@@_o6629 I was going to make a joke but I'll just leave you with this: a very very very small proportion of us have ever even touched a gun in real life, much less been shot. The movies and news aren't everyday life.

For one of my first clinical exam, I examined a patient with acute pulmonary edema because of Afib. I had done all the systems, cardio, pulmonary, neuro, vascular, as we were told to do in med school. Then I wanted to do the abdominal exam. So I remembered my textbooks and I told my patient, who was sitting in a chair, to lay down on the bed. I helped her, until she started drowning in her own fluids ! I was like "S*** !! I forgot about the edema !!"

We even have a word for that feeling in Germany: Fremd schämen. To feel ashamed/embarrassed because of something someone else does. And I do feel it here, too. 😅😆

Had an ER nurse do an abdominal exam on me that I still laugh about! Instead of asking about my pain she said, "You have the softest skin! I mean really!" (She raved for a good two minutes about my skin!) Finally she asked, "What do you use?" Mortified and in agony I said, "Soap and water!" (It's apparently one of the few good things I inherited from my family!") Later I was flattered but I definitely didn't feel it in the moment!

For my first abdominal exam the doctor gave me a patient lying on her side, looking like she was in a lot of pain. So I very nervously presented myself, asked her where the pain was located (epigastrium), also where I could see a big mass, and proceeded to palpate deeply on that very place. Yeah.... didn't even asked why she was there: She had stomach cancer. Didn't even observe, ausculted, nor percussed, I just went all in. Got yelled at and almost made that woman cry of pain. This memory comes back from time to time to haunt me, really 🥺.

For the practical portions of the physical diagnosis course, we broke up into smaller working groups. One person would be the subject for the teacher's demo. After reading ahead in the manual, I knew there were parts for which I really didn't want to be the subject "patient", so I readily volunteered for pulmonary exam. Having a dozen guys listen to my lungs wasn't bad...just a little alkalotic at times. Poor guy who was a little late for a class got stuck with genitalia.

As a surgeon i can attest that palpation does not cause the stitches to open up, unless you do it with extremely filthy hands that ultimately cause wound infection that blows the stitches out

@Jason Walter Actually this man & I have been friends quite a long time. Through several Presidents. You want to go there? He actually had to Move North to get ANY decent treatment at the VA the Georgia 1 only made him drug addict. MN caught him up in the surgeries he needed. ^^ Btw said Resident was kicked off his care

The idea is that he tried doing the deep palpation that we're supposed to do to check for abdominal masses, I think. The one where you use one hand to push the other down as deep as you can go, according to the textbooks. Never seen it in real life. I guess that could tear stitches?

This reminds me of when I had been a student in Podiatry school and my first foot screening event. I felt for the pulse in the wrong spot, but then corrected myself, had let the patients eyes remain open and looking towards feet while I did the neuro check, and then noted a bunch of calluses and corns but did not offer a referral to the guy - I gave him a clean bill of health- I hope somehow his feet are healthier today ...

Thanks for these pearls. I’m going to do my first year of clerkship this fall, so I’ll keep this in mind. Sometimes we get told to “always” do things, and be systematic, like a checklist, and so we are left wondering, should I do this thing that can be painful or should I omit it and risk missing something important?

As someone who has 0 qualifications aside from working in an ER for a bit: I think if you skip stuff because it might be painful, you're guaranteed to miss a lot. Pain seems to be one of the most important sources of information for surgeons. Though you probably don't need to put your finger up someones butt if they just hurt their arm.

as a patient, I don't want you to miss something- just please let the patient know that the exam you're about to perforn may exacerbate the pain symptoms.

I love this!! I make for the most embarrassing patient when it comes to abdominal exams because... I'm extremely ticklish. I'll let you imagine the picture 😅🤣😳 Have you ever had someone like that, Dr. Schmidt?

In nursing school they teach to have a ticklish patient place their hand under yours while you do the exam (doesn't have to be exactly, finger for finger underneath) with the rationale that it's much harder to tickle yourself- hope this helps you!

*Please help!! I cant find this answer anywhere!!* .. If a patient is having chest pain, can you give them Nitro if they have Hyperacute T-waves in leads 2,3,aVF (inferior). Even with a history of a inferior MI w/RVI.

I don’t think so. Inferior wall involvement is only condition where nitro not given and fluid given. That’s what I learnt in hospital duty. And the reason why you wait for ecg before nitro
