Yes, metabolically healthy obesity is possible, but not on a high carbohydrate diet. My wife is 71 and has a lot of subcutaneous fat in the hips and thighs. 5'6" 190 lb. She is very insulin sensitive. Her labs are always stellar. A1C low 5s. Fasting insulin 2.5 to 6. Adiponectin way up around 20. Fasting glucose 90s so HOMA IR is great. Trygliceride 60s, HDL 60s. Even LDL is pretty good. Thing is she has been fat adapted producing ketones for many years on a very low carb diet. If goes off that regime, she puts on 50 lbs and labs go to hell.
