Does Ozempic, Wegovy, and Mounjaro cause muscle loss?
This is the latest question that has been flooding my inbox. People are concerned & rightfully so! When we say, ‘I want to lose weight’, what we really mean is, we want to lose fat.
But, if we lose muscle instead, what is the point?!

Also, muscle mass loss can be mitigated by emphasizing protein intake and strength exercise. In my anecdotal n=1 case, I gained 12.5% muscle mass while on max dose of tirzepatide. So not all is lost ;-)

I’m on Saxenda and really focusing on strength training and protein intake so I don’t lose too much muscle so thanks for sharing your experience!

I’ve been putting on muscle and losing weight still! Come on get out there and workout if you are on the programme!!! I’m doing TRX… keep up your protein sources and as we need to chose when and what we eat… eat wisely

Well my personal experience is while I don’t know how much muscle I’ve actually lost I can’t lift like I could before. Im tapped out after half the reps.

It doesn't matter if you're losing 30% muscle vs 40%. You're going to lose muscle if you're in calorie deficit, as long as you are losing fat too it doesn't matter.

So, If I have a hundred pounds of muscle its ok for me to lose 30 to 40 pounds of it....? it will take years to regain that back.

@@mr.fluffy770 If you have 100 lbs of muscle then you are very likely over 240 lbs and you need to lose weight. The body isn't meant to carry too much fat or muscle. Too much muscle is bad too.

I’m a powerlifter but I am considering mounjaro, this is the main thing that’s holding me back right now. I have a large amount of muscle mass but I have a decent amount of fat on top. It’s so so tough to decide 😢

I’m in a similar situation to you. From the research I have done, it sounds continuing to train and eat 0.75-1g of protein per kg of lean body mass will minimize muscle loss. But keep in mind it is impossible to lose fat without a little muscle, just as it’s impossible to put on muscle without a little fat.

Why don't you just do a cut for 6 to 12 weeks and then eat only enough to maintain your weight? That's cheaper than being on a drug for the rest of your life?

I’m a powerlifters with a 1500 SBD and I lost 44 pounds on Zepbound. Unfortunately, I think some of it was muscle. My bench went down 85 pounds, dead lift 90, and squat 90. The strength loss happened pretty early on and continued throughout a six month process now. If I continue to stay on it, I may get even weaker. But for me, the fat loss needed to happen for health reasons.

​@@PaigeOnePicturesno. That's completely and totally 100% factually false. You can absolutely gain muscle whilst losing fat. And you can thus gain muscle without gaining fat. It's a complete training myth that you need to gain fat to gain muscle. Just Google 'maingaining'.

Mate, I’m on a low dose of 2.5mg and have put on lean muscle, in saying that I’m doing TRX as I was a former Army diver so I know it well… honestly I haven’t lost but put on muscle… as you do when you work out. Here’s some positive effects: I am sleeping better Craving for food are pretty much non existent. When I do feel like eating I listen to my body and it’s normally shouting PROTEIN!!! And salad. Due to the cravings dropping off I also lost interest in alcohol. Overall I have higher energy, reduced stress and I’m enjoying additional energy. I have lost around 9kg in 2.5 weeks

@@Cvreedy great!! I've had really no side effects, losing slowly and steadily!! And my walking speed and jog speed has increased with regular exercise, I noticed I don't get out of breath anymore with things like climbing stairs. Down 25 pounds this week.

You should always do resistance training while dieting. Make sure you get enough protein. Get 8 hours of sleep. If you do these things, you shouldn't have much muscle loss.

Also you have to keep in mind that with every cut/weight loss period first thing to go is glycogen and associated water in your muscle. For me that is 1-2kg that flushes away within the first few weeks. If you lose 4-8kg of weight that han easily be 50-20% of your weight loss. You get it back easily with creatine a slight caloric surplus after the cut.

You can still do strength training. In fact it can help manage. I'd recommend working with a Physio or athletic therapist to find movements that you van do.

I know someone on mounjaro. They eat a bite here and there and what they do eat isn’t quality food to begin with. This is probably going to lead to even higher malnutrition.

Isn’t MRI the gold standard for body composition? At least the most accurate, not necessarily most cost effective/least invasive.

You are correct an MRI is an accurate way to determine body composition albeit it measures volume vs. mass of adipose tissue - which can lead to over/under-estimations. Whereas DEXA scans have been demonstrated to be as accurate and reliable if not more so based on better delineation between various tissues based on density. Really it comes down to what is the goal? DEXA scans are the best for determining body composition and breaking that composition down into the various tissues present. It does not mean an MRI is not another option but we have demonstrated MRIs are better utilized for other goals ie. seeing if you have a tumor in your brain. Further as you noted MRIs are also cost prohibitive at present leading to a decrease in accessibility. As cost comes down and more research/better tech becomes available this could always change. Thinking about it if our only criteria for gold standard was the MOST accurate method possible - technically an autopsy with dissection and measuring the mass of each tissue type would be 'gold standard'.

I’ve seen where people who were on Ozempic because of the suppression of their appetite forgot to eat and only ate very little each day, causing the muscle loss

Building muscle via resistance training should be a corner stone with something like that. It helps to stabilize everything.

On Zepbound now. Wegovy before. Started in September 2023, lost 40 lbs...The muscle weakness is crazy!!! Fatigue for sure but the weakness is extreme. Am i working out? No. Too weak and tired! Help

I came to this video after seeing Naomi Campbell in a recent video. She looks much more thin and when I saw her legs it shocked me. In the past, her legs were thin but still shapely with muscle. In recent videos they look like sticks and it shocked me. Her legs used to be the best in the industry. Then I started looking up other celebs on Ozempic and their legs look similar... too unnaturally skinny. It's not looking right.

It doesn’t make sense that low calories and fasting would cause serious muscle loss. If our long past ancestors in a time of famine, were to lose their muscles, while waiting for the herd to reappear, they wouldn’t have the strength to break the famine and prepare the feast. Mankind would’ve disappeared long ago if this were the case. During times of famine, the body tries to prioritize the muscle, and use the fat, which has been retained for this situation. It doesn’t mean that you just lie around and do nothing, but continue to be active, eat A balanced meal with protein, and continue to exercise. The fat will come off, while the muscle will be preserved.

So its possible that the muscle loss is more affected by how fast the weight loss happens than the medication itself? So if people lose weight faster on a glp1 then it stands to reason that more muscle mass will be lost because of how fast it is

I’ve heard that these medication cause a general weakness. And that if you are active, these medications might not be for you. HIIT 3x a week here. I’ve had two friends be “as weak as kittens” related to this medication. Do we ever return to strength baseline? Why is this reaction rising- is it mitochondrial based? Physical activity balances my life and family. Energy and strength isn’t optional. What is the plan after a year?

If I had to choose between being thin and having big muscles. I’ll pick skinny all day lol. Weight lifting and muscles in a way caused me to be fat. An injury and all that muscle turned to fat in a couple years lol

Why is Mounjaro being down played in the market? Isn’t it doubley effective? Can you be sure to touch on whether these medication cross the blood brain Barrier. What is the half life. What is the plan after a year. I have three friends scares “to go off” despite that they’ve hit their goal weight. Can you discuss the loss of fat in the face and an aged look. Get real. Do you know people using this type of drug anecdotally?
