Neurosurgeon here from the US. We classify this as Body Integrity Disorder. It's due to a disconnect in the parietal sensory S1 receptor in which normally gives a person body awareness. I.e. you can close your eyes and identify your body as your own and spatially account for all your limbs and fingers. Simply put, a person with Body Integrity disorder has a lesion in this pathway so that they do not recognize a portion of their bodies as their own. It's most common in the left arm. It's almost as if living with constant anxiety that a foreign being is growing on your body. As if a stranger is constantly touching you. This is why it was previously treated as anxiety/OCD. Amputation "completes" their body maps and they can now account for their entire bodies. Further studies are being done to understand this disease

My understanding is that while a lesion CAN cause this, there isn't anything to substantiate that MOST of these cases are from "a lesion" (rather than some other anomaly) in the region-- can you point me toward a publicly available source? This isn't my field. That said, I don't want to derail from the general sentiment-- this is clearly a neurological disorder and there is a lot of documentation that it is organic in nature.

If my crudeness encourages even one idiot to keep their attention seeking invented disorders to themselves then that'd be a job well done. In fact I'm sure that idiot will go on to carry a much more productive and happy life than they would have if they had gone down the rabbit hole of attention seeking and self-pitying behavior.

I think he needs a good slap to be honest, there's people out there born with no legs and they get on with it, and if it was that bad why does he keep using it. Why not just accept he's got this thing that helps him walk and stay fit. If he gets a prosthetic that won't feel like his leg either, so just classify your leg or legs as prosthetic ones, and problem solved.

@@xandradoll I don't understand exactly what it is that he is feeling to be honest. They aren't alien legs, they are regular human legs, and why wouldn't they feel like his if he gets all the sensory information from them just like with every other part of his body. Even if I understood it, it still makes little sense as legs are very useful tools that help us function better than without them.

Can I ask where your amputation is at? Mine is below the knee, and wearing my prosthetic feels just the same as having my actual leg. Like if I never looked down, I would probably just forget I was an amputee. I'm curious to know how different it is for people who lack their knee joint

hey, nobody knows so its definitely a possibilty! I'm from Thailand, and we 1000% belive in past lives, and we believe it often manifests as a birthmark, as a form of a past life trauma, but from everything I've read about people's experiences, it seems that emotional trauma is also sometimes carried over. Either way, what IS known is that people with this disorder have a clear diagnosis in their brain scans. People with BID are literally missing a part of their brain that connects them to that limb. it feels like an extra limb to them, they dont identify with it, it's not just in their heads, it's a very real experience for them.

As someone who didn't choose their disability, videos like this are a bit difficult to watch. At this point in my life I've fully accepted my situation, I don't view my disability as a negative thing and wouldn't change it, but I can't say I would have chosen this life for myself.

I doubt it’s easy to live with such a mental disorder though either both is difficult and we can empathise with both. I highly doubt any of these people are messing around or making fun of it when they go through with surgeries and try and commit suicide etx it’s clearly very real to them. I read a surgeon reply here he says there is a legion on the brain that stops them being able to identify with the body part as they should.

I have a disability, and can't walk on my right leg. I try to be open minded and compassionate, and I understand that he and people with his disorder really are suffering. Still, it's so difficult to not feel a bit upset, especially watching him walk. Do you know what I would give to be able to do that? I'm not saying my life is miserable, I'm usually happy with who I am, and I know there's no point in being bitter all the time about what I can't do. Still, this world was built for people with his abilities, not mine. I've missed out on so many things because of my issues. I want to say that I support him 100% in whatever he needs to do to be happy, but it's so hard to believe his amputation would be the right thing. I guess that's more my problem than his. In any case, he and people like him deserve to be happy. I guess I just hope that ultimately comes from neurological treatment, and not from becoming willingly disabled.

I have a disability which causes me to need to use various mobility aids, primarily a wheelchair. I recently saw one of these videos in which a woman who had nothing physically wrong with her decided 'she felt most comfortable in her wheelchair' but would then get up and go skiing, hiking, etc. I get it's a mental disorder, and i didn't want to be, but was so incredibly offended. I totally understand where you're coming from. To see her 'act disabled' and then reveal she was completely fine and go skiing was asinine.

@@NOONE-cd4gu So what's your solution? Cut it off? In before we even fully understand whether or not he's going to get another "itch" to cut the other one off? What about the fact that removing a leg is going to place him at higher risk in the event of danger, such as being caught in a house fire, car wreck, etc? Or the fact that it'll be harder for him to simply be *mobile* ? People disagreeing that the best option is to go along with what the disorder is telling people they want never seem to make the argument that we should just line up suicidal people and watch them jump off buildings one by one. Weird, right? Big difference between treatment that helps someone manage a disorder and giving a mentally ill person whatever the hell they want, even if it's literally self destructive.

Okay, from what I can see, this guy is legit. He is real. He genuinely understand that his brain is rejecting his leg. He described it as growing an extra limb that should not be there. That's actually legit. This dude still shouldn't... you shouldn't take the leg off, it's not okay, but this man is far more understandable.

Ok so what I've gathered from the comments is that his condition is valid and can be compared to disorders such as OCD. His brain perceives his leg as an extra, unwanted limb that shouldn't be there. My first impression of him was that he wanted attention. But by seeing bits and pieces of his life and hearing about how he truly feels, I know that he has struggled internally with this disorder (I mean he considered suicide and all so there's definitely a problem mentally.) However, I'm not sure if going through with the whole amputation process is the right choice. It's obviously not up to me, but what if he regrets it later? What if his brain changes its mind? Idk I'm still a little confused. If anyone has any additional thoughts feel free to add on.

There was another guy with BIID who wanted both legs amputated. Nobody would do it, so he did it himself. He survived, and he is happy and content with himself (besides one leg being slightly longer than the other due to his hack job). Think of it like a transgender individual transitioning to their preferred gender. It's something they've had from birth, and once they align their mental image of themselves with their physical image, they are finally happy with themselves.

As someone who is physically disabled I know how much it sucks to not be able to walk around like he can. However, he isn't hurting anyone, he isn't getting benefits, he's trying to do something for himself even though we find it a little strange. He didn't choose this and he wants to fix it. So why not let him? I saw another girl who was disabled in these comments talking about how offensive it is but I don't see why. He's not attacking anybody, he's not getting money, and he just wants to cut off his damn leg. Yes being disabled is hard but if it's what he wants why is that so horrible.

respectable doctors won't make life changing surgeries like that (or end a life, even) without lengthy mental evaluation of the patient. and, obviously, people that want to remove limbs are not mentally healthy so they aren't considered able to make such a decision. medicine isn't here to do as people please, pointless surgeries to fulfill some irrational mental illness, that most definitely won't help his condition

There's a whole other sidw of this, too, where people physically feel and can manipulate an appendage that isn't there, like a 3rd arm or leg. Brain scans show activity exactly like a real limb when they're asked to move their extra part. It's fascinating.

I have heard of research where they have scanned the brain whilst the limb in question is being touched. The brain doesn't process the feeling in the same way/area of the brain. These people really do feel like the limb isn't their own and shouldn't be there. He must know the struggles he would face as an amputee but that the option of living with the limb is something far worse.

If we can do physical therapy and psychotherapy for pain patients to reconnect to limbs they've had to dissociate from due to excruciating pain, then the same methods could be used for this man. There's a clinic in arkansas that's doing breakthrough treatment for dissociation of limbs due to pain. It includes Neurofeedback and hypnotherapy among other therapies. I would cost as much as an amputation but it might help him.

If you don’t relate to your arms, legs, genitalia, eyeballs, etc… surgeons should not cut them off for you. The kindest thing to do is therapy to understand how to love your own healthy body in tact. It's not a case of simply not relating to your limbs. People with BIID feel as though their limbs are foreign or not their own/aren't meant to be attached to their bodies. It's a genuine neurological issue, they can't force themselves to feel connected with their body parts through just therapy.

This is fascinating, because, neurologically, there should be an explanation for why he feels this way. Even if it's a manifestation of a different, more commonplace disorder or if BIID is the actual underlying condition, finding out why one feels as if a part of themselves is not... its a spectacular enigma of a puzzle.

I heard about this disorder so long ago, I didn’t know the name of it then, I just remember a friend telling me about it, and like.. honestly? If it helps, do it. I’d rather people with BIID lose a limb or two than lose their life to depression and dysphoria. I would love to see more research done on the matter too, it almost sounds like alien hand syndrome, or the opposite of phantom limbs.

@@thebassguy0513 oh I'm 99% sure it's not italian 😋 because I speak spanish and one of my sisters speaks italian, and I know italian doesn't change the W sound to a V sound, as in German "Will" sounds like "Veel", also italian tone sounds different, German is different too, so it has to be some language from somewhere else 🤔 (plus, Italian doesn't have gutural R sounds) So it is not french (2 of my bros speak French), not european nor latin spanish, not italian, not German...not English xD ....I need to know

Unrelated actually. Sense of balance, sense of touch, and ability to move are all unrelated to proprioception (the sense responsible for knowing where your body is while your eyes are closed)

If they numbed it, and he closed his eyes, then yes. We would still know where our leg was based on proprioception, but if you numbed the leg completely (no touch stimuli), and blindfolded him, he wouldn’t be able to like we could, and he’d probably stumble and fall.

I understand that some, especially those that have lost or were born without their limbs, may find it offensive to see someone try to damage and remove their healthy limb, but even if that was your first reaction, you can still try to change your thoughts and feelings about the video. As it was mentioned, he really is suffering, and it's not like he is trying to provoke anyone. Try to put yourself in his position. The brains of people with BIID fail to recognize a part of their body as their own, in this case his leg. I don't really have a good example, but if you have a normal healthy body, imagine if you had an extra arm or leg. You would be able to move the extra body part, but your brain isn't wired to it, so it doesn't feel like it is yours.

If I could move it, I'd actually want an extra body part. Heck I'd take an extra pair of usable arms any day. The problem some people have with situations like people getting willingly amputated limbs is this: once they cut them off, they go on disability. Disability money comes from the pockets of citizens whether they like it or not. If they didn't qualify for disability money, most people wouldn't really care aside from saying it's a waste of a working limb and a disease that wasn't treated but instead allowed to progress. If a person thinks their entire body except the head should be missing, should a doctor give the greenlight for it while we still don't have the ability to go cyborg with 90% machinery? Well, as long as the doctor doesn't get accused of manslaughter and people don't pay for a head's life support...

I think it's similar to how someone with breast cancer who *had* to have their breasts removed may feel appauled by a trans person voluntarily getting top surgery and being happy about it. This is not a mental disorder, it is due to a disconnection in the brain. Telling him to get better through therapy is like telling someone with Alzheimer's to fix their Alzheimer's by going to therapy. Both disorders are caused by a physical difference in the brain

Saying that you are qualified to diagnose when somebody’s affliction is due to a mental disorder because you suffer from mental disorders is like saying because you breathe air you are qualified to assess the levels of air pollution in the atmosphere. It’s pretty ridiculous, especially since it seems like more people than not on the internet these days have mental illnesses. However, people who are qualified to talk on this subject aka neurologists have conducted research on topic. A study from 2011 which I would be happy to provide a link to stated that “Body Integrity Identity Disorder (BIID) is a condition in which individuals experience an intense desire for amputation of an healthy limb. Recently, McGeoch and colleagues provided the first direct evidence that this syndrome may be neurological rather than psychological in its origin.” The research goes on to talk about a lesion in the parietal area of the brain which is responsible for awareness of the parts of the body. Further research since this study has been done, and at this time in the US, general consensus is that the disorder is a neurological one caused by brain lesions, not a mental one.

This is beyond sad . I’ve heard stories about people who had the same mental disorder cutting off limbs . I saw a RU-vid video of a woman who put acid in her own eyes because she wanted to be blind . I see things like this and my heart breaks

As an amputee I think I can understand a little. I had a bad accident where my dominant hand was blown apart. After getting sewn up I was left with most of my thumb and some finger digits missing. I get phantom pain, especially if I think about it too much, I think most people have heard about this. If I close my eyes, I feel spatially aware of where the missing parts should be, I can even feel like I am moving them, I feel like I can bend the top half of my thumb that isn't there.

At least, those born in the wrong body can function properly after they have changed genders. In fact, many even look more masculine or feminine than others originally born into those genders. People who have had arms or legs amputated cannot.

this is so strange for me to watch because I think i have this disorder too... just to a much lesser degree. for many years I've had this secret and strange desire to have my left leg amputated below the knee (never had the desire for anywhere else). As a person who is already mentally disabled I completely understand all the disabled people in the comments saying how they wish they could walk, its insanely foreign to imagine someone wanting a disability of any kind. It seems crazy to me, and yet somehow I feel it. I don't imagine an actual amputation would do me or any other suffers any good, but yet I still desire it? its confusing, embarrassing, and isolating and I've never told anyone. I guess I'm lucky that its a much more fleeting desire for me than this, probably because I'm too distracted by other mental disorders than have a constant impact on my daily life.

It's hard for those with a normal brain and ability to think differently, to understand how those that suffer with this goes through. As a nurse myself I explain it in this way, if you had an arm hanging on your forehead would you not want that removed and feel it isn't supposed to be there? It's along the lines of how this that suffer this condition feels. They can't control it and often will do anything to feel "normal" . you don't always have to fully understand how they feel, just be kind, support them and don't judge unfairly

I once had an eating disorder and fortunately no one recommended gastric bypass surgery, liposuction, or and other weight loss methods. It's really the same for this man, a psychological issue. I truly hope no doctor is recommending amputation as appropriate treatment. He certainly needs help, but he does not need his body damaged.

Ok, so if your anorexia causes cardiac arrest or other organ failure , would you want the doctor to say “no , you caused this damage yourself through your eating disorder, i am not trying to save your life” ? No, the doctor is going to try save you by hook or by crook. Now, consider the problems, this man encounters with doctors being judgemental and saying “no this is totally unethical” but it would mean saving his life to do the surgery. Think about that one before you come over being high and mighty

honestly speaking, he should be able to do whatever that makes him happy. people kept commenting that he should be grateful that he has all his limps, but if he feels unsettled/upset, i'd rather he get an amputation as he wishes. after all, none of us have a say in any of this. it's his life, he can do whatever makes him happy, whatever makes him love himself, as long as he's not hurting anyone else.

I’m transgender and BIID is fascinating to me because being trans was considered an identity disorder until fairly recently. There’s a lot of similarities and a lot of differences, and I’m sure there’s a neuroscientist out there that could go into that in great detail.

I can relate to this dude. I've been wanting my leg amputated from below the knee for over 11 years now after I broke it on my 16th birthday. It's badly deformed, no doctor will touch it. I have a condition known as osteogenesis imperfecta which means brittle bones so doctors are wanting me to break the leg so they can fix it but you can't control where you break so it won't work. It was going to be amputated when I broke it but because I said okay - they put me in a psychiatric holding for 24 hours because I was okay with it to be gone. 😕 depressed and pain ever since.
