This is general information for overcoming bladder incontinence after prostate removal surgery through pelvic floor muscle training. Please follow the guidance of your medical professional after your surgery and consult with a pelvic floor physiotherapist for your post operative management.
This prostatectomy exercises video is an overview for initial pelvic floor training after surgery covering:
*When and best positions to start pelvic floor training
*How to do pelvic floor exercises for bladder control
*How to test your exercises
*How many exercises to do
1. When to Start Exercises and Best Training Positions
Most men start post operative pelvic floor training after their catheter has been removed and with their surgeon's approval. It's beneficial to start pelvic floor training before prostatectomy surgery.
Kegels positions:
*Lying down
*Semireclined
*Standing upright
How to do Pelvic Floor Exercises for Prostatectomy Incontinence
The pelvic floor muscles trained for bladder control sit at the base of the penis (bulbospongiosis) and deeper inside around the urine tube (striated urethral sphincter). There are 2 parts to Kegels after prostatectomy. The correct technique involves shortening the penis (retracting) and contracting as if to stop the flow of urine.
-Start by relaxing your breathing, your buttocks and your belly. Breathe normally and keep your upper belly relaxed
-Relax your pelvic floor muscles to resting after each exercise
Test Your Kegel Exercises
Test whether you're doing your pelvic exercises correctly by:
1. Stopping or slowing the flow of urine - only once a week as a test not an exercise
2. Watching in the mirror to see your penis move slightly inwards towards your body
How Many Exercises to Start With
*Start with 2-3 second exercises
*Repeat these exercises gently 5-10 times
*At first try to do this routine 2-3 times a day.
To Progress your Exercises
Over the next 2-3 weeks progress your exercises by:
*Holding each exercise up to 10 seconds, breathing normally and keeping your upper belly relaxed
*Repeating these exercises gently up to 10 times in a row using correct Kegel technique
Try to do this Kegels routine 3-6 times a day to improve bladder control after prostate surgery.

Having recent prostatectomy done within the last few weeks of this post, I am obviously still in the recovery stage. Meaning, no lifting of anything above 10 lbs, no strenuous pulling ect.ect. prior to the surgery, I was doing "brisk walks" on hilly terrain for at least 3 miles a day. Now, I am only walking roughly 1-1/2 miles on mainly flat, or moderate hilly terrain. Will this help to strengthen my pelvic muscles sorta like the kegel exercises?

I've done these prior to my prostatectomy 2 weeks ago. Catheter removed a couple days ago, and minimal to no leakage. I'm continuing the kegals. Very happy so far.!

I would like to shsre an accidental epiphany that might help other people. Recently I was looking to get my exercise walks in but the weather was bad so I decided to climb the stairs up and down. The effect was nothing less than miraculous. I have much better control than I had. It is not perfect but it is getting much better. I have been doing all the exercises that a physical therapist prescribed with little to no effect. I know walk my stairs 20×3 times a day. I walk the steps slowly so as to work the muscles. I don't know if it will help everyone but it sure has helped me

Yes, walking stairs does also flex your pelvic floor. Ideally you'd want stairs that are not too steep. Most stairs don't meet the criteria, and you don't want to over-do it.

A strong piece of advice for any man doing Kegals before and after a prostectomy, don't overwork those muscles in hoping for quicker results. Overworking them can possibly make the incontinence worse and in some cases, permanently. I found out the hard way.

Thanks Michelle. It was helpful how you describe the timing of breathing and muscle contractions. The doctor's notes weren't very clear about that so I wasn't sure if I was doing them correctly.

I had a radical prostatectomy nearly a year ago, and another surgery 12/31/19 to correct blockage in the urethra where it connects to the bladder. Had to be catheterized 4 times during a period of 8 weeks, with catheter in 4 weeks in total but not consecutive weeks. Been doing Kegels since January, but still have stress incontinence 10 months later. I'm dry all night and have been for months. I was not doing the exercises correctly until I found your videos in late April (my surgeon just handed me a thin pamphlet that didn't explain things very well and had no one show me how to do them properly). Have improved since then watching all your videos, albeit VERY slowly. I am very physically active and leak whenever I do yardwork, workout at gym, play sports, etc. Still using 2-3 Depends per day. I am confident that my technique is correct, and my breathing is proper. I currently do alternating sets of 10 fast and 10 long hold (10-seconds) standing, sitting and lying down for a total of 120 Kegels per day. Should I continue the alternating positions, or would doing all standing be better? Or should I locate a pelvic floor physiotherapist and follow an individualized plan? My surgeon wants to refer me to another surgeon in his practice for the bladder sling surgery? Any advice would be greatly appreciated. Thank you!

I had Radical Prostatectomy 2 weeks ago and catheter removed 1 week ago. Was wondering what were some of the signs of your urethral blockage. I'm still having painful,burning urination and the color and consistency is dark and thick.

I am happy that I found you on RU-vid the doctor did not explain on exercise excuse my language with the doctor said just like holding a fart I have never really tried to do that one question I am having problems I end up holding my breath it then I try to breathe I have to watch your video quite a few times I follow your instructions and then I put it on pause while I do the exercises November I will be 65 all this time I have seen quite a few doctors on my diabetes they never sent me to be checked for cancer recently I got a good doctor that did colonoscopy and prostate thank you very much

Dear Michelle: getting stronger -- in many dimensions- with your great videos. It's been a harsh ride with prostate cancer and recent prostectomy. #1 Cancer is luckily isolated to my former body part -- Yes cancer screening and early detection are key!!!! #2: Early days on the journey towards bladder control. Thanks for these pelvic floor exercise videos, Michelle. I first learned of your videos after doing a search on pelvic floor exercises back in October 2021. My question for you is: What should be my pelvic floor routine, relative to your two (2) videos that cover “AFTER Prostate Surgery”, a prostatectomy? I had a HoLEP procedure done on May 24, 2022 for BPH / oversize prostate, which was 70-90cc - xtra large. Cheers!

The most recent research that we have available for effective pelvic floor training post prostatectomy is in this video ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-B3APjwkqi3Q.html It's recommended these exercises are started prior to surgery and then recommenced (standing) when the catheter has been removed 6X10 long holds and 6x10 short holds so a total of 120 exercises daily which is quite intensive training but has been shown effective over less intense training regimes. Hi Michelle. I am scheduled for surgery this month and have been doing the exercises for the past 2 weeks in preparation. Post surgery, will I need to do these exercises for the rest of my life or just whilst the muscles strengthen?

Cooper you should do them ling term. You’ll find it challenging initially post op but stick with it - usually improves markedly in the first 3 months if you can do your exercises daily plus try to increase the number & length of holds up to 45 secs when you can. All the best for your surgery!

I am confused about something. After the prostate is removed, do we have to do pelvic floor muscle activation all the time to stop urine leakage? Or does the bladder only push urine occasionally or when full?

1st and 2nd day after removing my catheter and I have zero control. I didnt think it would be this bad. But for some reason when I am sleeping or lying flat I have control and I can wake up and pee normally? I am working on my exercises now.

That's how it is for me too. I also did not think it would be this bad. I would not say I am peeing normally but better when I wake up in the morning. Four days out from Catheter removal. I have improved but still dealing with Incontinence.

I have undergone prostatectomy in November 21. Now I have been tested PSA as 0.44 ng/ml . Is it OK or should be minimal. What exercise can I do to recover fully. Pl suggest.

@Mukesh Kapoor I think it’s very important that you discuss all your PSA results with your doctor. Exercises unfortunately can’t fix PSA however I always recommend that everyone perform appropriate general strength & fitness exercises for optimal wellbeing, all the best!🙏

I am not post surgery but have trouble holding back when I need to pee. I get strong urge and can't hold it back. Very frustrating. Will these exercises work for me as well, or what can you recommend?

Hi Shabiir try to do the exercises every day and at least 3 times daily if you can - usually when your muscles are rested for exa,maple when you wake up and more towards the start of the day rather than the evening. All the best I hope this helps

Michelle is it OK to have the Botox muscles contract while pelvic floor muscles are being contracted? Next question he’s non alcohol beer a bladder irritant?

Hi Dennis all carbonated drinks are potential bladder irritants but non alcohol probably less of an irritant. Not quite sure what you're asking in relation to Botox muscles, can you reword?

I love your video style and content. And I think I isolated the muscle that controls the "turtle movement". But it is confusing what muscle "stops the urine flow" . Is it the same one as the muscle thats "stops passing gas" ? Is it the same one I feel around my anus or to "hold my bowel movement" ?

Same pelvic floor muscles. But stopping uring flow is for the anterior/front portion of pfm, whilst stopping gas is for the rear/anus portion of pfm

I have undergone HOLEP for prostrate 2.75 years ago.right after my catheter removal i am dripping.I am advised to do keigel exercise and doing it 3times each 50 counts.very slow improvement.still dripping.night times I have control day when iam active it is not in control.stand up iam finished.now started using penis clamp(3 days gone i feel better now. I want to know is there any permanent solution.

It’s been 3 months and he hasn’t moved even taking teldalifil 5 mg. I’ve had no erectioms or movement since the surgery. I got sick for a couple weeks after starting the Kegel. I have anxiety on top of having a new girlfriend. Can you have an erection if you are leaking or does it have to stop. I’ve tried masturbating but it doesn’t seem to do anything. I’m worried I can’t have sex again and I’m so ready???
