Buccal fat pad removal has become a very trendy news topic, but the operation and its objectives have been around for quite some time. Dr. Zelken has been performing this procedure for many years, across all demographics, and for multiple different reasons. Very well argumented professional opinions, thank you. I feel that the current outrage over buccal fat removal is a bit extreme, since as you said, for many clients there are good indications for this procedure - and it is something important to those individuals.

Forreal, she has almost zero bucal fat, so of course she wouldn’t understand why people get it. Her input is invalid for most cases.

O problema da cirurgia plástica foi que eles quiseram padronizar os rostos, foi aí que começou a dar errado. Um rosto triângulo não pode receber os mesmos procedimentos que um rosto meia Lua ou um rosto retangular, as pessoas são diferentes, os rostos são diferentes e os procedimentos devem sempre ser individualizados, mas muitos cirurgiões plásticos são mercenários que só querem o dinheiro sem se importar com o resultado, aí as pessoas ficam com a cara feia, porque o cirurgião plástico vendeu mais procedimentos estéticos que o cliente precisava. A Angelina Jolie é uma que fez inúmeras intervenções e não parece, ou a Amber Heart que fez várias e ficou lindíssima, até o cirurgião pesar na mão no preenchimento de maçã do rosto, agora a Kendall Jenner por exemplo fez vários procedimentos estéticos no corpo e não parece, diferente de Kim e Kylie, embora os de Kylie tenha ficado até natural, é evidente que ela fez, afinal ela saiu de um corpo plano super magro, para um corpo padrão brasileiro, até o tom de pele mudou. Bom, eu diria que quando o cirurgião é bom e sabe os limites e todos os procedimentos estéticos são personalizados, I procedimento estético é a Fada Madrinha da Cinderela da vida real, só falta a carruagem e os ratinhos.

I feel like Stephanie's message comes across as shaming a celebrity for looking worse after their plastic surgery choices. But she'll say they looks beautiful before and after. Idk it feels weird coming from a beautiful young woman who hasnt undergone procedures. I prefer lorry hill who has had procedures done and whos tone is never saying what is beautiful but admiring natural features then going through a celebrities choices throughout their career. It's not judging but statement to try to remove the stigma.

She's had botox and fillers. Lorry has had far more work done, including work she regrets and can't reverse, so regret/guilt/empathy. She tends to be gentler in her videos bc celebrities have come after her for her content. Stephanie is trying to prevent ppl from having the regret in the first place on permanent procedures.

Why people coddle celebrities' feelings when they continue to put themselves in the public eye is an oxymoron to me. If it's "painful" for them to hear commentary on photos they intentionally put up, they need to seek help for their body dysmorphia and step away from the industry for a bit. It's hard to criticize something you make money from.

@nicolina1026 I think it's of value to not be harsh on people you don't know. Having empathy for strangers shouldn't be shamed. I believe in medical privacy and that includes what work they've had done, I agree with lorri that just them saying " I'm not all natural, I make money on my face so of course I do whati need to look my best to get work in Holly wood. Respect my privacy I'm not going into details." That would work WONDERS to help normal people in society not shame themselves for not being perfect.

Thank you for this video. I had this procedure done and have no regrets. My buccal fat was so much I was developing jowls and I also would bite my cheek regularly while eating. While I had the procedure for aesthetics I was elated when I noticed I no longer bite the inside of my cheeks!❤❤ I do enjoy Ms. Lange’s videos and I’m not a fan of filters, or dangerous procedures in the name of trends but some like me have had real benefit from buccal fat removal

Hell yeah doc! I’m really glad to see your input on this. I’m about one month out from getting my buccal fat surgery with you. Seeing all the negativity about this surgery is pretty disconcerting. The surgery is not for everyone, but it is for some of us!! I’m very happy with the subtle difference I see so far and I’m looking forward to seeing more of a change as time goes on. So glad to see you sharing you precessional opinion, great video!

Very informative video, thank you for your calm, balanced and professional approach to this topic. Do you have a video about the large “bags” you see developing under some people’s eyes as they age? You seem the perfect person to ask! There is an actor (Chris Noth) who developed some fairly pronounced and noticeable, large “bags” and then suddenly one day they were just gone (I’m pretty sure he said he had surgery). What exactly are they made of and why do some people get them in such a pronounced fashion? I’ve always been very curious about this. Please and thank you in advance. Cheers!

I love how you are able to break down her video. Stephanie is always triggered, angry and doesn't do her research very well, it's sad that she spreads wrong information based on her own shallow view of the world. I really dislike her tone and the way she talks about people anyway, I can imagine it must be easy not to want and judge people wanting cosmetic surgery when you are born looking good.

I dislike when people make videos about something they have no background in or expertise on. Her message overall to leave plastic surgery or procedures alone has good intentions, however she can only speak for herself. For some people, they feel they need something done.

I think Stephanie is not saying is NEEDS to be corrected in that sense but more in the sense that you don't fit the current beauty standard with chubby cheeks. Especially young people are sensitive to this. And that's another good point she makes: these trends change a lot. This gaunt look is very trending right now because of models like Bella Hadid, but she is a young model that already looks way older than she is. (Funnily enough her sister Gigi Hadid has very cute chubby cheeks and appears more naturally beautiful I think ) I think this Stephanies commenting on this is a MUCH NEEDED breath of fresh air. It opens up a needed discussion. But I completely agree with you that for the right candidate, this procedure is amazing. But that's the thing here...the RIGHT candidate. And that's something everyone agrees on.

I feel like we're talking about two complete opposites here. One is the very chubby cheek, the other is the hollowed out gaunt very aged look. I do agree that young people opting for the very gaunt look is alarming. If plastic surgeons don't select the right patients: chubby cheeks instead of normal cheeks, this will probably create a problem later. I mean...Sophie Turner looks like she aged 20 years.

I’m really glad you made this video. It was thorough, honest, and, most importantly, full of empathy. I’ve been really struck by the harshness of the recent anti-buccal fat removal videos for their often insensitive treatment of patients who have potentially undergone a cosmetic procedure. For all the reasons you mentioned, with just a photo, we cant guess what the patient’s feelings would have been about their pre or post op look. And further, soooo much goes into a pic. These kind of before/after criticism videos are so vulnerable to the very real possibility of a bad photo (lighting, angle, expression), of makeup that was either less than flattering or happened to accentuate an inopportune moment of lighting, expression, angle, OR subtle photo manipulation-shadow creation, elongation, even simple lighting slider tuning. So the before/after presentation in these videos is neither sensitive nor conclusive. And as someone who has watched a ton of content from surgeons and patients about positive buccal fat removals, these alleged result photos just seem unrealistically pronounced when, if anything, buccal fat removal generally has quite subtle results? In fact it’s the subtly to those results that have caused people like me to hesitate to get their own buccal fat removal. I do struggle physically and psychologically with facial fullness AND could very well financially compromise myself by dropping this kind of money. And I want a removal quite badly. If anything, my expectations may be too high. So-for someone like me, seeing these hyper critical videos with exaggerated results like the one the doctor watched today, it honestly makes me feel bad and adds more unnecessary confusion into an already fraught decision making process. I want results from a subtle procedure that i hope can improve my self image, but I’m being told that that procedure actually produces exaggerated ugly results and yet somehow also contributes to everyone’s body image issues in the process.Again, highly insensitive. Anyhow, all of this is to say that your ability to communicate empathy toward the video maker and her points (which you’re right, aren’t groundless), while also standing up for and empathizing with current, former, and future patients-it’s heartwarming and professional and helps create a discourse and society more promoting of dignity for all. I really love your content and appreciate what you bring to it.

It probably is painful for them to see comments on pictures that they intentionally put up of themselves post procedure... But I also believe it's more painful for young individuals who don't understand why they can't achieve a similar look, because they believe the celebrities who lie about the work they've had done. It seems like most celebrities have body dysmorphia, and its more and more prominent in society in general. The quote "Youre not ugly, you're just poor" comes to mind.

Aww you are so sweet. I almost got this procedure done but I decided against it because I didn't fully trust my doctor at the time as he wouldn't listen to my needs, and I just had a bad feeling in my gut about it. I'm 27 and still have cheek fat, so I still debate this procedure often. If I ever were to get it done I would want to go to you because you just seem like such a genuinely good and caring person.

I feel like a lot of people want to say celebs got some kind of surgery done but it could simply be a lotta stress and as someone who suffered 3 years of emotional trauma, my face hollowed out but the fat around my mouth stayed put, causing extreme defined cheeks, it's annoying and I had this all my life dealing with my weight and being skinny with stress....These celebs go through large amounts of stress daily and have to appear perfect daily, and some of those celebs don't handle stress as well as the more....ummmm....vein ones...lol...so yah....that will put strain on the face, I know I'm living it. I'm glad you are so kind towards peoples feelings, I wish I could show you my face because I have a very noticeable fat around my mouth and it makes my face look more sunk in while that part just sticks out, I would love to have it removed because I can't seem to gain weight in my face anymore so that would just smoothen my defined cheek bones, but I live in Canada and not sure where to even go for this or if it's offered....too bad, I seen how you removed the skin from in the mouth and since I was young I always wondered if that could be a thing...good to know I can still go for that one day.

Bom, eu cirurgião plástico fácil brasileiros disse que remover a gordura das bochechas só funciona no formato de rosto redondo, mas também tem o caso do quanto se remover, já que as pessoas dão diferente, também suponho que os cirurgiões não removam o mesma quantidade ou que aquele paciente tenha a mesma quantidade que outro paciente. A Cirurgia plástica não pode virar uma de Ken's e Barbie's, cada pessoa é única, então a cirurgia plástica ou até mesmo os corpos e rostos não podem ser padronizados.

Okay now I MUST ask you a question. I just had buccal fat removal and I only had 1 fat pad. One cheek had it and one didn’t. How common is this? I can’t find it anywhere on the internet.

Does your face get big again after buccal fat removal when you gain pregnancy weight? I know I'm a good candidate, but maybe I should wait until I finish all pregnancies. On the other hand, I think the buccal fat doesn't "grow" back, so not sure if my concern is only in my head. 🧐

​@@WD.909masseter is muscle not fat and it is further back on the face. The buccal is fat inside the cheek Botox can't do anything

@@WD.909I had Botox injected to my masseter muscles and I loved the results, my face looked less wide, however for me it only lasted 3-4 months. It didn’t get rid of my chubby cheeks though.

You've got a brilliant eye for aesthetics. At least, it's in line with my own, lol. So much nonsense being described as buccal fat removal when it's clearly a multitude of surgeries, including implants. I've yet to see a genuine buccal fat removal that didn't enhance the person's looks.

Hello doctor i have these weird bulge around my lips and mouth even though my body fat % is low and i have a strong jawline i asked my regular doctor who told me it can be orbicularis oris hypertrophy so I'm trying to reduce any water retention and trying to chew properly but any procedures to fix this state it's like a bulge right beside the corners of the mouth

A cirurgia plástica é velha o suficiente para a velha Hollywood fazer, assim como pintar os cabelos ou editar suas imagens. Hoje temos procedimentos seguros de recuperação rápida, na época de Marilyn Monroe não, mas os estúdios eram donos de sua vida, então os artistas não tinham muita escolha como hoje, mas no final do dia ainda parece que eles não são livres.

i must say the 'you don't HAVE to' argument feels disingenuous. you must know it's a figure of speech because you're an intelligent man with an intelligent job. i don't think you're truly recognising her argument when she says things like "now we have to pay a surgeon to remove our buccal fat, like whyyy". i see that it can be an insecurity for people that come to you for surgery, but for many the insecurity wouldn't even exist in the first place if it wasn't made out to be an imperfection by societal standards. the more people have the 'sculpted perfect' look, the more 'imperfections' come to our attention. happy to gather plenty of primary and secondary research for this. there's debates to be had that are bigger than any of us, but i think its pretty obvious what she was trying to say.
