Good morning, let me tell you my story I am graduated as a nursing technician in Brazil, I have already graduated for 20 years, I have worked in public records, asylum, in several hospitals and worked in a prison in the state of minas gerais this in brazil, i have a lot of experience at the health aria , i am walling in boston , i have social and work permit , i currently work here in painting , but i am struggling to learn english and take a nursing assistant course , i dont even know how much an assistant earns in MA in the region of BOSTON, I'M FIGHTING TO LEARN TO READ AND, WRITE AND SPEAK ENGLISH, ONE DAY I'LL GET IT IN THE NAME OF JESUS

Oi Edison, em Massachusetts, a media de salário de uma CNA É de 17 a 33 dólares a hora. O nivel tecinico que vc tem no Brasil, aqui é chamado de Lpn ou License practitioner nurse. Eu fiz meu Cna por uma community college, não precisei fazer toefl nem nada do tipo. Vc precisa saber e escrever pra a prova escrita e communicação é bastante importante pra a prova pratica pra conseguir o registro. Eu moro em Iowa. Boa sorte!

2 things missing: rising the bed at waist level before starting to prevent injury to back, and lowering the bed for safety (prevent falls) at the end

I'm currently in Florida and I was taught to always put side rails up while asking the resident to turn on their side. Even the instructor for the exam said to use side rails, as long as you put them down when you are finished. My place of employment also requires side rails when asking residents to turn to prevent falling, no matter how capable they are of staying on the bed. I understand it may be different depending on instructors and facility policy. Just wanted to share my experience with side rails. I love these videos and they help definitely helped so much!

Thank you for your input. The state exam specifically states that siderails will not be graded for the purpose of providing safety, so safety MUST be addressed in some other manner other than siderails. This video demonstrates the proper performance of skills without the use of siderails while maintaining safety principles. Many settings (home care, assisted living centers, convalescent homes, etc.) will not have siderails on the beds, so everyone must know how to perform these skills without risking injury to the patient without the use of siderails. Saying the bottom sheet is 'soiled' indicates the need to wear gloves during this procedure. If the bottom sheet is indeed soiled, and she touched it with her bare hands, she now contaminated everything else she touched after that point. I loved the video, but wished she would have put gloves on before touching a soiled sheet with her clean hands, or washed her hands after touching and disposing of the soiled sheet.

She saying soiled as in used not as in pee and poop is on it .. there are no open body fluids and according to the care plan we’re just changing the linen not changing “soiled “ linens

@@lauren9166 You are correct in that 'soiled' in this case is reffering to sheets that are dirty. That is not my argument though. My argument is that she touched the dirty, and in a realistic setting, probably pee/poo filled sheets with her bare hands and then proceeded to touch everything else. Once you touch something dirty, your hands are dirty and you cannot proceed to touch things that are clean. If you do touch things that are clean with your dirty hands then the clean thing you just touched is now dirty. The correct thing to do in this scenario is change the sheets without the gloves as she did, and then wash her hands before putting the new sheets on. She could also have put gloves on before taking off the dirty sheets, and then place the dirty sheets in a plastic bag or whatever the facility is telling you to do. Then you can take the bag of soiled linens to the appropriate place, take off the gloves, and proceed to go put on the new sheets while still maintaining a clean environment. If you want to be extra clean you can also wash your hands before putting the new sheets on as well. Remember, most of us are getting paid hourly, so if it takes us a little longer to do these things in order to provide proper patient care, then that is just what needs to be done.

Okay, to the people over here who did not do any research about Prometric tests, she is exactly doing what you need to do in order to pass the test. The checklist that the proctor uses does not include raising bed rails, locking the bed (it is assumed the bed is locked considering the patient has been "lying on that bed for some time), raising the bed to a comfortable level, wearing gloves, etc.. Honestly, a quick Google search brings this up. 🤷‍♂️

No it was very easy. Just make sure you wash your hands , make sure you don't say done till you checked everything and make sure you give the call light to the patient like make sure its in there hands. Because they will fail you. The test part was very easy I would say its mostly common sense .

You always ask the patient “is that okay” when about to start a skill. I tried that while practicing with my kids and they said “no, that isn’t okay” now I need to know what would you do if a patient refused? Lol darn these kids 😂

In the real world you are there to perform a specific task. You come in and let the person know what you are there for, correct? When you are performing the task let them know what you will be doing to alleviate apprehension. If you ask at every step you would never finish in a timely manner. Be professional, compassionate and polite.

Im suprised no one has mentioned this cool trick! When replacing the pillow case grab a a corner peice of the pillow case letting the pillow case hang inside out then grab the pillow by the tag with that same hand while still holding the pillow case and flip the pillow case right side out over the pillow! It makes it easier to not let the pillow or the case touch your scrubs!

The nurses said I was doing something wrong with this skill but I told them “if Mrs. patty says it right than it’s right” the nurses looked at me like “who is Mrs.Patty” lol

QUESTION...Regarding tucking the clean sheet "under" the soiled one...would not that be a risk of contaminating the "Clean" sheet?

Jamie Branch you may have found the answer by now but for some reason, normally they won't count it against anything for touching the dirty linen even if it could cause contamination

If you read the prometeric checklist for this skill on #4 of this skill it says : Remove and replace bottom sheet on one side of the bed, before turning resident to remove and replace sheet on the other side of the bed... So it is done correctly in the video. Hope that helps

Somehow I missed this comment...my apologies, but let me try to clear up this point for future students! Remember: Clean rolls toward you, dirty rolls away. So rolling the dirty sheet away and tucking the sheet under the patient, then rolling the clean toward you and tucking it under the dirty only allows the UNDERSIDES of both sheets to touch. So the part of the sheet that the patient way lying on is not touching the part of the clean sheet that the patient WILL be lying on. However, please be aware - the PATIENT is still in the bed. You know...the SOURCE of that contamination in the first place! :-) As soon as that clean sheet is placed on the bed, the patient is lying on it. Now, if the dirty sheet has liquid that has soaked into it, we don't want that liquid to get the new sheet wet, so wrap a chux around the soiled sheet roll before putting the clean sheet on the bed and this will prevent transfer of wet body fluids during the linen change. But this is not the case for the exam....we are simply demonstrating that we know the technique used to change the sheets. Hope this clears this up! Best of luck!

At 1:17 it says “for the exam there is no exposure to body fluids, so no gloves are necessary.” In real world conditions that won’t necessarily be the case.

The patients needs to be more realistic in these skills. Most of the residents at the nursing home aren’t moving freely as this man in the video. Most time you the CNA is doing most of the assisting in getting the resident in position to tuck the sheer. In real life there is no scoot over so I can tuck the sheet

I’m watching this before my exam to help me be more confident in my skills. Though the procedures are slightly different it’s is still very helpful I just have to remember the extra steps that are now required but this is still so great to watch a demo! It’s so helpful thank you so much! Tbh the only thing they are requiring that’s different is the sanitize the bed and it’s probably not every state.

i believe that during the exams in CA, you must wear gloves when the linen is 'dirty'

+mimi lee Gloves are not required for this skill for the exam. You can wear them if you wish, but they are not REQUIRED. These videos demonstrate the required steps for the skills exam and are not intended to take the place of actual classroom instruction: these steps will not apply in all circumstances as every patient is different and will require a different level of care. But for this scenario, for this specific patient, gloves are not required. Hope this helps!

you dont have to wear gloves. I took my prometric cna exam on the 23rd and passed. They gave me this skill and I did not wear gloves.

Why do cna have such dirty jobs it hurts there backs and makes them more airborne to diseases and infections that could be handled with more sanitary conditions not just washing hands but wearing masks and getting a proper time to do these skills

thank you so much for your videos! i take my exam thursday and these are so helpful! I was just wondering, if I do want to raise the bed for my comfort, can i leave the patient on their side while throwing the bottom sheet in dirty linens with the bed raised?

Sorry for the delay in answering! Yes, you can leave the bed raised as you perform this skill. The bed must be lowered before you end the skill, so if the patient wants to get out of bed, the floor is where they left it! Hope you did well on the exam!

Yes you definitely should. If you're comfortable with the glove habit they won't deduct points! I always wore gloves when practicing and during clinicals I'm glad I never forgot 🤧 very soiled linens there. Best of luck

Working in the nursing field for 20; years her first mistake is not wearing gloves to remove soiled sheets, then removing her contaminated gloves and putting new gloves on to put the fresh sheets on.

Always,wear your.gloves and change your gloves during procedures like this. And sterilizing your hands after the procedure is finished before leaving the patient's room so you don't cross contaminate

Ensuring that the patient has the ability to reach out to the healthcare team in an emergency is critical. You must ensure that the patient has access to the call light and it is within reach before you leave the patient's environment. This is clearly demonstrated near the end of the video (at 7:07) where I ask if he needs anything else before I go and give him the call light. Thank you for your comment!

I'm a big picture kind of a gal, so I know that remembering all of the small details within each skill is going to be a pain. Typical Sagittarius here ;)

Best of luck in your class! The small details should be presented in a manner that is easy to remember, your instructor will explain "why" each step is important. Our skills book also provides step-by-step instructions on how to perform each skill. Many schools use our books and videos, so you may see us again in training! Best of luck!

Isn't it considered dangerous to leave him on his side when you throw the soiled bottom sheet in the linen basket (4:13)? I thought he must be in the center of the bed when you're walking away. How can you avoid this? Maybe by putting the linen basket at the foot of the bed?

Since we asked the patient to scoot toward us prior to turning (1:50 and 3:45), we ensure that the patient is in the center of the bed when lying on their side (see overhead camera angle at 2:00 and 3:54 which shows that the patient is in the center of the bed). This is sufficient for safety purposes, since the patient is not positioned near the edge of the bed. You are still in the room, since the soiled linen hamper will be at the foot of the bed or near the bed for the exam, but having a patient lying on their side in the center of the bed is an appropriate position even if you were leaving the room. The key here is that they are in the center of the bed, not whether they are on their back or side. If you have a patient that is unable to remain on their side without support, the care plan would specify appropriate measures to take to ensure their safety. This patient (for the exam) has no physical limitations that prevent them from lying on their side unassisted. Remember, the CNA follows the care plan, the whole care plan and nothing but the care plan. Hope this helps! Thank you for your comment!

You are graded on the first two times you wash your hands (beginning and end of first skill), but you will continue to wash your hands until the evaluator tells you to stop. Hope this helps!

We are very happy that you have found our videos helpful! The care plan for the state exam asks the CNA to change the bottom sheet, top sheet and pillowcase only. So no draw sheet is required for the exam - they are only evaluating MINIMAL skills competency. Best of luck!

I always use gloves doing this for most clients are incontinent. Would I be counted off for wearing gloves?

For this skill, your patient is fully clothed and continent (holding onto all their own body fluids) so gloves are not required for the exam. If you try to put them on, you might be told they are not required. Remember than "most clients" are not "this client" and every person is an individual with individual differences that will affect the care required. Best of luck!

Prometrics in florida does not require a privacy blanket.. is there a reason you guys do so?

Because using a privacy blanket falls under indirect care. Think of it this way - as a healthcare worker, you are fully dressed and extremely active. The temperature is going to feel very warm to you. Your patient is not active at all and dressed in a thin hospital gown. The temperature to them is going to feel cold to them. Now, if you remove their sheet and leave them uncovered, they will feel very cold and exposed. Since your perception will be different from the patient's perception, it is important to take a moment and put yourself in their place. Using a privacy blanket will keep them covered and warm while you perform the skill. You could just leave the top sheet on while you are changing the bottom sheet, but it will probably get in your way or touch the floor (not allowed and will result in an infection control deficiency). It is way easier to use a privacy blanket instead. We use a blanket in all skills where the patient will be uncovered or undressed for consistency - that way our students don't have to try to remember "We use one for this skill...but not for that one..." which makes learning the skills more difficult due to the lack of consistency. Hope this helps!

if this were the state exam she would have failed. she did not raise the side rail opposite working side of the bed . she also didn't raise second side rail opposite working side of the bed.

This skill is performed according to current Prometric testing standards. Siderails are not required for the exam and their use is not graded, which means that you must provide safety during the skill regardless of side rails. Ensuring that the patient is always in the middle of the bed using the technique in the video will satisfy safety requirements. Thank you!

its not gonna hurt when you present the skill so long as you follow universal precautions for example in CA you still must wear gloves doing this skill because you are touching dirty sheets to clean hands than dirty hands to clean sheets cross contamination. you must use 3 pairs of gloves. resident/patient may have an diagnosed condition

your bottom sheet is fitted :0 if only it were like that in the UK!! every sheet and blanket has nursing corners! and no sheets must hang free, the must be tucked in. Matron would lose her mind else! and the top sheet, at the patient end must be folded back on itself lol shall be strange making beds in the USA

For the exam, you will be required to change the bed using a fitted bottom sheet, flat top sheet and pillowcase. Most facilities now use fitted bottom sheets to minimize wrinkles and reduce bedsore formation. There is no testing requirement that dictates turning down a patient top sheet (although it does look nice, it isn't a testing checkpoint). These videos show testing procedures only and should not be substituted for a comprehensive CNA training program taught by an RN. They are merely for demonstration purposes.

In North Carolina they don't make us, but usually so far it's been a person on each side so that's probably why, in the real world though if you are alone I think it's best to use a side rail

Ok. so this might make me look extremely stupid, but i seem to have difficulty when placing the first sheet on the bed because i don't match the corners correctly (it doesn't stretch long enough because i don't place it in the right corner, not sure if this is making sense) is there perhaps a rule of thumb to always getting the sheet at the correct corner?

When you unfold the sheet, unfold it on the bed. Position the sheet so that is spans the entire length of the bed before attaching the corners. This will make sure that you are putting the sheet on correctly. You can see how I stretch the sheet out before attaching it in the video starting at 2:45. Hope this helps!

You will receive a care plan with 3 clinical skills listed. You must do the skills in the order listed, treating each skill as a separate patient encounter (with the opening and closing for every skill). You will also be graded on handwashing and indirect care in addition to the three clinical skills. There is also a 60 question multiple choice test given the same day. Best of luck!

You should be wearing gloves when dealing with visibly soiled sheets and linens. Do not let the linens touch your body or outfit or they are contaminated.

So this is wild, ive seen it both ways. Why no blanket? Clinical skills are all centered around resident care not patient care and even in hospital setting the only time I saw patients with just a top sheet was in the ICU. But we also had a blanket warmer that I was reloading everynight 😂 these skills dont translate to real life. I understand that its to pass your exam so no hate on the material but is there an actual reason here?

+Janiyah Morgan For the exam, you will not have to check the ID bracelet - the patient for the test will not have one. Many patients in many settings (home care, assisted living, some nursing homes, etc) will not have ID bracelets at all, if it is where they live. So, the test does not require identification using this method. You will be required to verbally identify the patient by name (just like this video). Thank you and Good luck

Yes. I've noticed also that bed rails and ID are not checked in your videos. My school stresses doing this, this is also in our text books. I am taking the test next month....praying I pass!

LadyMs Moody why not just use the checklist?? The instructor in this video uses the checklist, if they cant take points off during the exam based on the PROMETRICS checklist then what your class states is irrelevant. Many of these schools are not that good, you should also download the pass rates of cna schools in your region on the PROMETRICS site and see how your school did.
