Onset: 28 weeks gestation
Integration: 5-6 months
Position: Child is supine with head in midline
Procedure: Support infant’s head and shoulders with hands, allow head to drop back 20-30 degrees with respect to trunk, stretching neck muscles
Response: Abduction of the upper extremities with extension of the elbow, wrists and fingers, followed by subsequent adduction of the arms at the shoulders and flexion at the elbows
* Note that this is a startle reflex and can be elicited in multiple ways including a sudden loud noise. Baby is 5wks 5days.

This is tough to answer because it could be from multiple things. It would be important to consider the pregnancy/birth history, health history, other signs/symptoms and findings. You aren’t wrong in recognizing that a BPI would lend itself to a different presentation. You may also see differences with stroke, cerebral palsy, seizure disorders, and a number of other diagnoses.

Could be neurological or orthopedic involvement on the side that didn’t respond OR if the kid is older and the reflex is starting to integrate you may not observe it equally

The thought is that its evolutionary and survival. The thought is if the infant starts to fall from mother they startle and then cling to them.

The value of checking this reflex is in knowing that the nervous system is in tact. If the child does not demonstrate the reflex OR presents asymmetrically then we know that there may be damage to the nervous system that we need to look into further.

If baby's do not have this reflex there might be something wrong. There are adults that have this reflex extremely. When they are startled they flap around like a fish on land.

In infants they don't have the same voluntary ability to "resist" the reflex. In adults you may still respond similarly BUT your neurological system is developed enough that if you needed to chose not to respond with this reflex you could. Assume you are carrying an infant and start to fall. Reflexively you might want to put arms out BUT you can overcome that and voluntarily still hold the infant and land on your back.

It is not a problem to have this reflex. It’s normal and appropriate in infancy. As he gets older it should naturally go away and not be a problem. If it does not go away then that is grounds for evaluating the infant for additional signs/symptoms and consider a diagnosis.

The value of checking this reflex is in knowing that the nervous system is in tact. If the child does not demonstrate the reflex OR presents asymmetrically then we know that there may be damage to the nervous system that we need to look into further. Note though that it is theorized that this reflex was initially innate in infants for the reason stated below by @ravenID429. It is thought to be a protective reflex.

It’s not a problem if the child is still a young infant. If your child is old enough that they should have integrated the reflex and it is challenging your child’s development then I would suggest you speak with your physician. There are things that you can do to help but it is very dependent on what specifically is going on with your kiddo.

There are multiple approaches to eliciting this reflex. However, how we assess the resulting reflex should be the same. If the way that I test looks different from what you are familiar with but you get the same reflex, then we have found the same information but taken a different approach. That is fine. I have taken this from a text for healthcare professionals and teach this to graduate students. You are welcome to test it the way that makes you most comfortable.

It’s a test used to check the reflexes. If the baby does not react the way it did in the video , it means there’s some damage to its central nervous system which would require further evaluation and treatment. So no, it’s not even remotely violent.

My technique/practice was taken from: Pediatrics Wanda G. Webb PhD, CCC-SLP, in Neurology for the Speech-Language Pathologist (Sixth Edition), 2017 "Moro Reflex The Moro reflex, along with the ATNR, is one of the best-known and best-studied reflexes of child neurology. It is present in almost all newborns except for small premature babies. With sudden head lowering, a rapid and symmetrical abduction and upward movement of the arms occurs. The hands open, and gradual adduction and flexion of the arms occur. The lower limbs also show extension and then flexion." Note: I recognize there are multiple correct methods of testing this reflex. I anticipate the one that you are speaking to might involve grasping the child's hands and bringing them to midline and then releasing them to allow them to "fall" and again lend to a startling reaction. This too is correct. Know that I have also seen physicians check this reflex with other startling events like sudden/loud clapping. It will be tested based on the educated practitioner's preference. If this is something you do already, I'm sure you have found your preferred method. Thank you for pointing out that I could have spoken to the other methods!

I am so glad you found it helpful. There are multiple ways to test this reflex so I can imagine it is hard to visualize. This happens to be just one way that I find is successful in getting the response and I feel comfortable doing.
