Going up the Purkinje the vectors goes in the opposite direction of D2 lead so it captures a negative wave..

if you take mobitz I and fragment with lost QRS there is no repolarization seen after P wave so this whole QRS obstructing view of atrial repolarization sounds like BS to me

At the end she mentions that depolarisation is when the cells become negative and the cells are positive at rest... Isn't this the other way around?

EKG measures the extracellular space, so it's the other way around.

She mentioned that atrial repolarization is buried in the QRS complex, but then why don't we see any atrial repolarization in rhythms like second or third degree heart blocks or in p-wave asystole?

Apparently it's a common misconception. I have come across articles that mention this phenomenon, and they say it's because the atrial repolarization is probably too small of an amplitutde to see on the strip.

The last part of the ventricles to depolarise is the left portion of base of the heart. So towards the end of the depolarisation of the ventricles, this left side is still polarised and electropositive but the rest of the ventricles are now depolarised and electronegative. And that causes a potential difference between the base and the ventricles and the direction of this potential is opposite to the lead II hence it reads negative potential.

PR Interval should be between .12 seconds to .2 seconds. longer than .2 seconds is a problem.

But seeing this surprises me that so many nurses (RN's) kept telling us they have "no knowledge of reading EKG's." Really? After needing to know all this in order to take the NCLEX-RN test? That's surprising to me.

how is depolarization negative and repolarization positive? depolarization is becoming closer to 0 mV while repolarization is moving away from 0

I believe your logic is correct. Depolarization is moving towards more positive, while repolarization is moving towards more negative. In EKGs though, we can only "see" or "measure" the outside of the cell. Thus, depolarization will show as negative on the outside of the cell (POSITIVE on the inside of the cell), and repolarization will show as positive on the outside of the cell (NEGATIVE on the inside of the cell. Hope that makes sense.

makingway1 my prof told us that when depolarization occurs (Na cells move in) that is an upward deflection on the ECG and is seen as positive charges moving towards the electrodes and that is why we get a upward deflection. That doesnt make sense since a depolarization is positive charges moving into the cell away from the surface of the body, meaning we should instead get a down ward deflection as the depolarization is moving towards the leads

Hugh Jazz What goes on at the cellular level is not always representative of what we can measure on the skins surface. That being said, you are correct, that if we were to put an electrode near the external surface of an individual cellular wall we would see it become negatively charged relative to the bulk of extracellular fluid during depolarization. The key bits being individual cell and bulk of extracellular fluid. What we have from a bigger picture point of view is a gradient of potentials with distance. This gradient gets even crazier when we go out the skins surface. At that point, all we can measure are potential differences across the skins surface. Depending upon electrode placement and polarity we could see the needle either go up or down during depolarization... so a series of conventions were created to avoid confusion. A the very end when she is talking abut the t wave, I understand why she has a - representing depolarization instead of a +. When cell depolarize, they are becoming more positive, so why is she saying the electrode is seeing the depolarized cells as negative?

+Marco Valtierra i'm agree with you, she made an reversion about depolarisation and repolarisation, but the explanation about the positive voltage fits because Lead II RA->LF= - ->+ it goes in the same direction depolarisation = - endocardium -> + epicardium and repolarisation= + epicardium -> - endocardium

There aren't really any BIG boxes on ECG strips. Every 5 of those big boxes (0.2 x 5 = 1 second) is a dash above the graph so you can easily see what 1 second is. But what she's saying is correct.
