Try the arcade port on the SNES instead. It's a fun game, but too much is compromised in the Gameboy version

You're kind of just not playing it right. Just grab the fruits and it's clear. If any enemy gets after you, you get a limited shot that takes a bit of time to regenerate. You can also use large smashing objects like digdug to take them down, chains even, but you'll get crushed too.

I thought Nintendo cabinets were the first made to change out the boards and art?

I think you got a little confused with this one. LOL. it's a pretty simple game, that's kind of like pacman where you generally are supposed to just collect the fruit and finish.
