This difficulty mode is as follows: No extra balls, no kickbacks, and no ball saves. Not even when multiball starts. Top this off with an achievement requiring defeating the Cyberdemon in this difficulty, and you have one heck of an accomplishment to go for.
Given how I didn't manage to get all the weapons, it made the final battle a heck of a lot more interesting. Then again, the Ammo Argent Cells can be a big help, and I'm not sure how ammo refills work while on the base pistol.
