It's always funny to me how many of the Ozempic peddlers will say, "Calories In Calories Out doesn't work/matter." But they don't even realize that the entire way Ozempic works is by reducing appetite so "Calories In" goes down.

Dr Oz was, at the Oprah discovered him, a legitimately good doctor. But a terrible example of quacks discovered by Oprah would be John of God. I don't think she ever apologized for putting the spotlight on that guy.

I love angry Layne too! If you listen to the science you can lose weight by obeying CICO. I am post menopausal. If I get 10’000 steps a day and burn 700 calories per day and eat 1,200 or less, I can lose weight.

It sucks to not be able to loose weight easily (less appetite control, less NEAT, etc). But honestly all those people saying that it's not within my control and that I should just give up are making me more depressed. Thanks for giving me some feeling of agency back Layne!

Hey Layne, I'm frustrated also as you are. I'm surrounded by people at work who are obese and eat more calories before 9 am than I eat all day. "I've a bone spur. I'll start exercising after I get special shoes I can't afford now", "it's just one sweet roll, then one cookie, then one latte" "It's the holidays, I'll start my diet when it's over, labor day- Halloween, Thanksgiving, Christmas, new Year's, Valentine's Day, Easter, 4th of July, birthdays bad days on and on" " What's wrong, my essential oils ain't working, I need detox,,,,,,,,,on and on and on,,,,,"

This. I know, ultraprocessed foods, bliss points, all that. But most of all, it's portion inflation: eating too much. What I thought was "normal eating" while growing up in a family of fat people was around 2-3x what is an actual normal portion.

Thanks Layne. I’ve lost 2 pounds in 2 weeks using your Carbon app. Now I get why it’s called Carbon hah. Got me some scales and been weighing all my food. You get used to it after a while. It’s like what you said, some foods a super satiating yet low calorie, while other meals are not that satiating at all yet super calorie dense. I understood the concept before but the process of actually weighing my food has been a real eye opener.

I think there's a misunderstanding around the statement of people eating whatever they want without getting fat. It's not that they can eat anything wihtout getting fat. It's that they can eat anything they WANT without getting fat. My girlfriend eat absolutely whatever she wants and she is 125 pounds for 5ft4 (approx 23% body fat). The thing is, what she want is around 2000 calories a day, after that she is force feeding herself. No she could not eat 4000 calories/day and not gain weight, but she can in fact eat what she WANTS and not get fat. She puts is absolutely no effort, her body is just intelligent enough to tell her to stop when she's not hungry anymore. On my side, even at 2800 calories (my maintenance) I'm starving. I'm comfortable at around 4000 calories. Even after years of eating healthy and maintaining my lost weight, it's still a struggle every day, while my girlfriend has visible abs without any effort. CICO is absolutely real for everyone, but appetite vastly differs.

Also your caloric intake on any given day is irrelevent, what matters is your weekly and monthly intake. On any given day I could eat up to 6000 calories and remain lean and give the appearance that I can eat whatever without getting fat. But the reality is that I just automatically eat significantly less the following days and by the end of the week the daily average wil be around maintenance.

​@@RickyVis i d say monthly is a bit overstreched as you could gain significant bodyfat in 2-3 weeks if you don t keep some accountability, that would mean you d have to get on a really strict and low calorie diet for the remaining week to even come close to net 0 surplus. I think the weekly average is best as its flexible enough for the majority of people while at the same time not giving enough time for calories and bodyfat to significantly accumulate

I think Mike Israetel said it best. We don t spend 24hours (usually) with the people who seem to eat whatever and however they want while still being skinny. They will tend to do a combination of more moving around, fidgeting, etc, as well as eat less or nothing at all throughout the day. They might 'quench' their hunger thoroughly in one or maybe 2 heavy meals and not eat much of anything the rest of the day

@@aisac21 Sure monthly on it's own is not a good idea. But using daily and weekly averages to calculate monthly averages is a good idea as it gives you a better perspective on long term weight loss. You don't have to lose weight every week, I mean it's unlikely someone gained weight every week of their lifes untill the point of starting their weight loss. All that matters is that you're in an average deficit over a prolonged period. Another good idea is to plan in caloric intake for the following week. So for example if there is a birthday or something next saturday you write in more calories for saturday and compensate by lowering it the rest of the days.

Thank you ❤ Well put! I was reflecting on the same thing. As a person struggling with weight issues, I don't question CICO. But, it is not really an answer about how to deal with hunger. Eating to either side of your "satiety window" is difficult for any stretch of time. I used to be like your girlfriend and not have to care about what or how much to eat. I felt satisfied on my maintenance calories (even though I had never counted calories in my life at that time), and I had went through three pregnancies. Medication for depression changed something in that balance for me, and now 10 years later (and not on any meds) I feel so hungry trying to eat at a proper calorie target, or even trying to just keep at a stable weight.

Thank you for caring about us. As an obese woman who is diligently trying her hardest to be healthy, the last thing I need to hear is I have no control and the only way to reach a healthy weight is through drugs or something else. I appreciate this. I feel like a narrative is being pushed to push drugs and I don’t like it.

I completely agree with everything you said. As someone who has experimented with various diets and now strictly focuses on calories in/out, I find the most challenging aspect, which I believe many others also struggle with, is accurately determining my basal metabolic rate (BMR). In my case, I tended to exaggerate my level of fitness and activity, leading me to overestimate my calorie needs and subsequently overeat. Although it wasn't a significant overestimation, it still hindered my progress in losing body fat as effectively as I desired. This misinformed mindset about our true energy needs is, I believe, a major obstacle for many people, including myself, in achieving our weight loss goals.

I totally agree! I used a calorie tracking app and was religious about weighing food but maybe lost 2 pounds in 1-2 months. I think I need to eat on the very lower end of an allowance.

@@jdawg414yep the best way to figure out your expenditure is to do weigh ins along with your tracking, you can see with great clarity how much you are actually burning each day by how much the scale goes down over time. The estimates are just a starting point.

It's registering food intake that's the problem for me. Consider Oprah apple pie at 11 at night, how much, as it could be 1000 kcal, did I note it down, for me pie is taboo as it is unaccountable food. Where for BMR I could be 100 kcal off, but it will stay totally predictable the same every day.

Im five foot seven 170 pounds. I literally eat like a horse. Snack all day. Carbs galore and gain no weight. It’s embarrassing how much food I can put away while my family stares in disbelief. Due to my work my annual average is over 20,000 steps a day. I’m a two legged furnace. KEEP MOVING!

The claim against CICO that breaks thermodynamics is the idea that one can eat less than their TDEE and still gain weight. Although I suspect metabolisms to be within 90%+ the same efficiency, eating a lot but not gaining weight doesn't defy CICO, and can be somewhat relative to each person. There are so many variables that go in like NEAT, portion size, etc. that it's really hard to tell what is going on.

CICO is a manifestation of the laws of thermodynamics, specifically the conservation laws in the context of chemical bonds. We are living in 2024 and some people still can’t grasp CICO! At first I used to think, it’s because of lack of science literacy in physics, but what I realized later is that a sizable chunk of my physics friends themselves don’t think CICO works (They use the ‘It doesn’t always work’ argument). This is disgusting considering the fact that a few of them come from Theoretical physics background and they are also doing “Research”. The real question is not if CICO works or not, but how one can design their diets in order to make the process of creating Caloric deficit/surplus easy and manageable.

Why bother worrying about what is causing them to consume more calories than what they expend, they just got to go CICO and everything will be fine and dandy.

The main issue why CICO doesn't always work well is because people assume it means calories eaten and calories expended but it's actually calories absorbed and calories expended and both are nearly impossible to measure with 100% accuracy and even a slight miscalculation can be the difference between losing weight, maintaining and gaining weight.

@@RickyVis That’s right! You brought up an important point. In order to overcome this issue, we don’t just set the trajectory of weight loss or gain as a single line, instead we set an uncertainty band around the expected trajectory line and make it in to a trajectory band. As long as we stay within this band during the cut/bulk/maintenance, it’s fine. It’s not only impossible to measure the Calories absorbed and Calories expended accurately, but even the Calories in the food. Despite this, tracking food Calories is the best we can do. On the other hand, the Calories we burn is the hardest to keep track of.

@@RickyVis Now I'm curious, how exactly are you "absorbing" calories that aren't being eaten? And no, a "slight" miscalculation doesn't make the difference, unless for some stupid reason you *assume* you can calculate it all exactly and think a 50 calorie difference per day is the key to losing or gaining weight. In practice people who try to lose weight can safely calculate a 250-500 calorie deficiency and lose weight, once you start monitoring loss from there you get a better idea of where you're at and you'll be fine. There's very little accuracy required.

@@Melesniannon Other way around. Just because you eat 'calories' doesn't mean you absorb them. So when you eat a 100 calories of potato chips you absorb a completely different amount than if you eat 100 calories of broccoli. And how is your 250-500 calorie deficiency going to be accurate when food labels can be off by 20% and you have no idea what amount you're actually using in a given day? It also doesn't help that in the US for some whacky reason things are measured in 'cups', I mean wtf is a cup?

A friend of mine ate crap and was stick thin / lean, two things I noticed about him, he had good muscle tone and when he ate his crappy foods, his appetite rapidly went to zero. This person was gifted good genetics for muscle mass, to keep his metabolism high and act as a store for glucose and also had a very sensitive gut / brain hunger axis. He obeyed the laws of thermodynamics, the reason for his leaness was him subconsciously obeying the law of thermodynamics better than most people, via genetic factors linked to energy sensing / appetite control. If technology allowed for an accurate, instant readout of how many calories we have absorbed versus how many we have used for metabolic process, only the very stupid or mentally unstable would become morbidly obese, but as we have no easy way of knowing our energy balance on a hourly / daily basis, many of us tend to gain weight, as it's easy to gain, but harder to lose.

I think part of the problem is that people (like the woman in the video) confuse the principle of CICO with calorie counting. Yes, calorie counting won’t work for some people. That doesn’t mean that CICO isn’t still in effect.

Let me see if I got this right. They're saying CICO doesn't work. They're saying ozempic does. Ozempic works by lowering hunger, therefore you eat less calories. That argument is as dissonant as my dad talking to me about hygiene.

Why are we surprised? This can be a very attractive argument to someone who has struggled with weight and body image their entire public life. There’s also a lot of money in this view. All these people are doing is creating content geared largely towards women looking for a group therapy session.
