Physician Visit with an Interpreter goes HORRIBLY WRONG 

When I started my nursing career, I was one of two bilingual nurses in the entire 316 bed hospital where I worked. This was before certified interpreter services and when they didn't seem to think twice about, or worry about, accuracy. 🤦🏻‍♀️ Just because someone is bilingual, it doesn't mean they're a qualified interpreter.

Hola Dr. Schmidt, I'm a Spanish professor. I love all your videos!!! I'm planning on sharing the "Interpreter" ones with my students in spring semester.

As a nurse who relies on medical interpreters on a regular basis, a good one is hard to find and worth their weight in gold. Typically I get interpreters who add in their own comments, try to ask their own questions, try to advocate for the patient, embellish the patients comments, I have even had them request additional diagnostic treatment, testing and consultations. I understand enough of various languages to know what is being said but I still require a translator for my patients when they are meeting with me and a physician.

I'd um...I'd think twice about going out with someone with a potential STI 😳 Is there discharge, swelling, redness? 😳😳😳

Now do a skit with a deaf patient who the hospital refuses to provide an interpreter for! Happens way more often than it should.

I had a doctor snap at me for answering for a patient, when she in fact had answered the doctor herself. We all had masks on at the time, otherwise he would have seen her lips move and known it wasn't me. She understood the doctor because he was speaking slowly and clearly, and her husband taught English (so she did know some English). Woulda been nicer without masks... Hate being yelled at when I was being "good" and not interfering with the doctor's conversation with her.

I learned ASL because we have deaf people in our family and even considered going the interpreter route beause having a medical background helps. I don't have good brain hand coordination. But I did hear a lot of funny stories from my deaf instructors through the years.

This has happened to me translating 😂 I had to call the interpreters even though I understood everything because I wasn't certified and I'd catch sooooooo many mistakes! This one happened way to often... when I would say the name of our hospital during a call, the interpretor would hear the word "Oregon" and translate it as "hospital of transplants" as of I said "organ" instead 😂 (not the in-house interpreters, the out-of-state ones over a phone service). The patient would freak out and I'd correct the interpreter, who would be surprised I understood! I worked there for about 12 years and this happened at least 2 or 3 times per year 😳

I work as a medical translator for the National Institute of Psychiatry and Neurology. This was hilarious, thank you. Funnily enough, I was actually a victim of attempts of the patient trying to pick me up once.

Doc Schmidt: We need an interpreter, can you do it? Me: Sure. I charge a flat $450 a day or $225 a half day, plus travel time, plus miles. Doc Schmidt: We'll just use this random person, I'm sure she'll be just fine.

I'm a vet student, and recently, in my internal medicine rotation, I got a client who only spoke French. As the only bilingual person on the rotation, I had to be the medical interpreter that day... It wasn't very fun haha

(Edited & added) Terrible encounter. A trained interpreter would not react with a personal comment on the subject nor speak in first person nor have a separate conversation with the patient and not interpret transparently. Many ethics principles are being ignored. This is indeed terrible. There are many unqualified bilinguals working as interpreters by need for their relatives from childhood. A natural thing to do. Thank God federal funds are available to cover the cost of offering qualified interpreters to all hospitals schools and courts. Many factors involved in setting this up. The American Disabilities Act and the laws guaranteeing American Sign Language interpreters to the deaf, together with other human rights laws make it a right for a person who cannot speak English to receive the services of an interpreter in these three types of settings. Also physicians have often no idea that their malpractice insurance does not cover misunderstandings and the consequences of these when they try to speak the patients native language & not use an interpreter. Last but not least it is the law to use an interpreter. Ignorance of the law does not justify breaking it. I can only invite you to check the famous lawsuit of the Spanish-speaking patient arriving to the ER sick to his stomach for something he ate, where the people with him said in Spanish he was "intoxicado", the bilingual Nurse interpreted it as intoxicated and the treatment given to him based on this condition left him quadraplegic! Many more lawsuits like this plague school systems, courts and hospitals, unfortunately. This is the company I work for, in case video remote interpretation is an option. Second largest in the USA. At least this interpreter understand both languages, and she made the patient feel better at the end of the appointment. I've seen medical interpreter doing way worse than that.

I had a patient who had an interpreting app on his phone. Very helpful.

I do registration in an ER and have to work with interpreters pretty often, most often via phone/video but sometimes in person, and it's usually kind of awkward. What mostly happens is I get chastised to not speaking to the patient so much as speaking to the interpreter. It's very difficult to talk normally to one person when you know the person doing the listening is actually someone else. I have 4 years of high school Spanish so can understand some portion of it and that makes it easier but the ASL or other languages I just have to take it for granted they're not talking shit or water. Once while using video for an ASL patient lying in the bed, I actually had something in front of the facing camera to the patient and interpreter couldn't see each other. D'oh!

This is so like when you are classroom teacher, its parent-teacher-student night, the student is fairly proficient, you know un poquito solomente español solo, mother seems to know no English, and the school secretary for good or ill controls the content of the discussion..

I worked in social services at a call center. We had a contract with a translator/interpreter service. We had a lot of callers that would just want a family member to interpret for them, but we couldn’t do that because of the policy, the idea being that the accuracy of the information needed to be legally protected (so people could be charged with fraud for outright lying, misrepresenting things, etc). I understood why the policy was in place, but 99.9% of the time people were honest and just needed help. Actual fraud was super duper rare. I only ever sent a handful of cases to the fraud review team out of the thousands of cases I handled. But, you know, government is very CYA, which is understandable. I only popped on here to say that most of the interpreters were awesome, but a couple of times it wasn’t really a pleasant experience. Once, a Russian interpreter was like yelling at this poor girl, and I felt awful. I always took specific, conscious care to keep a pleasant tone for every call, but I really ramped it up for that one. I hope it helped, but I have no idea if it did or not.

Oh my gosh I work in the call center and this happens all the time. The interpreter and the patient talked for like five minutes and then they come back on and they say the answer is no !! I’m like what??? It took all that conversation to say that? What am I missing🙄🤣

XD as a medical ASL interpreter this made me cringe. By the time you get through the interpreting program our clients can discuss ANYTHING and you don’t bat an eye. I remember my professor telling us on the first day: “If you don’t think you can see a patient’s butt, vagina, breasts, or penis, the hospital setting is NOT for you.”

Seems like she is untrained medical interpreter. I'm a medical interpreter and finished course, we always use first person for smooth conversation. Also we should interpreted all the conversation even curse. When I go to work, I always said to everybody " All the conversation in this room will interpreted and will keep confidential. Also I'm going to use first person directly talk to your patient". But I heard a lot of people who not trained well for medical interpretation, somepeople omit whole conversation...

My least favorite thing that a few interpreters do is omit some things I say to the patient out of embarrassment. I mean it's your job to translate everything, not determine what the patient should hear. Most interpreters are excellent though.
