Little confused about this video's title and intro. Lactic acid is not what causes muscle soreness, is what I'm finding online. Apparently lactate actually helps to reduce pH levels, which were already increased by some other process when* you work out. You are correct. This guy knows his biochemistry, but this is outdated. Hydrogen ions build up anaerobic exercise and cause the temporary burn, but not muscle soreness. Chemically speaking, lactate could act as a buffer by accepting hydrogen ions. However, the lactic acid myth was perpetuated because lactate and hydrogen ions were found in blood during studies, so they inferred lactic acid was an intermediate and short lived structure. This makes is doubtful that it does act as a buffer. Anyway, chemistry is not my area of expertise. And like you, I get a little irritated when I see myths perpetuated by otherwise reliable sources.

Also, the liver does not throw lactate in the trash, but turns it back into glucose (gluconeogenesis). Furthermore, adjacent type I muscle fibers may take up lactate and use it as fuel.

Found your channel by searching for acidemia. It's fun to watch your presentations. You should definitely make a video about C0VlD long haulers. For some reasons the average doctors can't find what's going on. They even skip the ABG test. Anaerobic ATP production is in progress, but they can't see it.

Loved your video! Great review of a very important topic, I didn't know about D-lactate in short bowel syndrome. Thanks for your videos, they have really helped me close gaps and improve as a physician. I especially love your coagulation series! I do have a question however: Over the last years, there has been increasing evidence and discussion against the use of bicarbonate for the treatment of lactic acidosis, even in very low pH. The rationale is that you only fix the extracellular pH but don't correct the intracellular or mitochondrial pH (and that's where it counts!). Bicarbonate gets metabolized to lactate in the liver, which kind of beats the purpose of giving it. There is also evidence of improved cardiac contractility in acidic pH, which might be beneficial for a patient in shock. Not to mention bicarbonate's many potential complications like potential cerebral edema. Would you still give NaHCO3 in this condition? What about tromethamine or earlier, more aggressive dialysis?

So if I would have raised my ph with alkaline drops, I would not have been so sore that I couldn’t walk after just doing a few exercises? I take propranolol for my heart. Would that cause problems?

Not sure you monitor this any longer, but could lactic acidosis cause chronic cramping of many many muscle groups constantly throughout the day? Trying to chase down why this happens, and is often initiated by sugar or grain intake. But is a slow burn for happening with any muscle usage at any given moment. Asking about lactic acidosis due to an elevated Anion Gap. Thank you! -Karen

Hi medicosis is it the lactic acid or the hydrogen ion that causes the burn when we exercise? i think the lactate gives you a few atp's and the charged hydrogen ion causes the burn. Lactic acid does not cause the burn as such. :) Love your videos.

I have seen 3 videos telling what you are telling. But none is it mentioned what is causing the pain. I did once 3 rounds in rowingmachine in high intence training where after my 3rd round I could not move in over 20 minutes. That day I was told it was because of lactic acid but you could than give me another explenation. Please do tell me what it was if not lactic acid causing my pain and being unable to move.

How can measuring the presence of both be a useful marker of any medical condition if lactic acid and lactate do two completely different things? If you know that either one of them is high, but not which, you can't draw any conclusions if the symptoms that manifest from either don't look the same. Are you just gonna say "well I see you have high lactate/lactic acid, and I also see that you have muscle pain, so I'm going to assume those are related". That's not scientifically sound.

Correlation is not the same as causation. However, correlation is not nothing. If a patient comes in with fever and a new heart murmur...Are they related? May be they are not, or may be they are (e.g. bacterial endocarditis). The purpose of an individual physicial sign or lab result is not necessarily to CONFIRM the diagnosis with 100% accuracy...It's to add more clues and pieces until we can solve the puzzle. Moreover, if the patient has muscle pain, we won't rely solely on the lactate (or lactatic acid). There is the history, physical exam, other labs (e.g. creatinine, creatinine kinase, creatinie phosphokinase, myoglobin, EMG), imaging (e.g. MRI), and muscle biopsy. Hope it helps!

Wrong!!! Lactic Acid or Lactate accumulation does not cause muscle fatigue during exercise it actually serves as a fuel source during strenuous bouts of exercise. Hydrogen Ions can contribute a small amount towards muscle fatigue but actually, lactate accumulation is helping not hindering

The 1st one I mentioned is not gonna give you anything anything but amazingness And why because God put that here on Earth so do you ever wonder why the names are quite almost alike and there's no research not enough research done on the number one that I mentioned but isn't research the ongoing documents and research and people's testimonies but do you hawell testimonies but you ever find it odd though that the FDA won't do anything about that no it's because they want you to buy the 1 that they make so you can eventually get cancer
