It's basically the bass, drums and guitar. They honestly could be yelling random noises and it would sound about the same. So long as they are autotune machine pushes his voice into pitch with the same 3 chords (G F C ) that just about every Crue tune is in.

The rest of the band never hears Vince live, they have click tracks and their own instruments or in Nikki's case a bass track to sync along with playing in their earbuds, occasionally the sound engineers voice and hi-hat click track counting down into a song and an earplug in the other ear. If they could hear Vince they would probably be distracted and incoherently bumbling as he is, but the bass would keep playing even if Nikki's fingers are flipping the bird.

He has definitely let himself go (understatement), however, I am sure he does not care what the collective people looking in from the outside think of him. Even if he was not obese it is hard to sing this way when you are older. I am older now and there is no way you can run around and sing. Axl Rose stands still singing and he is awful as well.
