In this video, Dr. Sachin Malde discusses:
● Just 1% under the age of 50, and 5-10% in elderly males experience recurrent UTI.
● Male UTI symptoms mirror females’ but can also include perineal, anal, scrotal and penile pain.
● Urology deals with the genitourinary system in males. This includes: urinary tract, kidneys, urinary bladder, urethra, testes, prostate, and vesicles.
● As males age, prostates enlarge affecting bladder drainage. Residual urine can attract infection.
● The prostate itself can also attract infection, known as chronic bacterial prostatitis.
● Pelvic floor tension can contribute to bacterial prostatitis, as can chemical irritation-when urine refluxes back into the prostate.
● Benign prostatic hyperplasia is non-cancerous prostate enlargement causing difficulty emptying the bladder, poor flow, slow starts, night-waking to urinate and frequency.
● Benign prostatic hyperplasia is well studied. There are many medications and surgical options.
● During UTI diagnosis, underlying issues with the prostate or pelvic floor must be ruled out. Then, standard and extended urine testing can be employed. In males, semen cultures can also be analyzed.
● Localization studies are sometimes required to determine whether the infection is located in the bladder, prostrate, urethra, or multiple locations.
● Because tissue penetration of antibiotics in the prostate is poor, a four to six week course of antibiotic treatment is recommended.
● In males experiencing chronic infection, the pelvic floor should be assessed. Dysfunction can contribute to problems with urinary flow, difficulty getting started, stop/start, takes a long time, straining, bowel and sexual symptoms.
● Dr. Malde treats males using Hiprex.
● Supplements Saw Palmetto and Quercetin have shown to reduce prostate size, when used long-term.
● There are also minimally invasive surgical options available to treat prostate enlargement.
● Evidence suggests that there is a microbiome in the male urinary tract.
● UTI-causing organisms like E. coli, pseudomonas, klebsiella and Enterococcus can be passed between sexual partners.
● Dr. Malde recommends probiotics and prebiotics (such as D-mannose) for both female and male patients, especially if they’re taking antibiotics.

Dr. Sachin Malde is a Consultant Urological Surgeon at Guy’s and St Thomas’ Hospital and the HCA London Bridge Hospital, London. He specializes in the management of bladder diseases (urinary symptoms, UTI, bladder pain, incontinence, bladder cancer), benign prostate enlargement, and pelvic floor disorders. He has expertise in the detailed investigation and treatment of male and female urinary incontinence, bladder pain, and recurrent and chronic UTI, and has an interest in new and novel techniques for treating these conditions. He also offers a range of treatments for other renal and urinary tract conditions, including minimally invasive treatments for bladder overactivity, incontinence, complications of previous surgery, fistula, and prostate enlargement.
Dr. Malde qualified from Guy’s, King’s and St Thomas’ School of Medicine in London, and completed his postgraduate research into the evaluation of men with incontinence after prostate surgery at University College London, for which he won the Gold medal for outstanding research. He underwent specialist fellowship training in female, functional and reconstructive urology at University College Hospital London, and has a strong academic interest having published extensively in peer-reviewed journals. He is also a panel member of the European Association of Urology Guidelines panel on male lower urinary tract symptoms.
