the ratio.

I appreciate your video. I had a MACR test done not too long ago, and there were comments in the Forum on MACR testing for which you provided clarification on levels indicating kidney damage vs. potential CV issues. I also recently noted the levels that the Cleveland Heart Labs listed in the past. In this video did you mean to say that the healthy MACR level for women is a higher level than men instead of lower (5:46 in the video)? In the Forum you mentioned

The number for men is lower. It has varied between 4.5 and 7 mg/gm. For women, varies between 7.5 and 14 mg/gm. For all those that are driven crazy by variation, have fun with that.

Ford Brewer MD MPH Hi Doctor. I just had my lab result from last month. I find this video enlightening thank you! The result was CREATININE, RANDOM URINE was 238, Microalbumin is 0.8 and my micro/creat ratio is 0.3. Is that okay? Thanks in advance.

My microalbnumin/creatine is 22.5. The mcroalbumin is 14.3 and the creatinine is 63.6. This is alarming to me. Since then I have decided to try a whole food/plant based diet. Have you seen evidence that this could help? Is the resultant situation (plaque/inflammation) reversible?

My point is that I THINK my 18.5 mcg/mg is equivalent to 1.9 mg/g which would be in range. Can you confirm that? I will watch all the videos in your course again. I think I've seen some of them already but out of order. Ive been watching a lot of Ivor Cummins videos The reason I get the MicroA/CRT test is because I have only one kidney so does that make my score worth twice/half as much as someone with two kidneys? Is mine equivalent to 9.25 mcg/mg or 0.9 mg/g?

There was no video where someone named John talked as you mention in the recap. I already know how to get the blood labs through Request A Test, Ltd. if my Doc wont order them under my insurance. I already know i'm insulin resistant and eat low carb diet.
