Urolift is a minimally invasive procedure for the management of BPH. It may not be right for all men. Certain factors such as a large prostate (over 100 grams) or presence of a large median lobe of the prostate may limit the effectiveness of the procedure.

Excellent synopsis doc, especially of the limitations of the procedure. These are very important points that you made and not applicable to everyone. I for one, and considering the procedure and I was wondering why I would not be able to have it done. Thanks again doc and great job!

Very helpful presentation. Also, I appreciate the comments from all the patients below with many indicating unsatisfactory or short-lived results. I'm going to look into the recent literature on complications and patient satisfaction.

I had the procedure 18 months ago. Before the procedure, I had bouts with acute prostatitis, occasional Foley catheters, intermittent catheters, acute renal failure due to an obstructed urethra, and medication. Since having the procedure, in conjunction with a bladder neck incision, I haven’t had any serious issues. I still have some days where my flow isn’t as strong as it was as some nights where I get up more often than usual, but I’ll take the good with the bad to stay off of meds and to avoid any sexual side effects.

Well, I just finished by second Urolift a few days ago. It was in office under valium and local anesthetic. Because it was a redo, a company rep from Urolift was present to observe. IT HURT LIKE HECK! That was on Tuesday. Today's Saturday and I still have significant blood in my urine and it still hurts a bit when I pee. The flow is a bit better, but not what I thought it should be by now. I assure you, this was my last Urolift. I read somewhere that the FDA shows Urolift has a 14% failure rate within 5 years. When I mentioned this to the company rep, she became defensive and corrected me indicating it was actually 13.5%. If this one fails at any point, they'll be hearing from my lawyer.

I had one done in Feb 13 2022 and today Jan 21 2023 my urgency to urinate is every 15 to 20 minutes .I can not stand it.

@@georgeguerrero7943 Sorry to hear that George. Has this been going on for the entire year? What does your urologist say about it? My situation is nowhere near as severe as yours, nevertheless, lately I've noticed my stream getting weaker and I drip a lot longer.

Two failed Urolift procedures. Stress, long walks, and ladder climbing increase pain, bleeding and passing bloody clots. Changing the size of the pipe--is a stupid response to a faulty bladder level sensor. The cramping and intense need to pee when I have 200 ml of fluid in my bladder was the original problem.

I feel your pain, Stan. My second Urolift has failed. I'm lucky to pee 2oz at a time with considerable discomfort during the day. At night, it's the opposite problem. Last night I was up four times in five hours and peed over 44oz (14,12,10,8). I'd like to find an attorney who'd consider a class action on behalf of us. My husband had the urolift procedure done in 2017. Now encrustation occurred when minerals from the urine deposit on the surface of the indwelling urolift implant. On 3/30/22, he had a cystolitholapaxy procedure to remove the "stone" encrusted on one of the implants. At the follow up visit, the urologist told us that only 50% of the "stone" was removed and my husband have to undergo another cystolitholapaxy procedure to remove the remaining "stone". My husband has low pain threshold, it was exhausting to take care of him postoperatively. He took Norco before bedtime, due to the constant irritation after the procedure. Then the narcotic induced sleep apnea happened, his pulse ox dropped to 88% at one point. It was a sleepless night for me. In my opinion , I do not recommend urolift procedure, it is a good quick fix but does it worth to go through the potential pain and sufferings in the future?

You must be an angel to do that. Did you find out anything about how frequent your husband's specific issue was? Who's to say that he would not have had worse pain, or you had more care taking to do than you already did? Thanks for posting your experience.

I had the Urolift procedure 4 years ago and have similar problems to your husband. I also got a hernia right after the procedure that required surgery. When I contacted Urolift they showed no interest. Statistically my operation is probably recorded as a success. After 9 months of suffering I had PEA for my BHP which was a success, but the Urolift implants still create problems.

same with me. My new urologist is going to perform a surgery to remove a pin of a failed urolift forming a calcium stone. stay away from this procedure.

My husband just had Urolift, he’s been in excruciating pain, blood clots. Went to ER was in the hospital 4 days, nurses irrigating catheter. Then after he was discharged he had to go back to the urologist who did procedure, he’s at office now!

Yes I had the procedure 3 years ago and It was one of the WORST decision I have ever made. I regret it. I am now suffering the consequences because the #Urolift procedure has made my condition worse than it was before.

I've had two UROLIFT surgeries. The second one was supposed to have corrected what the first one was unable to do. The second surgery made my condition worse. I had to stay in the hospital two nights and three days. I had one more painful experience. I would not recommend this surgery to anyone. Now I have urgency incontinence and pain in lower abdominal.

@@buddyjames8451 agree 100% It may provide temporary relief for some but my procedure made it worse and it was awful and miserable and still have issues today because of the procedure. My new urologist wants to try the water / steam method treatment. Forget the medical name for it.

My doctor only did a cystoscope and not an US or MRI. I would highly recommend anybody thinking about doing this procedure be adamant with their doctor about getting an ultrasound or MRI first to get a better picture of the size of your prostate.

Thank G-d this Doctor advised on his video that the Median lobe can be done with the Urolift 2 machine.This changed everything and he is the first Doc that apologized for saying Previously with a median lobe can't be done.So he is my hero.

#Urolift completely and permanently destroyed my life !!!! Now I am faced with my new urologist wanting to do another surgery (greenlight) to correct to the botched UROLIFT procedure. I am told I would have to self catheterize everyday possible for the rest of my life. My symptoms now are WORSE than before Urolift. Please be advised and don't say I didn't warn you if you get this procedure !!!!!

Urolift lasted for me for only two years. I was hoping it would be five or even ten. Urolift also refused to talk to me to discuss my problem. Does anybody have ideas as to how to extend the lifespan of my urolift implant?

I have not done the Uro Lift procedure yet cause I do not feel good about the.possible problems. Glad I saw your video. I saw an article about a arterial lol procedure that blocked blood from going to the prostrate which in turn reduce the size of the prostate - side effects?

Who should avoid Urolift? EVERYONE! I had it, what a disaster! I underwent many tests ahead of time to ensure I met all the criteria. I was told by my urologist I was a perfect candidate; that it would definitely provide significant improvement. The only results I’ve had are significant pain and some kind of damage to my urethra. When I am able to pee, it sprays everywhere except into the toilet. I wish I never had heard of Urolift!

Can you determine the situation with the median lobe by ultrasound? or do you need a camera up there to determine if the median lobe is into the bladder or too big. Also - what about over time if this stops working or becomes ineffective ... does it affect the next steps or remove some other operations from consideration? Can Urolift be done twice?

Hi Daniel, I have a urolift in my prostate that is relatively small, about 43cc. I improved a lot but since they put it on I started to urinate more frequently, because my bladder fills with only 140 miliiters of urine and I can't stand the urge to go to the bathroom. You can help me, thanks in advance.

Thanks, Doc, for a great video. I had the procedure done 10 days ago. I had complications in not being able to urinate afterward and had to have a catheter re-installed. My prostate is 2/3 normal size at 19 grams. I'm wondering if that contributed to the complication. I spoke to my Urologist over the phone, but have not visited with him yet for a follow-up. Doing fine now. But I wonder if you have seen this before? If the "stitch" is too long would that not just allow the prostate to return to its original configuration?

Had procedure done in January. I’ve had no problem since procedure. 3 days of a little discomfort. Complete 48 days of radiation clips where also used as treatment markers. Pee Like a firehouse

Too early for you to tell. Had my Urolift done in May 2020. Lasted about 18 months. Went to different urologist and was told the urolift didn't work for my kind of prostate. Had 6 Uti since. 3 in just a 4 month period in 2022-23. Funny my symptoms did get better from may 2022 to jan 2022. Then got worse. Looking for other alternitives.

Dr. RezniceK: Question: Would you recommend the Urolift procedure for a man who is normally very active? For example, an active old guy in his mid sixties who is of a healthy body weight (BMI) and who jogs, bikes, does a little martial arts (which includes jumping and kicking), and who also does animal flow exercises which includes a lot of squats. I'd hate to think of the device working loose. Ouch! If not, what procedure would you recommend? Thanks for your good videos. Keep posting, doctor.

No answer for these questions re to 19 gauge wires with second 'spring role' inside very sensitive body part. My second question was ' how can you pull it out if spmething wrong happen, and s.o. for reasonable new technik? No answers...

I haven't (yet) seen or heard of anyone breaking an implant. The prostate is deep in the pelvis and very much fixed in place. It is hard to get shearing forces or an impact that moves the prostate on one side relative to the other side. For example, if you got kicked in the perineum, the whole prostate may move up and down but the implant isn't stretching or moving apart.

I was extremely pleased with the Urolift procedure, but after only 3 months my BPH symptoms were as before or slightly worse. Could this be due to the urologist’s not evaluating the prostate adequately before judging it suitable for the procedure or possibly not properly positioning the sutures? If a reevaluation deemed me a good candidate and if the procedure can be repeated, I would consider having it done again.

Daniel, thanks for the video, great stuff!! I am on blood thinners, as long as I am off them prior to the procedure, you feel that may be OK? I have always had my cardiologist get me off them about 4-5 days prior to any procedure/surgery.

I had this procedure done Dec 2023 after i had seen the urolologjst for being in urinary retention. Although i had requested aquablation, the urologist talked me into urolift. Worst decision. The procedure was painful and should always be done with general anesthesia. But most importantly it just didnt work. After the catheter was removed, i still couldnt void - so the dr showed me how to self cath. I then got a second and third opinion to include cysto and both dr advised i was completely obstructed. I recently had aquablation and nearly 90% back to normal. Not sure why some dr are pushing this procedure, but i would not recommend it. Best of luck!!

I have been on finasteride for a few years now. It has relieved my urinary issues and, I assume, the perineal pain I was having. I would like to get off the drug as it seems to be affecting my "libido" or perhaps being 72 is the cause. If I get the urolift procedure, will my prostate enlarge to the point where I have perineal pain again?

Thank you for the information. I'm interested in long term. How long has this been around. Do you have to do it again. Do you end up having the rotor rooter anyways. Does that foreign object cause increased infections or mucus plugs. I was surprized how the body reacts to a catheter. I have been dealing w/ a blocked cathater, 4 UTI's w/ 4 courses of antibiotic treatments.

I have had s the urolift procedure..I have an obstructive median lobe.......the procedure failed.....bph has returned....had cyctoscopy performed .....recommendation was turp.......Question:.....what happens to the urolift wires and anchors during turp?.....is this a complication? and thank you!! great info..

I would like an answer to this as well. During my Urolift procedure one of the needles hit a bone during the procedure. I screamed out, WOW that hit a bone! No reply from the DR. When we finished up, I asked him again about it he just said the procedure went well. It's now been 2 years since I've had the procedure. It has never worked like advertised. For a week after the procedure I felt urgency to void but nothing would come out. Dr. said this is normal and would get better soon. The only relief I could get was sitting in a bathtub of warm water. I usually sit to piss to keep it from going everywhere. I have pain in the area where the needle hit the bone during installation. It radiates out to my hip. Reading the replies on here I now have another thing that concerns me relative to stones in the bladder. I wish I never had this procedure performed!

I had a Urolift procedure done in Houston in February 2021 and I have gotten no relief. I am still taking Flomax to ease urination. I asked my doctor and he gave me additional medication for overactive bladder, infection, etc. to no avail. He never suspected if the procedure may have been faulty. finally, I asked him to use the scope and see if my prostate is open and there is no further abstraction. Upon using the scope he explained that there are tissues hanging in the middle that blocks the urine to flow easily and he suggests going in and removing the extra tissues but I have to wear a catheter for few days. He also added that this is very unusual. Frankly, I didn't trust him, his explanation was at least questionable. I know Urolift is approved by FDA but I suggest the doctors and the FDA need to go back and revisit their analysis.

@@WildnUnruly No, I have become completely disappointed with any and all procedures. I spoke to another Urologist who told me not to waste my time and money on these fancy procedures and do Turp which is the only effective way.

@@esfanintan Are you talking about the Green light laser pulverization of the prostate? Because once the exes of the prostate tissus are removed, it takes a very long time to re grow and most of the time it doesn't.

My urologist at the VA told me yesterday that he has had to remove many Urolifts and that there is about a 50% failure rate. He says the CT showed a normal sized prostate, but I have a curve in the urethra he has to ream out. I am scheduled to have surgery in March 23 and I am going to get a second opinion.

This was a good video , but I have a gripe with the commercialization of this procedure. While searching for reviews the youtube world only seems to contain testimonials and patient reviews from clinics were the patients state no catheters needed , very little pain and after a couple days later they were back at work peeing like a racehorse. I have gotten the procedure 2 weeks ago and am hopeful it will work , and people reviews to these videos seem so state that is very successful in the end but that actually the pain free experience seems very rare. As for my experience so far the actual operation went well under General Anesthesia . The doctor put a catheter in just in case (he stated he always does this as a precaution) and told me to keep it in for one day. It was a terrible day , my Bladder kept having spasms. After 1 day I removed the Catheter to about 15 seconds of blood. At first I had to urinate about every 10 minutes with very little coming out and was still having painful Bladder spasms ( it was very painful even taking AZO). After about five days the urine stream started improving and I would only have to go about every hour (day and night) and while it still burned it was now much more tolerable. One very bad problem I had was that I got severe constipation even though I took Miralax and a stool softener. This was made much worse by the fact that every time I tried to push a little to poop my bladder would severely spasm. Even though the constipation is finally easing up (day 8) I am still having some urination problems where after I finish peeing I will have several bladder spasms about 30 seconds later where a little more urine will come out. I still get up about 3-4 times a night. My doctor says this is to be expected and give it a couple of WEEKS to subside. As I said if this works I am okay with it but I still think these testimonials should also include some realistic people that have had side effects. I am keeping my fingers and other things crossed.

Hi James, I think your right the side effects of this procedure should ease off over time but everyone is effected differently . I have just been told I have bph and the doctor has asked me to start taking the prescription .

Hello Dr. I just came across this video. I am experiencing BPH . I just had mine looked at through one of the procedures you mentioned. My urologist says I am 28 grams but want say about how big it is . He is pushing the urolift. . I don’t want it . Now after watching your video. My question is what happens in the future prostate want to get a little bigger with a urolift. Would that pull the outside clips into prostate. ?

I was told the Urolift was good for 5 years when the prostate grows again and blocks the urethra. I also was scheduled for 8/9/2022, and was told by my Internist, to reconsider given the issue, "if you can still urinate downhill, do not bother with this procedure" so that says it all.

Thank you for your video. I learned much about urolift and I'm intersted in the procedure. I want to preserve my ejaculatory functino and my prostate size is within range. Would the UROLIFT be compatible with an avid male bicyclist who rides 30-60 miles of bicycling 4-6X per week ?? Is bicycling a contraindication for the UROLIFT procedure and hardware ??

Urolift only lasted one year. I suspect my prostate was too large and one of the clips did not fully extend. Also, I believe I had a medium lobe that extended into the bladder. I initially wanted the urolift to avoid the more complicated Laser Turp Surgery. For me it was a bad choice. Medically, the urolift was easier (done in the doctors office) with a fairly quick recovery. Initial results were good but nowhere near the relief after I had the Green Lite Turp Surgery. The surgery was extensive and recovery was long and difficult but ultimately it made a HUGH difference in urinary flow.

@@mytaylor432 They removed the clips during the green light surgery. I had the catheter in for a week to 10 days. It was a relief to get the catheter out but healing continued for another week or so.

Good. Some questions: How many procedures should a surgeon have done to be considered an expert? Has painful ejaculation been reported as an adverse outcome? Is there a risk that the prostate may become distorted and out of shape as it is squeezed by these retractors, thereby in fact aggravating the problem it is intended to ameliorate? Thank you.

Getting surgery in three days. the doctor did not give a cat scan or MRI I have a catheter in for a month. I am 80 years old. My doctor did a scope and that t was it. Before the cather I was peeing a lot and incontinence Plus retention of the bladder, but it was comfortable a few Depends should I have the surgery I may be worse off after the procedure .

I just had the procedure done almost a month ago . Still need a catheter . Every time they take it out . Having hard time urinating . Just recently found out that I have an infection . I’m having second thoughts on having this done . I was able to urinate before urolift . Now it’s a struggle to urinate . Urgency , pain and discomfort . Now I am seeing new urologist for second opinion . I believe I’m in good hands now . To who ever considering this . Do your research and read reviews . Not saying it’s not going to work but be mindful . It’s going on almost a month and I’m very concerned . Dr that did my surgery basically told me that everything is clear and there’s no blockage. My question is . Why can’t I urinate ? I should be able to pee like a race horse . I’m on antibiotics now so maybe that might help . New doctor is going to take care of me . Should have just had turp . I’m 46 and been having urgency problems for over 10 years now . Has anyone else had this procedure. Who could relate ?

Very similar to my situation. Had the Urolift done 2 weeks ago and I still need the catheter. Was able to urinate before. They say it's still healing. Good luck.

@@RonHartmanNet Hey Ron it’s almost a month and still having hard time urinating . Now I’m seeing new urologist . Thinking about having Turp done . He looked inside. Still swollen. I’ve have catheter in and out for about a month . Catheter back in . Scheduled Turp in mid February

@@danielandino3193 Hi Daniel, good luck with the Turp. I had looked into that but the Urolift sounded much less invasive. But they really downplay problems with it. I see my doctor in 2 weeks, and if I'm not doing better, will need to look into other options.

I vacillated before deciding to have the procedure--the more so given the fact I am 81 and had previously had radiation treatments for prostate cancer. At this point, two months after having the procedure, I am beginning to regret doing so. I still get up to urinate several times at night and I also have something new to contend with--greater urgency in the need to pee (so that I have to watch how much I've had to drink before I have meetings scheduled during the day). Perhaps that need will calm down more, but overall I am certainly not a urolift success story. I'd suggest that if you are are older guy in particular, think twice about having the procedure.

Well, almost correct, Doctor. The FDA did not approve the Urolift PROCEDURE, but only the Urolift DEVICE. The device approval was based upon the L.I.F.T. study of 140 operated patients and 66 control patients. This is a small number of cases, and I would have expected reference to a much larger database of outcomes of the treatment, particularly since the device was approved on the basis of a limited number of cases 12 years ago. Can you provide longer term followup in a getaer number of cases? T. Holohan M.D., FACP

I have BPH. The tradition symptom of restricted urine flow is there. But also a knotted pain. It can be very painful. My urologist believes this is also a symptom of BPH. My question is will the Urolift relieve the pain?

I'm not sure what "knotted" means. But some of us just have highly sensitive prostates, so sticking needles in it was, in retrospect, a stupid idea. Now the pain is SHARP on ejac. :-(

Doc - what about use with a prostate that was radiated for PCa? My BPH symptoms have been slowly coming back since I finished ADT and testosterone has been recovering. PSA remains undetectable (has been for 2 years.)

We have done a number of patients post radiation. Urolift is a much safer option than a transurethral resection of the prostate after radiation and so far have had good results.

@@pnus - thanks much for the response. It reassures me on having this done. Just back from a Urodynamics Study where the nurse couldn't get a catheter past the prostate (she tried 3 different diameters). The study was successful in identifying where the restriction is - hopefully a Urolift might be of help. At least I can still urinate, even if it takes a while. Thanks again!

I've had urine retention a few times. Seems the prostrate flairs up with infection and swells pressing on the bladder so when u go bursting to go u don't fully empty and have to go again and again.. If it stops as has done with me first time hospital to be catheterization drain it off and antibiotics.. I was prescribed ciprofluxian. 10 days 500 mg... second time I self did it as was given them at the hospital very easy to do... I just phoned up surgery doctor who prescribed Trimethropin 1 month course it cleared it. I was self catheterization for about 4 days before it started the prostrate to reduce with infection.. I'm. Now awaiting laser holep surgery very soon
