Weakness of the medial triceps is very common. The medial triceps is like the VMO of the thigh; it activates the best during humeral external rotation and at the last degrees of extension.
Its weakness is commonly associated with triceps tendinopathy, ulnar nerve entrapment as it courses through the arcade of Struthers, as well as medial dislocations of the nerve or even the medial triceps itself (severe circumstances).

Im a bit late with this comment. I am also facing a triceps tendonitis on both arms at the moment. In case you had something similar, how are you doing now? Did any specific tips help you the most? I would really appreciate if you took the time to answer because it could help me out a bunch.

I've searched the internet for evidence that corroborate what you say, and I literally could not find anything concerning both medial head tricep weakness and ulnar nerve entrapment. Of course I believe what you say, and have implemented your exercise as a part of my routine while my tricep tendonitis heals. Can I ask how you found out about this?

Hey so I bench pressed the exact way you described: supinated wrist and humeral external rotation. The only thing is I did it with heavy weight repeatedly. I think that causes my ulnar nerve problems in addition to shoulder problems.. I'm curious if this rehab will work for me since this is how I caused the condition. Also how does that grip affect the rotator cuff muscles. I feel in that grip there's more teres recruitment during the bench press doing that. Is that true?

Would you also recommend underhand cable pushdowns as a standard triceps variation for bulletproofing the area?

@@MSKNeurology Always do, since you've helped me tremendously. Just wondering if I switched to underhand grip for tricep on push day, If that would have a similar benefit.

@@Marco-pf3te If no pain, then go hard for a couple of sets with pronated grip and then go SLOW AND EASY as I show here with max. supination
