Physiotherapist Guide To Pelvic Floor Exercises After Hysterectomy
When to Start Pelvic Floor Exercises?
Most women start pelvic floor exercises around 4-6 weeks after hysterectomy but only with their surgeon's approval as this can vary from one woman to the next. Seek your surgeon's approval before recommencing your pelvic floor strengthening exercises after hysterectomy surgery.
Pelvic floor exercises after hysterectomy or any pelvic surgery should be pain free during and after exercising.
This video demonstrates 2 positions for returning to pelvic floor exercises after hysterectomy surgery; lying down and sitting upright. Ideally commence your exercises lying down.
Pelvic Floor Exercise 1: Lying Down Position
Position your body so that you are either lying down on your back with a pillow under your knees, or
on your side with a pillow between your legs for comfort.
Lying down eliminates the downward force of gravity making this a useful position for pelvic floor exercises after hysterectomy.
Lying Down Pelvic Floor Exercise Technique
Before starting out make sure you understand how to find and feel your pelvic floor muscles see www.pelvicexercises.com.au/wh...
* Try to picture your pelvic floor muscles wrapping in and around your 3 pelvic openings.
* Very gently squeeze and lift inside around your 3 pelvic openings.
* Keep squeezing and lifting for a few seconds.
* Gently relax and lower your pelvic floor muscles back to resting position.
* Breathe deeply to relax your pelvic floor muscles and rest.
* Repeat this exercise again for a total of 1-3 repetitions.
Pelvic Floor Exercise Mistakes to Avoid After Hysterectomy
Some of the common mistakes made doing pelvic floor exercises include:
* Drawing in or pulling in the abdominal muscles strongly
* Squeezing the buttocks or thighs
* Holding the breath rather than breathing normally.
Pelvic floor exercises can be performed daily and kept within the range of personal comfort - avoid overdoing your exercises especially when starting out.
If you experience any discomfort during or after your pelvic floor exercises after hysterectomy stop and let your body heal before recommencing. If any pelvic floor discomfort persists have this assessed by your doctor.
Sitting Upright Pelvic Floor Exercises
Doing upright seated pelvic floor exercises after hysterectomy helps to encourage the pelvic floor muscles to lift upwards against the downward force of gravity. Upright exercises can be used as a progression position from lying down pelvic floor exercises.
* Sit upright on a comfortable chair ideally a dining chair rather than a soft lounge chair.
* Check you have the correct posture for pelvic floor exercises: your spine should be lengthened and you should have the inward curve in your lower back.
* Feel your pelvic floor openings against the surface of the chair.
* Squeeze and lifting in and around your 3 pelvic openings for a few seconds.
* Relax your pelvic floor back to resting position and breathe into your belly.
* Repeat 1-3 exercises in a row within your range of comfort.
* Try to avoid the mistakes listed above when doing seated pelvic floor exercises.
Progressing Your Pelvic Floor Exercises After Hysterectomy
Pelvic floor muscles become stronger when challenged to work harder. This challenge needs to be applied gradually over time as your body heals after surgery.
Techniques to progress pelvic floor strengthening:
* Doing more exercises up to 8-12 consecutive exercises (up to 3 times daily)
* Holding for longer during each exercise up to 10 seconds at a time
* Contracting your pelvic floor muscles strongly
* Doing your exercises standing upright
* Decreasing the rest between exercises
* Contracting your pelvic floor to hold up against the downward force of a cough or sneeze.

Thanks for being so neutral with the explanation, im a trans man and just had a hysterectomy and ive been feeling really down with the emphasis on femininity and womanhood which isnt bad but it brings my mood right down sometimes. I really appreciate the video being to the point and so professional. Its the small things making my days lately

Thank you, Im watching you in Dec. 2018. Im so lost on how to get better. Im almost at week 4 after my Hysterectomy. Ill be going back to work after next week and wanted to get back to life but didnt want to push it.

Hi Michelle my hysterectomy was exactly 4 years ago to the day! I thought I’d watch your videos again as I have never lost my big stomach since my total abdominal hysterectomy. Tummy muscles have never felt or looked the same. I’m an incomplete spinal cord injury and full time wheelchair user so only have full control above the belly button.

Thank you for posting this..I am five weeks post surgery and my doctor told me to start my core exercises, but did not give me enough direction. The two exercises he told me to do were the crunches and planks...but never told me to modify. I'm glad I found your videos, will definitely be doing these daily! :)

I'm only one week post surgery and I literally got sent home from the hospital with nothing. No paperwork. No here's how to wash or here's how to sit. Not even a belly binder or anything. I'm just getting stuff from my friends who have gone through it already and asking as many questions as possible. I think they do these surgeries so frequently that they don't realize that the person who's going through the surgery has never had it before and has a million questions. They don't view it that way since in their mind they've done it a million times.

I'm 2 weeks, full hysterectomy, leaving 1 ovary. I had Endometriosis, Adenomyosis and PCOS prior and am hoping to feel way less pain. Did you experience pelvic floor pain? I no longer have any incision pain or anything above the bellybutton 😂 but bellybutton and below feels like menstrual cramp city! This is what brought me to this video. Have they helped you?

When is it save to start these excersizes ? Are we just pulling up the muscles in our lower belly? Because that's what it feels like . I'm 2 months away from my surgery and just want to do this right

I queef a lot and kind of loud even when not during intimacy. Will pelvic exercises like this one and the ones from your other videos help stop that? It’s something I feel embarrassed about and I’m very young so I’m not sure why it’s giving me so much trouble

I started on my 6-7 weeks, I didn’t push though and actually have a break the 2nd day after excercises. Seems alright. It’s going to 8weeks now

Hi, i was a ballerina I am an aerialist and teach aerial yoga pilate and what not....I have had a radical trachelectomy and laparoscopy dec 2nd 2021 and by day 6 i was doing my 30 min walks and 30 min easy gentle yoga pilates stretching mix (my own!). It depends how fit you were before and whetheryou listen to your body too. I have no pain but feel very swollen still. I find cat and cow move very comfy and it relieves pressure from the back since my core has been gone for about 9 days now. Down dog feels amazing too as well as minimal spine rotation actually. If you are a natural mover you want to try mobilise asap. GENTLE!

Well i am still recovering from my hysterectomy and i got my lower incision not hurting me however my upper incision is killing me right now i had it done almost 3 weeks ago a laporscopic vaginal

hello i have a question, i had my appendicitis operation done by gastroenterologist, last december 02.2022, questions are why i still have pain after the appendectomy? during the operation the surgeon drop a HEM-O-LOCK CLIP inside my abdominal, would the clip be the cost of my long healing? i need you help because my surgeon does not want to take responsibility about this? here in Finland?😢😢😢

Hello ma'am, thank you for your suggestions. Total Laproscopic hysterectomy was done two weeks back. Bowel movement is for three to four times per a day. But it's troubling the abuse area. Could you help me with this. I am following all your suggestions after surgery from day 3.

Like some other commented, I was told by physio at hospital to start pelvic floor exercises 48 hrs after laproscopic surgery. No doc check recommended..... This seems like it would interfere with healing and internal stitches. In your opinion, what is the timeframe we should begin pelvic floor after surgery?

Just had my hysterectomy on the 15/11/21, a week after that my internal stitches broke and i started bleeding heavily, was rushed to hospital and had a 2nd surgery. So, im 3 weeks post opp from surgery 1 and 2 weeks post op from surgery 2. I feel very strong and ready to move so starting this today. I went into the surgery fit, i run a lot, have completed marathons and do weights. I am convinced this has me bouncing back quickly at 46. A few kilos lost also as i was so bloated prior to surgery. Hi Michelle, quick question, when you talk about lifting and holding in the beginning exercise is it similar to a kegel? I'm trying to figure out what exactly you mean but I do know what a kegel is.

Hmmm very interesting. I’ve had a hysterectomy but never was told to do exercises or sent to pt. Although I was quite young. About 33 I believe. Maybe I’ve just been really lucky to not have issues- as far as I know anyway not related to it- knock on wood.

Hi , X an i ask I had a total hysterectomy 34 years ago and have a problem where it hurts to touch at the top of my vagina , my doc gave me a hormone tablet which was on a stick and had to be inserted high up in the vagina , every time I used it it hurt to touch the top , I am also unable to have sex as it feels like cut glass when my husband penetrates me . Would I benefit using kegal balls or just exercise . Hope you don't mind me asking this .

HI I think this needs to be investigated by a gynaecologist - I would avoid using Kegel balls for now they will be unlikely to help your condition. You need to find out what is causing the discomfort and then access appropriate treatment, all the best

@@michellephysio thanks for your reply . I have had a ultrasound scan and although it hurt with the probe high up inside he said he couldn't see anything , I saw a gynocoligist and he said the same maybe menopause and gave me tablets then after 6 months peaseries, nothing seems to help it .

my doc , said, no boys, no toys, no tampons..lol no problem with that..but I would really like to get back to some yoga ..is it best to wait the 6 weeks? I have done a bit.nothing really....please and thank you!!

Not a professional, but I had extensive surgery on top of hysterectomy and my dr. said ok to do some gentle stretching when I saw her 2 weeks after surgery. I would emphasize 'gentle'
