How to use reflective practice as a doctor in NHS

Reflective practice is a very important skill that we must have in order to progress and learn the most out of our everyday experiences. It is also important for the appraisal and E-Portfolio as well. In simple words reflective practice is a form of learning.
As primates we are equipped naturally with the skill to learn from our experiences, and this ability is **called instinctive learning***. It is very primitive form and there is minimal contribution of cognition. After that comes the ***social and emotional learning** and this is what we learn in society and school. It involves learning to move on after making mistake, ability to share your emotions, handle disappointments etc. We use these basic forms of learning every day and unintentionally keep on improving.
Reflective learning is higher form of learning and involves intentional processing of the scenario and equips you with the tool to guide gauged actions. It is form of critical reasoning, coming up with better solutions and then applying it in future. It is a refined and a fore-brain driven response. Gibbs reflective cycle is a famous model of reflective learning;
1. *Description* **of the event**; what happened?
2. **Feelings**; what were your thoughts and feelings?
3. *Evaluation;* what was good or bad about the experience?
4. *Analysis:* what sense can you make out of the situation?
5. *Conclusion;* what else could you have done?
6. *Action Plan;* if arose again what would you do?
Simple form of reflective practice is “informal reflection”. Here you go through the experience and re-think about and consider your feelings and factual circumstances. After that you analyze and come with ideas to improve your skills for next time. A simple phone discussion with the friend after a difficult day is one of a very common example of our informal reflection.
To make it formal, we consider more systematic approach and then you can improve better and write it in your Portfolio.
Portfolio examples-
Unsuccessful procedure or even successful procedure (positive events improve our motivations, but as we get more learning from our mistakes- to enlist a mix of both is better)
Example # 1; Unsuccessful cannula/LP etc.
1. State the whole process/scenario/ be factual/ or just write timeline of the event
2. Your feelings; I felt guilty/ incompetent, when I asked for help/ felt judged
3. Analyze: Critically analyze the whole situation/see broader view- impact on your patient care/colleagues /yourself
4. Come up with some better solutions/ alternative options- even ask colleagues (need more observations with ppl with expertise/ get familiar with the equipment/ get second pair of hand/ assess if looks difficult ask for help appropriately)
5. Then have an action plan and use it in future. (My tip is to give yourself a timeline and review your progress after making those changes in your practice.)
Example # 2; Pharmacy errors;
Double check with BNF and if i am writing it first time i will confirm with my colleagues just in case
Example # 3 My nurses feel a bit distant/ mean to me? What should i change?
1. What exactly is the situation- see with fresh eyes
2. Facts; am new/struggle to understand jargon, take longer than others to finish task, new to social and cultural attributes
3. Analysis; Most of the staff are local- or have been in the department for long time , Know each other well , Share same background/ culture , I feel left out
4. What next?
5. Think about it (devise some strategies)- Try small changes
1. To engage in work and improve my typing skills, write on handover list, surf around the hospital system, politely speak for myself/esp what am I feeling/ be direct
2. Know names of all of my colleagues, talk to your supervisor, discuss with your friends in different hospitals, avoid negative judgments and always avoid confrontation
6. Set yourself a timed target and review your progress again after implementing those changes.
*Be kind to yourself and keep on going*
NEXT LEVEL
Reflective practice in action - this is a higher level, when you critically think while in the difficult/challenging situation and act very wisely.
