Urolift vs TURP: minimally invasive surgery takes on gold standard treatment for BPH

Daniel Reznicek, MD explains the differences between a TURP (transurethral resection of the prostate, also known as a "roto-rooter") and a Urolift procedure. Both are treatments for men’s lower urinary tract symptoms due to benign prostatic hyperplasia, but are very different procedures and have different risks and benefits. The Urolift was FDA approved in 2013 and is a minimally invasive procedure to help open the urethra with the use of very tiny implants in the prostate. Rather than destroy tissue, it compresses the tissues and has a lower side effect profile than the TURP. In 2018, the Urolift was added as an option that should be considered for the treatment of BPH in the American Urological Association Guidelines. Trials have shown good efficacy and a low side effect profile. The treatment is not for everyone, it works well for some anatomies and not for others.

Had the urolift done in 2014, a year after it came out. Eight years later had the TURP procedure done reversing the urolift. Found out one of my clips was embedded into my bladder. Those eight years I had constant UTI’s and ended up with a bladder infection until my urologist took out that clip and saved my life. The reason the urolift doesn’t work for some patients is the urologist is not skilled enough to install it. Make sure you have a good urologist.

Had a Urolift 3 years ago. If I had known what I would go through, never would I have agreed. It has done nothing but make my ability to urinate and properly empty my bladder much much worse. Along with worry and stress to the extreme over the time since I have had it done. I now am very reticent to go back to a Urologist so they can have another go. I fear every time they touch me, it will get worse. Still have discomfort and strange pains.

I too had a failed Urolift procedure. Changed to new Urologist who said Urolift’s are a joke. New Dr performed the Turp, (yes, a few more recovery days) and I’m much better!

My urologist did a Urolift and it shifted after a few months. I just had the same urologist do a TURP Hopefully it will work! I expect to be very tired for the first two weeks.

My Dr. told me my prostate is too big for Uro lift. Looks much more pleasant. I guess I will go through TU RP.

Are you saying the results of a Urolift procedure can be similar to that of a TURP? I have to mention. So far no Doctor I've seen in any of these videos for the Urolift, has mentioned the discomfort or pain associated with it. It seems to be downplayed. The pain was intense and traumatic for me. The pain lasted for about 2 to 3 weeks. I had a Urolift in 2019. It helped after a month. THAT success lasted for about 2 more months and I was back. My Urologist did a cystoscopy and explained I had a median lobe. He explained we could put another urolift implant in or we could go the route of using greenlight laser surgery to remove the tissue at the median lobe. I did THAT. The pain here again was also traumatic and intense. lasting for about 3 or 4 weeks. That was done a year and a half from the first Urolift procedure. That worked for 2 or 3 months, and now I'm back to where it was, again. Do doctors have any idea how painful this procedure is? And after all is said. Here I am experiencing complete "Urolift failure" twice. I can't be the odd case. The failure doesn't bother me as much as the pain, and knowing I'm going to have to go through this once again at some point in some way. I have to say, "I'm scared to death". This is consuming me. I'm pretty sure, Rezum, a TURP, a TUNA, any and all of these will be extremely painful. There's gotta be a better way. But don't for a second assume or convey that these procedures are somewhat painful and not a big deal. They are in fact Traumatic. I wish that would be understood.

I am looking at one procedure or the other and appreciate your comments. How bad was the pain after the urolift and the laser procedures? Pain urinating or general pain in the penis? I suffered from urine retention and the pain from that was unbearable. Thanks, and hope you are feeling well.

@@robertrosanelli444 It was pressure and soreness pain as I urinated. I did expect some of that. But, it WAS more intense than anyone had expressed to me. To be fair, I have heard others say it was painful but not a big deal. The discomfort was both, a little retention the first time around, but when you have to urinate it comes on like a massive wave of intense pain. It does calm down after you go. But in the beginning I had to go a lot. The laser, for the median lobe which I did a year later, was in the same pain category but having been through the urolift I wasn't as freaked out when the pain came on. I also knew that if there was retention, and it was too much for me, I could just go back to the office and they'd catheterize me for another 2 days. Which is what happened the fist time. I was just so freaked out at the notion that I was unable to urinate that I couldn't hold out any longer and ran down to the office. Maybe had I waited it would've been O.K but that pain was more than I could stand. My doctor WAS honest. HE DID tell me, I was gonna be miserable for 3 weeks, especially the first week. some of these testimonials on you Tube here, seem underplayed. One guy said he was fine the next day and his flow was fantastic. I wouldn't count on that.

I had the Urolift procedure done about 10 months ago and the pain was intense, especially after I went home without a catheter, and had to go to an emergency room and have one put in to be able to pee at all. I peed blood for a couple of weeks. Now at 10 months later I am better than I was before the procedure was done.....but, I hope there is a better way.......I would not recommend the Urolift...

I had the TURP surgery and was told I had a "larger than expected" prostate. The result was bleeding for 6 weeks and incontinence. My Urologist wanted to give it another 3 months with a follow up visit in early December. I also have 100% retrograde ejaculation. After doing some research I discovered the Macro Plastique procedure is good for curing incontinence created by the TURP surgery. My question is whether or not there is a chance the Macro plastique procedure will also help reduce or eliminate retrograde ejaculation?. My Urologist is hesitant to answer this question and only suggests the Macro Plastique procedure may reverse the effects of TURP and leave me with problems urinating again. I could use some professional advice on this.

I had Urolift five weeks ago. I still have a catheter because I cannot pee at all. I changed doctors and I am now waiting for TURP to be scheduled. Urolift has caused me a lot of pain, discomfort, and worry. [Update] I have had HOLEP and it solved everything. Super easy, very few side effects and everything is working great.

@@hoffy1003 definitely felt pain but the more water you drink the less irritation . It’s about flushing your system out . It’s important to stay regular too . Don’t want to be constipated . For the first couple months your going to feel it . I’m over 3 months and I’m still healing but a lot better.

I had TURP done on 10/02/2023 the best thing I have done. My other doctors were just giving me pills for years. They just about destroyed my bladder. I was having to use catheters sometimes to pee. Just find a good surgeon who knows what they are doing. That is what I did was to find a good doctor, who told me what I needed. I pee so good now, I wonder why they made me suffer for so long. Pills do not work they did nothing for m

I've had friends who have have had each of these, I had the prostate pinch 6 months of pain. (Urolift) My friends who have had PAE are the happiest, Sure it takes a while for it to work, but for pain in recovery and performance afterwards, they say. They are glad they didn't go with a Urologists!

I had a urolift, a PAE and a TURP. The TURP is the only one that made a difference in my urine flow, but my urologist (the fourth I've been to) said my having the urolift complicated the TURP procedure. I spent about three months fighting UTIs, and my urine flow is better but not great. The good news is that I still ejaculate about as much as I did before the TURP.

Questions! Do we have any longitudinal studies of results? In other words do the effects last a long time or does something else need to be done months or a few years later? This is a relatively new procedure so that data may not be available, but normally this stuff has been tested for a long time before it reaches common use. Does catherization of a patient prior to a euro-lift procedure impact the ability to do the procedure? One urologist has implied that catheters cause infections and infections then tank the ability to complete the procedure.

I just had the Urolift done, feeling internal swelling? I guess this is common, due to the compressing of the prostate tissue? Hopefully, it will improve my quality of life in a few weeks! Took approximately 30 minutes for procedure from start to finish!

My dad is scheduled for urolift at 9:35 AM. Now It's 1:52 AM and after going Through several videos and comments We decided "NOT" to forward with the procedure. Tamsolosin is a better option.

Very painful during and six days after. I can’t take pain meds so I yelled out in pain. Had to stay on Flomax. Retrograde ejaculation. I feel like I’m shooting but very little if any cum. Very strange feeling. Very difficult to control when to release cum. But my experience may not be yours. I was 66

If urolift fails (eventually), and one has opt for turps, how do you get the urolift pins out? . I assume turps can't be done if the pins are in place.

The urethral endpiece can simply be grasped with a cystoscope and pulled off of the suture. It adds just a few minutes to the procedure

Had a urolift procedure. Not to much pain involved except less that one year it ruptured and now looking for a TURP solution but not to giddy about it since there is always another chance at failure and lesser quality instead of better quality of life. But I suppose you can say that about all surgeries. Sometimes I wonder if Urologist just do these procedures to make money.

Thanks for the information. I wonder if 1) Can the patient feel the foreign bodies in his prostate after the procedure? 2) What are those pieces that hold the prostate open made of and can they be a foreign body problem? Like, a friend of mine had a hip replacement and then had to have it removed because the metal in the replacement was toxic or had been recalled. 3) Can those squeezer pieces be removed, and if so how?

I had a Urolift. It remained effective about 2 years. So then I went in for my second Urolift a week ago. Just letting you guys know the first week after a Urolift isn't easy. Especially the first 3-4 days where you can only go an ounce or two.

I have been told by my urologist that I am a candidate for the urolift procedure. However, while your information suggests an application of general anesthesia, I’ve online that general anesthesia is not required. I’m asking because I have been diagnosed with a heart condition known as AFIB. I’m told this is a danger.😮

Had a Urolift about 4 years ago. It's no longer working, so I am now considering aquatherapy OR laser. TURP is NOT an option due to the pain and recovery time. So i fyou get one, there's a good possibility it will work for a while then the prostate will still enlarge around the top and bottom and then you will need more treatment.

Look into PAE. My dr told me I needed a TURP and I was too fat for Uroloft (I’m 280 and 6’ 0” and sent me to a specialist. The specialist said I didn’t need any surgery but he would only do PAE. They go thru you leg/pelvis area, not your penis. Anything thru your penis can cause incontinence and retrograde ejaculation. You could be wearing diapers for the rest of your life. DONT DO IT

I had TURP done in 2011. The urine flow increase rate was welcomed (like a horse) the retrograde ejaculation not so much. Sometimes I have blood clots in the urine after having sex but no other time do I have blood in the urine. However, after these many years my urine flow rate is tiny again.

I had this procedure done a couple days ago. 24 hours later, I was rushed into the ER because I have complete blockage. They drained 2000ml out of me. I thought I was going to die the pain was so bad. Do not believe them when they say you won't need to be catheterized...The Urolift manipulates the prostate and that causes swelling. The swelling can block the bladder completely. That's what happened to me OR SO THEY SAY. Who knows? They could have botched the implant. I'll wait til' Monday to see the Urologist's excuse for this so called simple non-invasive procedure. I'm scared when the catheter is removed , It will be blocked again. If I had to do this again, I WOULD NOT.

Marc Kittleson I think this is a new procedure and they are learning all the time. Everyone should be patient but take precautions. I think the doctors should have implanted a catheter immediately or at least had you under close supervision for a few days. I had TURP. 4 years ago but it looks this procedure can preserve ejaculation and ED.

I had a similar experience about 10 months ago. i had the Urolift done and went home after a few hours without a catheter. I peed a little blood and urine right after the procedure, and I suppose that was why I was sent home without the catheter. Later that day I had to go to a hospital emergency room because I could not pee at all. The catheter they installed was removed a few days later, and it was still difficult to urinate, and what came out was mostly blood. The bleeding gradually stopped after about 2 weeks and now, months later, I do see an improvement compared to before the procedure. I still have to get up several times a night, but starting a stream is easy and there is no straining required to urinate or dribbling afterward. I am better than before I had the procedure, but I could not recommend it to anyone...try something else....

Urolift completely and permanently destroyed my life !!!! Now I am faced with my new urologist wanting to do another surgery (greenlight) to correct to the botched UROLIFT procedure. I am told I would have to self catheterize everyday possible for the rest of my life. My symptoms now are WORSE than before Urolift. Please be advised and don't say I didn't warn you if you get this procedure !!!!!

How many methods are used to kill prostate cells? I read about a microwave treatment but the urologist had not heard of it, Can the temporary catheter requirement be a self catheter?

I had the Urolift in June of 2022, wide awake, no anesthesia. It was horrible and extremely painful to the point I was screaming. I was supposedly inhaling nitrous oxide gas, but it did nothing. Similarly, even though my doctor shot six clips into me, after a few weeks, I was right back to the same low quality urination as if nothing had been done. Now it is March 2023, and I am supposed to have a TURP procedure next week, but after seeing this I may cancel. I was not aware it guaranteed retrograde ejaculation and possible erectile dysfunction. The Urolift, unfortunately, is not the miracle cure it is made out to be in these RU-vid videos.

I did go ahead with the TURP procedure in March of '23 and only needed a catheter for a day or two and have been okay ever since, though still taking the Flomax/Tamsulosin at least two or three days a week. I have had no ED, and as long as I haven't taken the Flomax for a day or two, normal ejaculation as well. I should also add that when I had the TURP procedure, the urologist removed at least two of the Urolift clips. Not sure if it was any more than that.@@Richie8614

I had a TURP after a urolift, and there were some complications because of the previous unsuccessful urolift procedure. But, I am urinating better and still can fully ejaculate. No incontinence. The main thing about having a TURP is finding a doctor who has done a couple thousand of them and you will benefit from that experience.

The new TURP procedures technology are made with low temperature Plasma....do not destroy tissues....and have not secondary effects.

@@UnaSolaTorah My TURP worked well for me, for the enlarged prostate, but it created scar tissue around the outside which made it difficult for my surgeon to perform a radical prostatectomy because of my prostate cancer that developed four years after the TURP was performed. Does the low temperature plasma prevent scar tissue around the outside of the prostate? Because the scar tissue binds to the nerves controlling erection. That’s my understanding. But maybe my surgeon did not use low temperature plasma. Actually I remember now he used something called green laser. I bet the green laser causes scar tissue but the low temperature plasma does not?

I have seen online where there is talk about new findings of Urolift being helpful for median lobes. Is that untrue?

No, that is a newly developed technique for Urolift over the last year or so. It has recently been developed and the evidence is not as strong for this technique as it is for its other indication yet. Certainly this may change as it is being studied actively. I think the results are promising. Currently the AUA recommendations are for BPH in the absence of an obstructive median lobe.

The Procedure can be done if your Prostate Size is as small as 20 centimeters to as big as 80 centimeters .... Ideal size I had was 50 centimeters .... your Urologist has this information about each patient?

Do you perform this procedure without a female cna or NURSE? Do you have male nurses for men who refuse to have any more procedures where they are exposed and females are the ones prepping and handing instruments.

I had the Urolift procedure done and it destroyed my life permanently. I am now faced with having another surgery to correct the Urolift procedure and I am told I would have to self catherize daily for the rest of my life or the scar tissue would come back. Save yourself a very LONG NIGHTMARE and do NOT get this procedure done!!!

I also had a failed Urolift. Didn't open up the prostate at all. Had to start using a catheter. Switched doctors and had a TURP a couple of months ago. Very successful. Was urinating the next day. Completely healed now.

I got a uro-lift a couple of years ago. It stopped working after about one year. Pretty much as my urologist predicted. Just got a greenlight laser turp from the same urologist two days ago. Improved results already, and for me a bit less painful than uro-lift. The best thing about uro-lift is that the clips can be removed when no longer working.

Seems like everywhere only the people who had problems post their experiences. Either that or they all have bad experiences. I'd like to hear from someone who had a successful ANYTHING ... which according to the number is the large majority of people.

Aquablation or HoLEP seem to be the best procedures, I had to have TURP at short notice due to insurance, deeply regret it
