Hey everybody! Today we’re talking all things Oprah and her beliefs on weight loss. Please note that although the concept of weight loss is simple, implementation can be incredibly challenging in our daily lives. Thank you for being willing to make a video that may not be monetized. It shows your dedication to fighting misinformation which is so desperately needed right now. I can't say I'm surprised by what Oprah said but damn, she has access to so many things I would only dream of and is still this clueless...

It certainly makes sense to frame issues around overeating as a disease because in many ways it does involve a kind of addiction. One thing I heard from a fitness RU-vidr is that the distinction for overeating from other diseases is that it is the only disease for which you will forever have to consume some form of what you are addicted to. Imagine if alcoholics had to drink two beers but no more than that every single day.

I got into an argument with my husband about eating late at night. He thinks that it is the worst time to eat and that you should not eat after a certain time, which I explained is not scientifically correct and he wasn't hearing it lol. We got into this argument because my teenage son is overweight (he went to the doctor yesterday for his annual wellness check and it did go well and his weight has put him in the red zone) I did explain to my husband that the problem is that he is in a calorie surplus, regardless of what time he eats. My son and I have set up an actionable plan for how we can get him healthy as he already has high blood pressure and getting his blood panels done Friday. We are going to walk together outside 45 minutes everyday and strength train together. We have discussed how we can eat to better fuel our bodies. We talked about getting more fruits/vegetables and protein into our meals. Hopefully I am doing right by my son. I am not overweight but I am doing whatever it takes to support him

I love your action plan and that you will be creating healthy habits together. I have an 11 year old son and we often go for long walks together and it’s the best. It’s been such a good bonding time for us and he opens up and tells me all about what is going on at school, with his friends, etc. I hope you and your son love it as much as we do.

@@valerieodonnell6764 thank you so much! I think it will be great for us to get out in nature together. We are very fortunate to have amazing walking trails and mountains by our home in Utah. We have a treadmill, but my son prefers to walk in nature and it will be great for both of us and out dog

Being obese is all about mental health for soooo many people. We can know all about the technicalities of how to lose weight but it's addressing the why of our food or eating fixation that's the issue, and makes it incredibly difficult to start or commit to behaviors that help with weight loss (calories in, calories out etc). My goodness why is this psychologist up there trying to disprove how energy works when they could be leaning into their actual specialty and discussing how many people could benefit from healing their minds which will then help them heal their bodies. As an obese person myself, the wasted opportunity there with such a huge platform is so aggravating lol. Thanks for your vid... I appreciate your work!

Person-first / identity-first language are both fine, depending on the person's preference. Some people, myself included, prefer identity first because we believe that person-first stigmatises the disability and treats it as something that can be considered separate to the person, rather than something that permeates every aspect of our lives and is very interlinked with who we are. I definitely don't mind which you say - both are valid - but I do think it's ableist/misinformed of this psychologist to pretend like there's a consensus in this space

IMHO- the people who claim or are assumed they eat whatever they want and don’t gain weight are either extremely active or don’t eat the high calorically dense processed food we think they eat.

Yes! And we also have to remember that our perception is EVERYTHING. Jordan Syatt's book talks about this really well, i highly recommend giving it a read :)

Yes, or they don’t notice that whenever they really eat a lot they naturally eat less the next days because they are good at listening to their body’s signals. Or they don’t notice that they eat less meals than others. I always thought I could eat tons of food, but I’m just very active and don’t eat much in between meals, plus my breakfast and often lunch as well is low in calories because I only have a big appetite in the evening. I guess that’s how it is for most people who can “eat whatever they want”.

I love Oprah for many things she’s brought to public consciousness but I do think she has to check her ego sometimes when talking about things she really doesn’t know anything about. She also seems out of touch with the “average” person living in the world- I mean why wouldn’t she be, she’s been insanely rich for many years. I heard an interview of her several months ago and she said she’s NEVER BEEN TO THERAPY. I work in mental health so perhaps I was specifically irked by her sharing that. I mean it’s obviously her prerogative but my gosh…she has shared about significant trauma she’s experienced in her life and has also given life and spiritual advice to millions of people on her various shows over the years- not to mention she could clearly afford a quality therapist- and she’s never seen one!?? It made me lose respect for her, honestly. It felt like she wanted to have a platform to “tell others” how to live their life but hasn’t deeply examined her own.

As soon as I saw her new weight losss, I had a feeling it was due to Ozempic. Oprah did introduce Dr Oz and Dr Phil so there's that. When Oprah says something, it is still seen as 1,000% accurate. Also Kelly Clarkson was on an add touting some rice and apple cider vinegar thing that prompted her weight loss. I like Oprah and Kelly but this makes me shake my head. BMI is not a real measurement either but medical community uses it as a tool. Sorry for the rambling. I was a registered dietitian and a personal trainer/fitness instructor and the amount of misinformation is amazing. MOST people eat more calories than they realize. Obesity IS a disease but it is a mental disease and a calorie surplus disease!! THIS is why I became a Respiratory Therapist after 28 years in the industry because the steroids and diet bullshit were getting to me. Thanks for carrying the common sense torch!

Eh, hormones can be a real problem especially if you are being too restrictive causing you to mess up your metabolic rate. When I was in my worst of calorie restrictive dieting I was down to only 1100 calorie a day but would struggle to lose weight. My issue was trying to lose to much weight a week over focus on building healthy lifestyle changes. My hormones had me at a lower metabolic rate but rather than focus on building health habits that were sustainable. I know that I will have to work harder to do a substantial weight loss. (I have PCOS and Fibromyalgia by the way). Though the worst weight gain I have ever had was when I was under extreme stress due to a new job. So I think in the way people throw the word hormones around you are right but some people genuine have disorders which are now to make weight loss harder. The irony of alll of this is the more you weigh the worst I battle symptoms of my disorder. But I am not disagreeing Calories in versus Calories out is a huge factor to of course! But for me I know how to do the nutrition but I am one of those who struggles to find time for it all. Eating healthy can be a full time job at times! My doctor recommended starting with just focusing on one meal while we work to stabilize my fatigue and it was the best advice anyone gave me.

Also, I love that you actually acknowledge the nuance of this all. I feel like so much of this talk and turn into “personal responsibility” but then it’s also not entirely just “completely out of our control”. But the control we have is more impacted by socioeconomic factors/our environment/systems/etc and not just “our brains are different. Nothing we can do”.

I think this video is actually horrible misinformation. Eating at night is TERRIBLE not just for gaining weight, but for the other reasons like terrible digestion, GERD, ulcers…etc. Also have you ever heard of the concept that the gut is your second brain? These two organs, your brain and your gut work together, its not just “fast metabolism” when people can eat whatever they want, and exercise a little or not at all and not gain weight. How can you create a video claiming these people shouldn’t be talking about diseases, and then claim that you shouldn’t be talking about it either…but yet here you are posting this? These GLP-1s are not just helping with obesity, they are help a whole slew of issues. If you’re going to make a video about this subject, why not talk about it as a whole and not just the part that makes it seem like you’re literally shaming the concept (and proven fact) that obesity is a disease?

Recently my husband decided he wanted to lose weight and then put on muscle. His Instagram algorithm figures this out and starts feeding him gym-bro and quack nutritionist content. He got SO confused, every day he thought maybe he was doing it all wrong, and that makes it impossible to achieve the consistency REQUIRED to lose weight sustainably. It's so frustrating, I can't imagine how you feel on this platform.

The "can eat whatever they want people" if you track them over weeks, whatever they want balances out. They don't want to overeat. If they over eat for a meal here, they will under eat later. Thank you for bringing up the food chatter thing. If you never have this, you will never understand how hard it is to have your brain so focused on food all day long that you will eventually not be able to take it, and you just binge to make it stop.

I agree with the premise of this video but categorically disagree that a Psychologist has no place talking about 'disease'. Many, many psychologists practice clinically and an interdiscplinary approach to disease treatment and management is increasingly being considered best practice. Dr. Rachel Goldman is a Clinical Associate Professor at NYU medical school who specializes in weight management and disordered eating and if you browse her recent publications several of them are on the mechanisms of food addiction, so it's actually NOT inappropriate for her to discussing obesity from a disease model. I enjoy your content but diminishing credentials and downplaying the important role of psychology in obesity/management doesn't sit super well with me.

Not here to hate on you but just want to highlight that I don’t think you’ve checked he education history before making this video and I agree, it’s unfair to dismiss her the way you did. Dr. Rachel has a post-doctoral fellowship with a specialisation in obesity and baristric surgery from a medical university so I think she would understand energy balance and the nutrition side of things. I think her expertise is valid as long as she’s not prescribing or advising - which she wasn’t doing. Although you’re right in that her arguments were quite superficial, I do think she has the education to back her specialisation in obesity and health behavior change.

For years, I struggled with anorexia. And I was told over and over, it's not much fault and my brain chemistry was changed by starvation. I fell I to a victim mentality. I've been recovered for nearly 20 years and why I agree I did not choose anorexia, I had to come to an understanding that my behavior had to change first. That allowed me to think more clearly, and realize I didn't have to believe every thought that ran through my head.

I would say it’s very mental. It is easy to say eat less but then your body is telling you that it is always hungry. I am trying to take care of myself, health and lose weight, now that my kids are a little more self sufficient.

I read a book speaking of dis-ease, versus disease. The American culture promotes dis-ease within the body bc our food supply has been made into a science experiment, we are stressed from working like mules, and more sedentary. Therefore, our brains can’t send out the proper signals for satiation, or we just eat emotionally. Once I really figured that out, We started spending just as much on quality food as our rent. And we are more active, even if it’s just getting our steps in. We can’t travel as much etc, but our physical and mental health have improved. I know not everyone is as fortunate, but we sacrificed for our health.

Hi Justina, just wanted to give my thoughts, obesity is considered a multi factor disease, meaning it doesn't only come down to weight and bmi. It is supposed to be treated from many sides, one of them beaing mental and psychological implications. So it is actually appropriate for a psychologist to be talking about the topic, even though in this specific case I don't agree with these "experts" Hope I made sense, thanks ❤

Totally! And I discussed exactly that in this video. What she shouldn't be explaining is calorie deficits like she does in this interview because she's completely wrong. It's about staying within your scope of practice.

I can see why she had a psychologist in the panel… In my case Obesity was sort of tied in with my ED. I went to an Obesity Clinic at Children’s Hospital and they had patients seeing Psychologists.

Absolutely makes sense! And like I said in the video, there’s a lot of great stuff a psychologist can help patients with. But she doesn’t understand the basic concept of energy balance, so she shouldn’t be discussing that. The same way I wouldn’t act as a therapist.

The psychologist should be talking about the role of trauma with disordered eating .... where do these issues come from? I believe that Oprah has a history of childhood sexual abuse. They just are working on the symptoms, not looking at the source .

This video was very hard for me to watch. Not your fault, obviously, in fact I suffered with you and for you all throughout, but when that psychologist started rambling about CICO not being true for everybody, I died inside. My sister, who's being struggling with weight gain for decades at this point, tells herself (and others) the same story whenever someone mentions diet or fitness-related terms: she says that CICO is not true. And the most tragic and deeply unsettling thing is that she actually has a master degree in chemistry. A master degree. In chemistry. Okay? Good thing we're not American nor English speakers, otherwise she WOULD totally use Oprah's takes to validate her "science". When you say that there's a huge issue with the way we are getting our basic fitness/nutrition education you are absolutely correct...I don't know how these things keep on happening, but they terrify me, especially when there's a concerted effort to actively keep on spreading misinformation going on...

This is the deal. When people struggle with weight (which I do...not a lot but I carry about 30lbs more than I should), the mindset of "it's not my fault" is overwhelming. You are searching everywhere for a reason that doesn't have to do with how you eat and move.

I feel like she's promoting an excuse to her audience for her using Ozempic and not being realistic and making the necessary long-term changes needed to support fat loss. Ozempic is no magic pill and everyone who doesn't change their habits will change in the long run. As you mentioned, nobody can escape the laws of thermodynamics!

I love your videos and your workouts! As someone who’s on a weight loss medication, I do want to say that, for me, it ended my nearly uncontrollable cravings for sweets and binge eating. I think this speaks more to the point of psychological/hormonal eating issues, but for me, it’s been a life changer. Im actually able to lose weight and shut up the food noise you mentioned in the video. As an added, unexpected bonus, I actually want the healthier foods I “should have” been eating all along. Thank you for the video!

I feel like everyone says “healthy food is not available to a lot of people”. People can eat the same food they eat now, just less of it. Instead of two plates of food, eat one plate. That may be enough to get someone into a calorie deficit. Eat the same food, just less of it. And also frozen vegetables and canned vegetables are at all grocery stores.

I wonder if you have poor genetics/missing receptors or deficiency's,.. are you able to break it down as effectively? I know water with some of its contaminants (lead, and other metals) are related to increased obesity.

My mom was always so backwards about weight loss too. I have been underweight my whole life. I just didn’t want to be anything like her. My mom idolized Oprah too so I always thought she was a joke. I am 42. I don’t believe in the chart either, I eat healthy exercise and I feel better 5 lbs under weight according to the chart.

I really needed to hear the ‘you can eat at any time.’ Cos in my little recovered settled mind, I was always a lil unsure ab nighttime eating. As someone who eats, sometimes, up until 12 or 1am. And whose family says eating past a certain time makes you fat (obviously nonsense but 🤷🏽). So. That’s good to know ☺️

For those women out there in/post menopause who may be feeling really after listening to this...hearing her say that weight gain is not about hormones...take heart! This has been my experience, with my daily data for 7 months to prove it. When I was Justina's age....absolutely calories in / out philosophy made sense and worked! Smaller portions of any kind of food. Calorie deficit. Also doing lots of cardio caused weight loss. HOWEVER--after menopause (about age 51ish for me) , this did NOT work. The inflammation was unreal--felt like a boiled sausage ready to pop! (I know you say hormones are no excuse, but...) I did all those things and kept gaining. I just could not figure it out. Tried being vegan for about 18 mo and lost weight, but felt and looked terrible!! September 2023 I started more animal based-low fat eating, meticulously weighing & tracking my food/calories, kept a daily journal, weighing myself daily, getting in a good (400-500) calorie deficit (I'm 5'7"), increasing protein, cutting out processed food, ate low carb, and did start losing 3-4 lbs a month. (Maybe only 2-3 times ate slightly over my calories this whole time.) I started strength training but also did some cardio. I got to a stall for about 3 weeks in Jan-Feb. I then took out my plain nonfat greek yogurt and berries, stopped HIIT, started moderate walking for my cardio and started losing again. I was eating lean animal protein only and some non starchy veg. But not sleeping well, skin dry, had low energy. BIG change 1 month ago: lowered my protein (to about 80-00g/dy), only non-hormone, fatty meat, no dairy except butter, no seed oils, increased my animal fat intake significantly (AND my calories)...I lost about 10 lbs in March and feel better than ever. Inflammation gone! Sleep improved, stress lowered, energy, skin also. My calories are massively up via the fat--but my body is really changing now for the better. Have only vegetable / fruit a few times per week. I have all my journal entries and dates to prove all this. I was eating roughly 12-1300 calories/day for about 5 mo....now I'm eating about 1800 and losing faster. Maybe it "doesn't make 1st law of thermodynamics sense"....but it absolutely is working for me at age 62 :) I am 10 lbs from my goal weight of 130 at 5'7". Hope this helps someone out there to at least not feel like you're crazy ;)

Hey! I never said hormones “aren’t an excuse.” Everything you’re describing is exactly what I said. Hormonal changes can affect and change your resting metabolic rate (as well as many other things that this video isn’t about.) So you didn’t defy the laws of physics and the countless studies that support it. You changed your metabolic rate.

I think it's a bit unfair to say that a psychologist has no business talking about obesity. I mean, obesity isn't an isolated subject. We all know how hard it is to lose weight and if it it were as easy as "calories in calories out", then nobody would have a problem. I'm not saying you don't need a deficit, I'm saying it's very difficult to get that because of very deep and conplex psychological (as well as many other!) aspects. Health psychology, for example, is a huge field especially looking into behaviour change for physical health. An example of that would be psychologists being involved as a part of beating HIV in the beginning. How do we understand the mind and motivate people to behave in a way that directs them to health or prevents illness? Don't get me wrong, Justina. I'm not trying to attack you at all and enjoy your content a lot. I just think this comment is a little short sighted. Otherwise, thanks for all the great content. Looking forward to the next one 😉

I think what is grossly missed by the Phycologist is the mental health aspect of obesity. If you've ever watched an episode of My 600 pound life, so many times the patient benefits from therapy while working on the "calorie in - calorie out" of dieting. The mental and physical are linked (case in point extremes like binge eating or anorexia). Even as a person who doesn't suffer from those extreme conditions, I know that stress makes me eat more. Stress and anxiety make me "snacky" - particularly chocolate in my case. I was my own self-reflection that lead to me understand that. To see how I would unconsciously snack when a stressful event happened. So yeah sorry, I do feel mental and physical are linked. I don't think the doc on this show does a good job and explaining that (if that was her goal at all). We can all count our calories and exercise but if you're not mentally in the game, its not going to ever work.

Really tough subject to tackle. Especially since obesity affects such a wide demographic. Just wanted to share my experience :) I personally struggled for about 4 years after my daughter was born and finally learned my hormones were working against me. 120lb 5’6” my whole life but I ballooned and gained 50lbs in less than a year it got up to 200lb at one point. I’ve gone on fast after fast and calorie counted to lose 2lbs in a month next to my partner who did the same thing, larger portions and lost so much. Yes I calorie counted, yes I worked with a nutritionist, yes a cut out alcohol, yes a lifted weights. Turns out if you have high insulin high cortisol and chronic stress this advice has to wait until that’s under control. The food noise was insane, I woke up thinking about food, went to sleep thinking about food, insane cravings despite eating keto (that didn’t last long) - body never got into ketosis. Changed where we shopped went organic, cooked all my meals. I didn’t put anything in my mouth unless I tracked it, mentally the roughest 4 years of my life. Turns out it wasn’t my fault it was my hormones and medication was the answer to help my body better metabolize insulin. I don’t feel puffy/bloated, I don’t feel food noise, weights coming off steady and slow. I’m grateful. So yea there’s just no one size fits all advice everyone’s experience is gonna be different.

I was obese since i was a kid and struggled with losing weight until I actually took time to understand nutrition. Once i got my eating habits under control, prioritizing nutritious food, reducing processed food and understanding triggers that caused me to use food as a coping mechanism - I lost almost 50 pounds in one year and I am at a healthy weight now. Not fully there yet, but close. Maintaining for 3 years now. I am not saying this to toot my own horn but to share that before I took time to understand nutrition, I would have been ripe for the kind of message Oprah is selling - as I had repeatedly tried and failed to lose weight. My entire frikkin life upto that point. A pill to make that problem go away would have made me sign right up, damn the long term consequences. But our health is more in our control that we realize. Imo, what Oprah is peddling is downright dangerous. I won't even go down the Big Food, Big Pharma rabbit hole. A whole other topic.

My problem with calories in, calories out isn't that it is inaccurate, it is that it is presented as a simple solution. It is so complicated, however, because equations calculating calorie expenditure cannot cover every factor that affects metabolism and so are estimates at best. Most of the equations I have seen use height, weight, and age, and if you believe the calorie calculators are infallible and see someone else the same age, weight, and height, eating the same food and exercising the same amount who is losing weight faster than you, it can be difficult to feel like weight loss is only CICO. And if people understand that things like sleep, cortisol, hormones, lean mass, etc., affect your metabolism, they can be empowered to learn about other factors outside of diet and exercise that they can try and adjust or control. Saying that weight loss comes down to calories in, calories out, while technically true, is a simplification, but I believe that saying it is not just CICO (while technically untrue) is another simplification, which can lead to a more nuanced understanding of human bodies and better ownership of one's health.

It's VERY difficult to be exact, but the best way to is to find a few ways to measure progress! Scale weight, how your clothes fit, physical measurements. If you want to track calories, I would start first by tracking what you typically do for a week. Take the average and then subtract from there. If you want to do it more intuitively without calorie tracking, I'd track your portion sizes of protein, carbs, veggies, and fats through the week and moderate your portions after that.

Hold on. When the psychologist says, "It's not calories in and calories out," that's not really what she means. That's short-hand for what I assume her argument will be. YOU are wrong. It is not EXCLUSIVELY calories in and calories out--or shouldn't be. The problem that dieticians and weight loss scientists have is the same problem economists have when looking at economic systems. PEOPLE ARE NOT LOGICAL ROBOTS. People deal with emotional flux. The question is, how can someone who is obese MAINTAIN a fitness (diet plus exercise) routine that is SUSTAINTABLE FOR THEM OVER THE LONG-TERM (and possibly for life, more or less)? This requires motivation and discipline, neither of which are easy to maintain--and some life-hack experts feel are not real anyway so you have to approach them in different ways that expected (or that even some psychologists think (wrongly)). So yes, you are correct that the primary goal should be on reducing energy consumption and increasing energy expenditure. However, that is not the ANSWER. The answer is for the INDIVIDUAL to work out how to (a) achieve those two things, probably in tandem; (b) maintain them over time for a very long period of time and possibly life (with some adjustments)--in a way that works for them. This requires a system that includes MENTAL forms of jiu-jitsu to deal with psychological (and physical) obstacles to developing and mataining that system WHILE ENJOYING THEIR LIFE. In fact, a solitary focus on calories tends, more often than not, to self-sabotage and hopelessness, which can lead instead to weight GAIN. You cannot do the physical without the mental.

Yeah, as someone with multiple chronic illnesses, I can say that I know things like inflammation in the body and hormone levels CAN affect our weight/how much water we are retaining/how the body looks and this can totally make it SEEM like the calories in - calories out model isn't 'real' to an extent. Because again our body weight isn't just fat - it's also water, muscle, bone - and again, sometimes being able to control our weight really IS something that's more challenging given different factors. Also you're so right - the TYPES of food we eat are also going to affect our appetite, how much we are PERCEIVING we are consuming vs. how much we are actually consuming, and again I do believe that the quality of our food is going to have an effect on SO many other body systems, which can play into our metabolic rates, levels of inflammation, levels of water retention and so on and so forth. So in a way it is cut and dry/simple, and in other ways, it isn't for a lot of people. But overall, for the average person, I think you're likely correct - it's just really difficult to accurately understand how much we are burning and how much we are consuming in terms of the average population, because most people dramatically under estimate how much they are eating and how much they are moving/burning. What things like this do is obfuscate the real issues - which I think is really what the problem is here. This doesn't really empower people to better understand their bodies, food systems, to better be able to change their lifestyles in ways that are actually accessible to them and so on. It comes across as being more diet-industry non-sense that leaves people feeling like they have some sort of control when in reality it takes people farther from actual solutions. Because I believe that people like Oprah and many of the people on that panel are just out of touch with the lived reality of MOST PEOPLE. This didn't' address the actual issues people are facing that cause them to struggle, as you so aptly pointed out, and thus, it's just another distraction in the long line of confusing distractions that the diet industry is.

It looks like, similar to Joe Rogan, she is increasingly prone to believing hype over substance, drawn in part by the now natural desire to seek out clickbait material but is now finding it second-nature to believe it herself so as not to feel like she's hurting followers.

This is in line with people aren’t fat; people have fat. Regardless of the reason for the problem or the approach for treatment/change, everyone is worthy of respect. But you can’t successfully make change, if you don’t accurately identify the problem. I really don’t understand Oprah pushing weight loss drugs though. Oprah ran a marathon, and when she did, she was in phenomenal shape.

I remember in the 80's when people were taking phen phen, essentially speed to lose weight; and they did, but as soon as they stopped all the weight came back because they never learned how to eat (portion control & adding movement). There is such a lack of basic knowledge about how our bodies work...thank goodness there are people like you that can break things down and help people understand more about health and fitness. xo p.s. About 90% of my clients are females, and oh my God, you are spot on!! These are the things that I have to explain over and over again. Seems like the pattern is the same, they recieve advice from a friend to fast, or the body type or the diets, etc, and i always have to pull them out of that mentality. I for one really appreciate when you use gender neutral language :) It makes your channel feel much safer for someone like me. The person first labeling is highly individual. It comes from disability research and is often rejected by actual disabled people and I know within the fat community, it is also contentious.

I’m not very far into this but why when I know where this is going do I have a shred of hope that maybe the psychologist was about to talk about behavior still?? Like for SOME things a therapist could help with eating behavior as it relates to things you mentioned like not eating enough during the day and making decisions at night that put you in a calorie surplus every single day leading to weight gain. But alas that was not where she went.

I feel incredibly confused 😭. I did not watch Oprah's special, because I knew it would be sketch. However, I understand why she lands where she does. I feel completely helpless re: my own weight. I have complex health issues (EDS, etc). I take meds that possibly cause weight gain, and I probably can't stop taking them. How does CICO come into play with meds that cause weight gain? But also, I don't overeat (I try to stay at 1200-1600 calories). Maybe that is overeating? But again, I get confused, because that could be considered undereating? I easily feel massive shame for how many calories I do have (even if that's 1200 calories). I'm 5.6 feet, 186 lbs and most of my weight is around my belly (my legs and arms are pretty thin). And I don't know how to measure my metabolic rate - especially with all of the complex factors of my health. I don't feel terribly confident that anyone can at this point, because my health is complicated! I don't even know how many calories I actually burn, because I have POTS (postural orthostatic autonomic syndrome). My heart rate can be overly elevated, or it can even be sluggish. Just because my heart rate is fast, it doesn't mean I'm burning calories. But I can't exercise with vigour on bad heart rate days, because it's getting dangerously high. There are days right now that I can't even stand up, because it's dangerously high, and I might need to go to the ER. Sometimes my resting heart rate is 160bpm (obviously this is getting thoroughly checked by a cardiologist - I'm very closely monitored). For the last several months, I've been paying close attention, tracking my calories, upping my protein, and exercising as faithfully as possible given my health struggles (4-6 times a week, for an hour and half a day). But I am not losing weight. However, I was sick for the last couple weeks, and I struggled to eat...and was eating 400-500 calories a day, and I did lose weight (I did not exercise). I did lose 5lbs (this might not even have been fat loss). But quickly plateaued. This isn't a safe option either...but seems to be the only thing that ever helps me to lose actual weight. I have bouts of gastroparesis every now and then, and that's what happened the last couple weeks. Nothing would go down, and everything I ate, almost came back up again. I live on brown rice congee when this happens. There are times when this lasts for months, and I do lose 20-40lbs in these seasons. BUT it's not at all a healthy way to lose weight. When I do eat a bit more than I should, it's not usually because I'm hungry, but because I'm faint, and if I don't eat, I will pass out (literally black out). I have some blood sugar issues and some very low blood pressure issues, which both make me feel faint. So when I am eating, it's to keep me upright. And so this can cause me to overeat a little bit (getting to 1700-2200 calories a day), just so that I can survive a busy day. It's usually some kind of protein that I try to eat on these days. I definitely gain weight when I eat this many calories. I feel confused because every single doctor is unhelpful and judgmental. They tell me I'm morbidly obese and must lose weight. They assume I binge eat, which I do not do at all. I follow protocols with dietitians or nutritionists or personal trainers, and they get confused too, because I gain weight (and I have been following all the things correctly). And so many of them put me on crazy diets, that had me restricting healthy foods, and did not consider my unique health struggles. For example, a Keto diet kills my digestion (wayyyyy too much protein and fat). But all the doctors/nutritionists have put me on Keto (and other diets - I have probably done them all, following a doctor's counsel). There's no one on my health team that can answer any of these questions, and sadly, they just traumatise me, and make horrible assumptions about how I eat. They look at me, and assume I eat horrifically. But because of a myriad of food sensitivities, I can't eat processed, high sugar, high carb or deep fried foods, even if I wanted to! I mostly live on homemade soups, gf ramen or brown rice and chicken breast. And I'm careful to not overeat the ramen and rice! I feel so defeated. No one can give me answers for why I gain weight so easily (even in a calorie deficit) and why I can't lose weight (unless I'm basically starving myself). I've never lost weight in a healthy way. It's only been during periods of gastroparesis, when I literally can't eat much at all (and I'm not able to exercise at all during these times, so it's terrible for my heart health and my joint health - I HAVE to have regular steady state cardio and gentle strength training just to keep mobile because of the EDS and POTS - otherwise, I'm literally in a wheelchair). I've just been working so hard and carefully, trying to be balanced and not do any crazy fad, unhealthy diets or exercise regimes. But even though I am working so hard, so faithfully, I am not seeing any results and it's so discouraging. Anyway, sometimes it doesn't seem as simple as calories in, calories out 😥 . I obviously know my metabolism is a wreck, but I feel so confused on how to remedy it with chronic health issues, and I feel so confused about how to lose weight (for the sake of bad knees and longterm health). I have to exercise, which means I do need to eat more than 500-800 calories a day (because I'm far too weak to exercise safely at that intake, and clearly, that's very unsafe period). But I won't be trying any of these weight loss drugs either. There seems to be evidence that they can cause gastroparesis, and I already have that issue, so they would be a dangerous option for me. I am very discouraged, and feel very judged by many people around me (especially my health team).

I am thankful for your video though! I just feel frustrated with my body, and my medical team and how much pressure is on me to lose weight or I might die (in their words), and how helpless I feel trying to accomplish weight loss! I am thankful for your videos because you speak honestly and don't spew nonsense (which is what I get from a lot of different professionals). I wish there were people who could speak into complex situations, where there are numerous health issues at play, and not just shame them and scare them (so this is why I said I understand why Oprah lands where she does - it's hard to land in a good place with that much public shaming - but this in no way means I agree with where Oprah lands on this issue). I also have endometriosis (hence drugs that cause weight gain, but I can't afford to stop them - I'm in debilitating pain otherwise). Sadly, the endometriosis causes SEVERE bloating (as does the EDS). It's very hard to track progress due to the extreme bloating. Anyway, it's confusing to be told by professional doctors that I'm overeating and binge eating, when they NEVER EVER ask me to write down what I eat in a day. I do track carefully, and I don't binge eat, and it sucks that people assume that I do. It is very disorienting. But I do really appreciate your videos. You might not be able to speak to complex situations like mine, but I'm thankful that you're able to help the average person from falling for fads and crazy diets, etc.

Ohhh...and the medication that I'm on, the pharmacist says it's unknown why it causes weight. Some meds cause more hunger, leading to more food intake, but that's not the case with the med I'm taking. It just seems to cause weight gain. More research needs to be done. And who knows if it even is a factor for me? It might be my other health issues that are the problem. I'm also on a number of other meds that can also cause weight gain. I do wish that's what Oprah had talked about lol! All the meds that doctors prescribe that can contribute to weight gain, for a variety of different reasons (and not always just because they are eating more). Haha...maybe they did touch on this subject! I didn't watch it lol. I have multiple doctors, and none of them ever consider which medication they put me on - but they all shame me when I gain weight. I'd love more information on how to combat the weight gain that meds can cause, or at the very least just admit that there's not much some people can do, especially if they cannot go off their meds. Stop blaming the patient! That would be a lovely start to the conversation for those with health issues. But obviously, certain meds I'm stuck with, whether I like it or not...

Hey Lynette! I’m so sorry you haven’t felt supported by your current team. It can be really tough to find the right doctors who are going to bat for you. I think first off, is the weight you gained from the medication impacting your health and life negatively? And if so, I would recommend working with an RD if you can. They’re much more equipped to deal with the nutrition and habits side of things. I know I’m not much help, but I really appreciate you taking the time to watch and share your story ❤️

I haven't watched any clips from Oprah's special, but based on your reactions in your video, I feel that you have a blind spot understanding an aspect of obesity, which is the mental health aspect, and that perhaps the obese brain is "wired differently.". I lost close to 90lbs about 4years ago with the important tools you mentioned: nutrition, exercise, other healthy habits, etc. Pretty proud of this! But I can also tell you that throughout those 4years and even now, I have periods of overeating and bingeing because my mental health is not the strongest or I'm triggered. The food chatter and noise in my head have never gone away, and I feel hopeless that it might always be like this. Also, one of the things that's fairly common with a lot of obese people is personal traumas (childhood, sexual, financial trauma, etc.). You just need to watch a couple of episodes of My 600lb life to know that these patients have experienced some serious trauma in their lives, and that is probably the primary driver for their obesity. I think your reactions and advice are all common sense. But it also feels like a response from a privileged person in a smaller body by also saying, it's just CICO and eat less and move more. Yeah, I know that. Trust me, every obese or fat person knows that too. But there is something else going on that makes it much harder to treat. I don't know if taking drugs for the rest of your life seems like the best answer. But if we do think that obesity is a type of disease--mental health disease? obesity as a result of PTSD?---, then why shouldn't an obese person be able to take weight loss drugs like this for treatment? You wouldn't tell a person with depression to stop taking anti-depressants for the rest of their lives because obviously, to feel better, you just need to have better sleep, smile more and have a positive attitude.

Hey! I think maybe you only watched a bit of the video because I discussed all of your concerns. I talked about how a psychologist, like the one they brought on, is very well equipped to help with the mental health aspect of obesity. And I even discussed how saying “eat less move more” is terrible advice. I also discussed my privilege and privilege in general multiple times.

This sounds a bit more tough-lovey than a lot of your content, because I'm pretty sure I've heard you say that some people are in bigger bodies (maybe not obese) and that they can be incredibly fit and healthy. I think it came up in a few of your FITT Down videos. Would love some links to videos and conversations you've had about different sizes that are more thoughtful than the Oprah fluff.

Plus both the "it's a disease" rhetoric and the "you need to be accountable" factor ignore the fact that some big people are healthy, so for them there's nothing to be accountable for and there's no disease to cure either.
