Gian Zannoni, MD, Fondazione Policlinico Universitario Agostino Gemelli IRCCS, Rome, Italy, discusses the readiness of the FIGO 2023 classification for prime time in endometrial cancer. He highlights concerns about its universal applicability, particularly regarding the distinction between aggressive and non-aggressive isotypes and the role of lymphovascular space involvement in both categories. Prof. Zannoni emphasizes the challenge of access to molecular biology in various countries, creating a disparity between those with and without such resources. He argues that FIGO should be seen as a staging tool rather than a comprehensive risk stratification system, noting that risk stratification and staging are distinct processes. In his view, FIGO classification should be integrated into risk stratification rather than serving as the sole method.
