+Jasmine Hughery The checklist does not specify that you must clean from cleanest to dirtiest and you are using different areas of the washcloth to cleanse with each stroke using the leaves method, which decreases cross contamination (the whole point of clean to dirty). The area around the urethra really should be cleansed first to decrease the likelihood of introducing bacterial contamination into the urethra which could cause a UTI. This is the rationale for the method shown in the video. This method conforms to all current testing standards.

+Sean McNamara No. You cannot sign with CNA initials because you are not technically a CNA yet. Simply sign your name, first and last, as you would sign any signature. There are only 4 skills that require documentation: pulse, respirations, feeding and emptying drainage bag. All of these are simple "concrete" measurement documentations of numbers, not long narrative types of documentation. Usually, in a clinical setting, concrete numerical documentation of these measurements will not be signed. For the exam, you are signing the documentation not as a CNA, but as a testimony that this is the documentation you are recording for testing purposes (that way later you cannot come back and say "No, I told the evaluator that I got 14 on the respirations, not 20" since it is your reading in your handwriting and signed by you). The indirect care principles for Prometric testing states, "The only time the evaluator listens for the candidate to address a specific comfort is in the bathing-type skills (e.g., catheter care, foot care, hand and nail care, partial bedbath, perineal care) with the expectation that the candidate will ask the resident about comfort with the water temperature." So this means that the testing student MUST ask the patient to check the water for ALL of these skills, according to the clinical skills checklist. This is taken directly from Prometric testing standards. But you should NEVER roll a patient toward you - this is a safety risk because gravity will always try to pull them onto their back because their weight distribution is along their backside. If their back is on the other side of the bed from you, you will not be able to stop them from rolling backwards (and possibly off the bed) because they are rolling away from you. The clinical skills checklist lists the checkpoint for cleaning the labia first, but does not specify that the labia must be cleaned first (they had to pick one to list first but that does not indicate that one specifically must be done before another.
