Radiofrequency ablation or RFA procedures can often provide temporary relief from back pain. The downside is that the relief typically lasts for three to six months and then comes back. Do you any idea what 3 months of no pain means to a person in chronic pain?? It's hell to have pain every single day of your life and it just goes on and on. I would be thankful to have one week of no pain.

I herniated my L5-S1 disc in my late 20s and lived with chronic pain for decades, eventually developing osteoarthritis and degenerative disc disease. I had a successful anterior fusion almost six years ago, but that didn’t completely relieve the debilitating pain. My first RFA was actually done a month or so before the fusion, but as pointed out, RFA is rarely a permanent solution for facet joint pain relief, unfortunately-but those 6-9 months can be a real blessing. I have had RFA like clockwork every year and the most miserable time are the several months of pain after the effects wear off while battling with the health insurance company to have the procedure again. If this newer, permanent solution becomes more widely available, I would definitely be open to it. Living with debilitating pain isn’t living, it’s barely surviving.

Yah the funniest thing is. The whole idea of Pain Management used to make insurance companies happy, because it eliminated OR put off expensive surgery. But now that they changed the system of people getting Pain Management instead of surgery, they have now clamped down on approving PM and put rules in place creating more hoops to jump thru. It’s ridiculous, USA’s corpo healthcare is shameful! Prime example, I took a break from PM because the results (pain relief) wasn’t good enough for Corpo Healthcare. So two years later my general doctor has to write ANOTHER referral for PM even though I have been going to PM since the early 2000s. Corpo Healthcare says: “Not so fast!” and they try to get me to go to pathetic Physical Therapy prior, I have been through PT over 44 times since injuring my back in 1994 and it has NEVER WORKED (it has inflamed and irritated my low back though). So I think to myself, I wonder what will happen if I visit a Neurosurgeon again solely to trick Corpo Healthcare into thinking I going the surgery route. 2 weeks after my Neurosurgeon visit Corpo Healthcare mysteriously approves my Pain Management referral to have RFA done. 🙄 Absolutely ridiculous and shows USA Healthcare has NOTHING to do with patient care and everything to do with greed/making money, in this 👆🏼case saving corpo health insurance from having to pay for an expensive surgery!

I'm watching this video for trigeminal neuralgia but I'm right there with you L5-S1 herniations and double surgeries, dozens of steroid injections and nerve ablations and they wanna do a fusion and I'm not ready, insurance is constant nightmare. Surviving is right. I'm only 34 and this isn't how I envisioned being a mom to my toddler :(

his procedure is no different, whether you use a scalpel laser or RF on the nerve you get the same result...good relief that may or may not come back...personally mine has not come back yet but when it does I am happy to repeat it to stay out of pain...

Sorry if I haven't been keeping you up to date. The RF/A took almost a month to see a result. It has been 2 months now and definitely getting relief. I was skeptical, as I wasn't feeling a relief from the pain within days (a figure I had read), but it seems at about week 6 I am finally out of extreme pain...maybe a 2-3 on the pain scale (down from say 7-8)

Yaaaassss! I"m so happy to hear this, Richard! As the pain goes away it's time to start doing some light exercise. Water walking. Elliptical. Nothing with impact. Get back range of motion first. Are you on an anti-inflammatory diet? This is a good time to invest learning in that as well.

I just had my 3rd one. 6 to 9 months of so called some relief always comes back today is the day after and I dont remember it hurting this bad. They said I could have 2 to 3 weeks of increased pain b4 I see relief

@Nichole Domine I'm almost 2 months out from my 1st ablation and am still absolutely miserable. I'm having to ice constantly and by evening my entire back is so knotted up I can't sit still. The site is extremely sore, right where a shirt tag should be, and I'm past frustrated. I asked my pm dr about it and he said I probably had big nerves in my skin tissue and they are irritated from the procedure.. really makes me wonder if I want to go thru this again..

You're so welcome! I truly admire how you are taking control of your own care. It's the first step toward getting better. I hope you find a great doctor. Most RFAs (Radiofrequency Ablations) are performed by pain management physicians. You should look for one who is double board certified-first in anesthesia or physiatry, and second in pain management. Be cautious of practices that rely heavily on narcotics and require frequent visits for refills. BestPracticeHealth.tv certifies doctors who do not use narcotics for chronic non-cancer pain. Ensure your doctor performs a significant number of RFAs. You've got this. Hang in there, and I sincerely hope you feel better soon.

My exact thoughts. I started epidurals & abalations 1 1/2 years ago. I have to get them every three months. I suffer migraines from DDD. They definitely help, but clearly I need some more permanent.

What kind of crappy RFA technique gives you 3-6 months? Use a 16-18G RF needle, LA first, parallel to nerve (SIS style) and you should get a year. From a practicing pain physician

Sounds like a great technique! Do you document your outcomes? The literature is pretty clear that 60% of people get an 80% pain reduction for an average of 10 months. If your data is better let me know so I can help get it out there!

@@BestPracticeHealth In my own audit of 50 consecutive lumbar ablations (first ablation) the median duration of greater than 70% pain reduction was 11.5 months. For 20 cervical patients it was 16 months.

I rather have a year free from pain and px drugs. Its miserable and just torture not being able to even sleep writhing in pain..watching your family in a miserable helpless state is even worse. I thank God for allowing me to be reduce my misery 80 percent with steroid shots and will have my first ablation tomorrow. Your prayers will be greatly appreciated🩷

I had this procedure performed twice. The reduced pain time lasted around a month, each time. My pain doctor told me that surgery is my only option, but there is no guarantee it will help the pain.

I had the RFA procedure on my neck. 1st side without any pain meds. Wow was it painful.So bad I was creaming & squirming so the doctors said to halt it. So came back maybe a week later & they did both sides. Too soon for the side they had started on the week before. bang! The worst pain now right after the treatment. Now on serious pain meds for 3 yrs. & the pain is something way worse then when I went there in the 1st. place. The pain was like when U wake up with a 1 side stiff neck. Painful,but it came & went. Now it is 24/7 & I can no longer tilt my neck back to gurgle or look up at the sky or shelves in a store. When I stand & walk I can't look forward because the neck pain is horrific all the time.I have to look down to relieve some of the severe neck pin I now have. I also have to juggle magnesium to prevent constipation from pain meds, etc. I also now have pain from a slightly forward head & military neck. I never had pain like this from these issues.. I think they over burnets my never by doing the same side too soon. Why? Because I now have germinate constant pain in my neck I never ever had before the RFA.. I didn't have pain meds because I had had a coffee with a bit of cream so they said well can't give her anything for pain. I had heard people did it withOUT pain meds & said it wasn't that bad so I figured I could do it too. No way! It was so painful. I wonder to this day had I not had milk in my coffee & had pain meds. I would have pressed through the procedure & felt better not worse. I guess I will never now. I am now wanted disc surgery, but can't find any doc good at it who will accept y insurance & would consider me a good candidate for that procedure. They speak of fusion, but I can see on the x-ray my neck has already automation fused at level 3-4. by itself so to me fusion is not the right answer since my neck is already fused.. I want Dr. Bae to do it but he doesn't take insurance & isn't too keen on doing it on older woman like myself.. So I don't know how I will B free of this 24/7 new pain & get off pain meds too! I wasn't taking pain meds before the RFA,but sure did after. Trying to take as little as possible, but so far 3 yrs. & still taking some. It's a nightmare or is it a law suite?? Hard to prove I would think.. But suing did cross my mind. I think they over burned the nerve & killed it or damaged it in someway it didn't grow back & created worse pain then before or ever for that matter..

@@RameyRocks my health insurance denied it, they are also trying to not pay for the injections I had done to help with pain. However another doctor that also works out of the office prescribed me qulipta 60mg it has helped a lot with my pain they also want me to try acupuncture for the back of my neck. I’m scared my insurance will deny that as well, but I’m still going to go for it, because I still get pain in my neck and head, just not as bad since taking the meds.

i had spinal fusion (L3-L-4) in May of 2021. In the last 6 months pain returned enough to warrant another visit to spine surgeon. He felt RFA may help ( for possibly 6-12 months.) It has been 24 hours since RFA...minor improvement in pain levels...I will keep posting as time goes on

Dorsal endoscopic rhizotomy, which I call direct visualized rhizotomy (DVR) is the way to go. It's not widely available. Watch for my youtube live show to discuss! Coming up in the next couple of days.

Thank you for taking the time to post your videos. I have been to 4 different doctors and gotten 4 different treatment plans! Very frustrating! All say I have DDD L4 and L5 with one herniated disc. Neurosurgeon 1 suggested RFA, he did a nerve block and it did help. got opinion 2 from OD he said microdiscectomy, then went to #3 neurosurgeon to break the tie and he said fusion due to the disc degeneration and said with RFA I would be back ! wen to doc 4 OD he said hip pain was from IT band and shot me with cortisone in hips which was terrible. Any suggestions? I have had decent success with cortisone one time in l4 and 5. I went to 4 doctors before deciding between RFA or microdiscectomy and now I have a fusion and IT band thrown in there. I would love and appreciate your thoughts. After reading this page and others I think no to RFA and I am not ready for a fusion. I am 57 and active, teach yoga walk but standing is the worst! I went through literally the same with different dr opinions which were the alif is what I really need to fix the issue but they said wait it as long as I can because it will make me stiff. I ended up doing all of the procedures 1 after another. The lumber disectomy just shaves the tail bone and relieve some pressure which fixed the issue I had with the pain going down my legs for a few years. Then I had to get the alif surgery done and it was great best I have ever felt. The more I walked the less pain I felt but unfortunately I tripped 5 days after coming home from the alif and it has been hell all over again. Dr's were saying they couldn't understand why I was in pain so basically treated me like I was lying. Until I found an OMT (osteopathic manipulative treatment) Dr and they like magic but it would only get so much better. So my pain management Dr tried the facet injection and it pin pointed that's where my issue was (I felt no pain at all while the numbing medicine was in there) so he suggested the rfa. We did 1 side 1 week and the other side the following week. I can say I fell a difference in pain but still sore but I'm only 2 weeks in on the first side and that side is less painful at the moment fingers crossed. I hear it can take a few weeks to really feel the difference. I hope this helps you because the Dr's told me the same as they told you

Hi best practice health, I been dealing with lower back issues for the last ten years L5-S1.I finally just had a RFA done on my left/right side. But seemed not to work, I’m still in pain. The frustration and mental issues that follow with back issues don’t help. Would this operation be helpful

I've had 5 of these procedures, and this last one was on a different set of nerves. But my Doctor has basically said all this doctor has said, to me. I really appreciate his candor and his efforts to find a different approach for me. 2 to 3 months without pain is indescribable when you live in chronic pain and that area causes pains in your head also, feirce tension headaches where it feels like someone is trying to pull you scalp back, coupled with electric shocks and crushing headaches. It can be debilitating and it has been for me for over 6yrs. My doctor is calling me a regular at this point 😂 The best advice I can give is find a good pain specialist, they are like little detectives 🕵️‍♀️

I am suffering from unilateral testicular pain ( mainly the muscle around the testicle may be cremasteric muscle) . My pain specialist advised for Pulsed RF or Genital branch block of GF nerve . Can I try for nerve block. Any side effects?

After getting differing views on time it takes to take effect,..after 3 weeks ( i had been told it would take anywhere from a week to a month)i can report about a 20% improvement...not much but the doctor informs me that it may nee additional RF/A to hone in on the exact spot. I will get an SI injection first then wait 2 weeks before an appointment for another RF/A

Someone told you RFA takes a week to a month to take effect? I would love to find out how they came to that conclusion. The pain relief of the RFA procedure comes from disabling the medial branch fibers. It's just like a root canal. The medial branch pain fibers are burned in the procedure. They never work worse than they do right afterward. So, whatever pain relief you will get from the procedure should be present immediately afterward. You do have pain that comes with recovery from the procedure. And that may feel the same. No matter what, 20% relief is not enough. The average is 80% relief for 10 months. Either they missed, or it's something other than the facet joints that hurt. That's where the SI joints come into play. SI injection is a good idea. However, I'd follow it with chiropractic treatment for your SI. The RFA for SI is even more hit or miss than the low back. And chiropractic care really works great for SI joint pain.

​@@BestPracticeHealthRFA is said to take a week or 2 for the nerve to die off from the burn. Also most Drs I've been to do NOT recommend chiropractic work on SI joints as it's more likely to aggravate it, because typical SI pain is from too much movement...

Im 64 and 6 years ago I had 3 epidural shots for my lower back (I believe L5-L6 or 7). They lasted that long. In the past 6 -8 months Ive gone through 3 new Epidurals and one more (4th) Facet shot. The epidurals lasted 2 days each, the facet lasted 2.5 weeks. So now they said they want to do an ablation. I had an ablation for my neck twice now in the last 4 years. I dont want to go through this over and over for my back. At times, I cant walk well and cant walk for long distance. Are there new developments or is DVR still the best option. I live in Orlando FL.

Yes. There's no downside to trying RFA before DVR and DVR is not widely available.

I've been on the fence for a few months now. I was advised to take RFA for my facet joint arthrosis that's been painful for straight 8 years. Should I do it dr.? There's no DVR where I live.

Had this done last November and December? Was done 2 weeks apart to do each side. But unfortunately it didn’t work at all. Where is best place to get those electrodes in? Back of the head or under the skull line? Mine was done back of head instead skul line.. I feel specialist chose wrong area . should’ve done it back of neck ..

Please tell me why right after my BOTCHED nerve abrasion of my neck I now have constant pain & can't look up anymore? What went wrong? I do know I didn't have any anastitic because I had had some milk in my coffee that morning, but I had heard people did it without any pain meds. & it wasn't that painful. Well I decided to do that. It was ever so painful. I was screaming & writhing in pain. So they decided to stop & not do the other side. I think I came back a week or so later & they did both sides meaning they did the same side 2 times. I think they should have not done that. I believe that is why I now have pain I never ever hand in my neck. My neck pain was very little & only sometimes. Much like a morning stiff neck sometimes. But I could tilt my head back to gargle after the NA I can't!! Can U tell me what happened? A doctor I saw about my neck which is military & slightly forward, probably due to breaking my L4 some yrs. ago & maybe my ribs moved forward because I lost height, lots of height. No doctors fixed my compression fracture to regain my lost height so now my lungs have less space & maybe that is why my head is now forward & I have NO waist anymore. My ribs almost touch my hips now.. Very uncomfortable as is the 24/7 neck pain & very limited motion. I was told my bones R too thin to do any discs & I don't want my neck fixed. If that happens I will have even less movement than I now have. I am very upset about this situation. I don't understand how my bones R too fragile to have discs pit in when I had THR last year. The screws R holding up well. I even fell on concrete & nothing came apart & nothing broke.. Any thought?? I do believe something happened when I had the nerve ablation, because I didn't have the pain before & now have pain all the time in my neck.. I am sure I had military neck & forward head to some degree, & I did not have this constant neck ache & did not have any problem tilting my head back to look up or gargle.. What happened? The spine surgeon I saw said he agreed something must have gone wrong with the nerve ablation since I had no pain before the procedure & after 24/7 pain & limited movement.. Very unhappy now.. I feel my life is ruined..

Hello I had an acute injury causing disc herniation l5-s1 6 years ago. I have gotten multiple epidural shots with no relief early in...then physical therapy....chiro...physical therapy...with no pain relief. My latest MRI says I have spondylolisthesis grade 1 at l5-s1...Neuro surgeon said I need an alif/psf. I am not ready to commit to a surgery like that as I am 40 years old and concerned about long term effects. Pain management told me this RFA/ facette ablation has no side effects and can be done as often as I want. You stated that this procedure has diminishing returns in terms of pain relief? So is getting this procedure harmful long term?? I Was hoping to get pain relief even for a little 1 year so I am able to do core strengthening to treat the spondylolisthesis. Is it better I just get this procedure or is RFA first then this procedure if RFA works but wears off in a short period of time. Please advise. Thank you in advance!

For some reason my branch block removed all my tightness and neck pain but I still have limited neck mobility. Definitely an interested in pursuing a DVR/Endoscopic Rhizotomy

I’ve had it 3 times going for my 4th. 1st got about 12 months and only slight pain came back. 2nd time less than 6 months pain came back way worse. 3rd time got over3 years which was amazing. Was able to play sports with my young kids and have fun with them. Pain is coming back and gradually getting worse, hence why thinking of having it again.

Where does the pain comes from David? My husband had the RFA in 2018 summer, it last until 2022 summer ( pain was never gone but waaay more manageable). (previous surgeon said nerves can grow back?). Now, since last June, the pain is unbearable he can barely walk. His nerve damage is caused by spinal stenosis. L3,4 and 5. Just did 2nd RFA waiting to see if any relief will happen. He’s 70. Is this DVR procedure good for EDS patients? I have EXTREMELY SEVERE EDS, my Beighton score is 9/9. I am having ablations every 6 months on lumbar and thoractic spine. Then also getting breakthru injections at 3 months because my insurance wont cover the ablation until 6 months. Im on very high levels of pain meds as well. My ribs in between the shoulder blades dislocate many times PER DAY at connection to spine. I need something to help the pain in that area and down at the tail bone area.

i’m so sorry to hear about your condition. It sounds like you’re caught in a trap. One thing to always remember is that chronic pain medication’s cause chronic pain. They change your body, and the way that you process pain signals. You cannot be out of pain while you’re on chronic pain narcotic medication. in some studies, 50% of people in chronic pain have no pain after they taper off all narcotic medication. I strongly encourage you to start by considering the role that the narcotics are playing in your chronic pain. Try going off all narcotic medication, and conjunction with and addictionologist. That’s a Doctor Who handles people who have been taking chronic narcotics. once you are off all medication for at least six weeks, you can reevaluate what’s actually hurting, and whether or not you need additional treatment.

So sad that Pain Management doesn’t tell of the potential dangers of RFA and other Pain Management procedures. I had this procedure done for 7yrs 2-3 times per year (until insurance changed it and can only get 2 per yr.) and now have extensive nerve damage. Pain Management is now the equivalent of going through a McDonald’s drive thru. No time for questions, do you want the procedure or not, if you’re lucky YOU MIGHT get 3 rushed minutes with doctor before the procedure. I am meeting with attorneys now for malpractice!

No. The medial branch nerve that are "burned" by RFA are sensory only. They do not affect motor function.

Exactly what i was looking for in terms of a explanation. Are permanent nerve blocks available in which the nerve won't grow back or transmit pain signals in the future and are there any drawbacks? The big push from doctors onto patients is the neurostimulator for chronic pain control versus taking opioids but thats a can of worms that can create issues which I wouldnt wish upon my worse enemy.

Yes dont do the internal stimulator. Hear is bad my dr wanted me to do it. Had spine surgery in 2015 no fusion did good but after two years it got screwed up but mri showed kidney cancer so had that fixed did something do no what but my back was fine for 6 years then hurt had l4/l5 fusion it was mostly ok boy part of screw popped out dr didn't believe me its on my dresser part of allograft fell out had revision of l4/l5 fusion six months later then my whole left leg felt like a brick. Had knee replacement 4 months after which hardware on recall. Had hamstring tear still not fixed. Then three months later back hurt like hell mri showed huge disc herniation below fusion. Went to different spine surgeon he redid the fusion extended it from l4 to s1 put in cage and pelvic screws but disc was so stuck to nerve root couldn't get it all out back is good left whole leg still useless. Cortisone shots useless i had in other areas. So now how locked muscles every night claw toe. And since i cant feel my foot was walking with things in my hand foot went half on half off my 4 inch concrete sidewalk fell right on my elbow went to work then er broke and displaced olecranon and radius. Had splint then surgery with plate and screws. Worked a week with it broken. Had tears in both shoulders different years. Have pvns in knee so surgery every year for that with occasional meniscus tears. Im worse then an athlete told my boss i need 40 million a year salary. Im going to be a display in the home depot hardware department. No dr will help me with this leg probably have to eventually get it amputated. And of course no pain medicine which i always weaned myself off without drs telling me. Was going to convert my basement into an operating room since surgeries are like taking out the garbage for me. But at this rate im going to build a state of the art humpty dumpty medical center.

⁠ I have extraordinarily brutal nerve pain due to peripheral neuropathy in my feet and hands. When my tibial nerve in my feet starts to go crazy, I’m in a level of pain that is really unlivable. I suggested tibial nerve ablation to my pain management doctor, but he said it would paralyze my foot muscles leaving me with foot drop and possibly unable to walk. As it is now I only get up to go to the bathroom. QUESTION: What do you think about what my pain management doctor said? Can I get tibial nerve or sciatic nerve ablation? I’m in extraordinary pain that is quite close to unlivable.

The smart govt trial 5 year follow up on the intracept procedure shows basically the same or more pain reduction as at 3 months. So that seems to be pretty permanent. I can’t find safety information on surgical ablation of the basivertebral nerve. Wiki seems to back up that nerve pain relief is only temporary at least for facet rf ablation. Nerve recovery might be a blessing in disguise if one were to rely on the body healing itself in the meantime with support like addressing infections, dvt, increasing circulation to the spine, uva exposure improving osteoblast balance, ozone therapy to name a few. But per the smart intracept trial, the pain reduction appears permanent so maybe they are using more energy since it’s in a bone, and the nerves don’t come back. I just saw this on wiki page for nerve block “Because neurectomy of a sensory nerve is often followed, months later, by the emergence of new, more intense pain, sensory nerve neurectomy is rarely performed.” This does seem to happen, because the pain axons just regenerate, which kind of makes sense since unlike motor neurons they don’t need to exactly tie to a muscle just sense injury. I suppose if you cut the nerve farther away from the target then the claim of longer term relief might be correct. My question is what happens to muscle mass when you cut the nerve? Does the muscle atrophies? Dies , gor example i had servicle fusion brfore that my rough arm started shrinking and my tricep practically desapaired, After the surgery i regain most of the muscle mass back, but after i had covid the pain came back as bad as before the surgery, with this surgery will one loose mass and end up eith one normal arm or leg and one skinny one?? In my view, Craniocervical Instability (CCI) is a real but very rare condition, typically stemming from significant trauma. In my career I had one case, a boy hit by a car, and after I “reattached his head” the story went around the world in the news. Most individuals experiencing neck pain are likely dealing with untreated facet arthropathy, commonly known as spinal arthritis. That said, some people have reported positive outcomes with cranial cervical adjustments. It's essential to note that this procedure is a specialty and isn't performed by most chiropractors. While there's no harm in trying this approach, I would strongly recommend exhausting traditional care options first. The standard protocol for nonspecific neck pain involves flexion and extension X-rays, followed by physical therapy, and an MRI scan of the neck. If the pain originates from a facet joint, it should appear swollen on the MRI, which can be addressed by a pain management doctor through radiofrequency ablation. Alternatively, you might have an annular tear, causing discogenic neck pain. This condition is best treated with anterior cervical discectomy and fusion. This surgical procedure is typically performed by a spine surgeon, either a neurosurgeon or a fellowship-trained orthopedic surgeon specializing in the spine. It's crucial to explore these conventional treatments before considering more specialized or alternative approaches.

There definitely has to be a risk, even with the RFA there is a 20% failure rate in the procedure being successful and can lead to the patient being in more pain than they originally had before the procedure whereas cutting the nerve must definitely have a higher failure rate.

did he have a repeat MRI after the tumor was removed? It's important to make sure they removed the whole thing and that residual or recurrent tumor is not the cause of pain. If imaging shows the whole tumor is gone then the nerves may be scarred in by the healing. Membrane stabilizing drugs like gabapentin and lyrica can often help reduce the pain in this situation. Stay away from narcotics of any type; they're addictive and don't work for this type of pain.

TBH there are percutaneous fusion techniques for cervical facets not that are permanent and very safe... so, DVR is not often needed in the neck.

I had vein ablation done as a preventative procedure. I never had any previous issues besides noticing the veins starting to appear on my legs, which prompted me to get evaluated before I had any issues. The problem is, it's now been 10 months, and I still have swelling in that area (lower medial part of my leg). It's believed to be scar tissue. Is it normal to have scar tissue and swelling 10 months later? How does someone get rid of this?

It sounds like you have impaired Venus return after vein ablation. Talk to the surgeon who did the ablation. It may get better overtime, but one thing you can try. In the meantime is Ted hose applied in bed before you get up.

I’m 43 and have advanced Ankylosing Spondylitis my facet joints are completely fused from AS. The pain is getting unbearable with activities. I’ve had RFA twice now. It helped for about 8months, had to have it again. Getting on and off the table is getting harder, I’m loosing the ability to lay prone. If there is a more permanent option I would definitely want it. is it riskier than RFA?

I'm scheduled for this procedure in literally 24 hrs and I'm a nervous wreck. Will they sedate me for it? Will I need pain meds for a few days? Does the site hurt even worse for awhile afterwards? My pm dr is doing it to my cervical spine and said if I get good results we could try it on my entire spine (D.D.D and severe stenosis plus arthritis in my spine)

@Best Practice Health Yes, it was done last Tuesday afternoon. Thankfully I was sedated. My upper back felt like it was throbbing deeply the 1st 24 hrs. I've been rotating my ice packs since then. The site he did it on is VERY sore still. It's hard to rest at night because no matter how many pillows or positions, it hurts. I've been surviving full body RSD/CRPS for 16 years and am chalking the pain up to it. My hubs says everything looks normal at the site so I'm assuming it's just me being me? I'm so ready for the pain from it to calm down so I can enjoy the benefits of having it done! Do you have any advice on how I can get the site to calm down??

I had nerve ablation on two lumbar facet joints but disappointingly it made the pain in my buttocks and thighs worse, beware that things don't always get better with this procedure. I'm hoping that this damage will right itself, although 3 months after ablation, it hasn't.

@@ronik1974 Sadly no improvement- plus there's no process to talk to the doctor concerned, therefore he's ignorant of which ablations are successful and those which make the condition worse. I really don't see how the UK NHS arrive at a 70% success rate for this procedure. Good luck with your recovery, hopefully time will tell.

Had Ablation two days ago in Toronto... so far so good 2 years of 24/7 pain....This guy is mis information... the nerve grows back slowly you can have it done again.

Hey, timothy. Thanks for your comment. I'm delighted to hear RFA has worked well for you. I always loved doing the procedure back when I practiced because so many people, like you, were able to get great pain relief. Unfortunately, that's not always the case. For most people who do get relief, it typically has worn off by 10 months. Retreatment after RFA is always an option, but sadly often does not work the second time. To be clear, the nerve doesn't actually grow back after RFA, because it was burned and not cut. The pain comes back when the nerve recovers. DVR is a good option for people when RFA stops working or does not work at all despite an effective Medial Branch Block. Whew! This was wordy. But I don't think we're actually saying different things. I do disagree with the 'misinformation' label. If you'd like to discuss this further please come on my Facebook live. It starts next week. What was your pre procedure pain if I may ask? I’m scheduled in 3 days and freaking out. My pain isn’t that rough tho I’d say 2-3 on average migraines tho can be really bad and the slight headache is almost every day. I’m doing it mainly for the case treasons- was in an accident and that’s what they told me. What was your pain like before?

when you think back to the medial branch block that came before the RFA, did you actually get relief? Because if you did, the next step is to repeat the medial branch block. If another medial branch block is positive, then the doctor just missed with the RFA, and it would make sense to try it again. otherwise, the first medial branch block was a false positive, and it’s time to consider something else.

An Interventional Radiologist is offering pudendal nerve cryoablation to treat my severe left scrotal pain. I've had 2 100% successful nerve blocks done at the ischial spine. He wants to do a block at the Alcock's Canal, then treat there. But that's distal to my previous blocks AND to the most common entrapment location for that nerve - the spot where the sacrospinus and sacrotuberous ligaments cross. Apparently all ablation types for treating pudendal neuralgia get done that way. This doesn't make sense to me and I can't talk to this doctor until the day I fly there for the block. What am I missing? I tried the DRG snd am an Electrical Engineer who spent my career in Silicon Valley developing novel medical devices, including implantable defibrillators and RF generators for performing ablation procedures - which is what i was expecting to be offerred. I have had my genitofemoral nerve and the perineal branch of my left pudendal nerve cut.

I don’t know any doctors in Pennsylvania by name who do the DVR surgery. The best way to find them is to look for endoscopic spinal surgeons. Thank you for reminding me, I keep meaning to get a good list of surgeons who do this procedure and put it on my website. In the meantime, look up, endoscopic spine surgeon near me on Google.

What are the dangers of cutting the nerve? How will your body know if you're having an emergency if there is no pain awareness. In my case, occipital neuralgia. How would I know if I'm having a stroke related headache if there are no nerves to inform me?

Pain is a warning, anso so it makes sense that not having pain could make you vulnerable to developing a worse problem. However, as a practical matter, I'm not aware of any long term effect ever shown from not feeling back pain after RFA.

Hello. Thank you for the question. We get that one a lot. Since pain is a warning sign, and a red flag, in theory not having pain would lead to damage. In practice, thankfully, life doesn't work out that way. By relieving joint pain and making people more active RFA reduces arthritis and does not increase it. Literally millions of RFA treatments have been done, and there is no known increase in the incidence of injury as well. We hope this helps. Please reply if you have any more questions.

Thanks for your question. Instead of Radiofrequency Ablation , or RFA, the procedure when cutting the nerve is called a Direct Visual Rhizotomy (or DVR). DVR is done through an endoscope, which is a fiber optic device the size of a straw so the surgeon can see through the tip, yielding more accurate results than in an RFA where a surgeon uses a needle to burn the pain fibers but does not have a direct visual on the area. Cutting the pain fibers is not expected to cause numbness, weakness, or any other neurological complication. When DVR began, many patients were concerned that being unable to feel the pain they would further damage their spine. After five years’ experience, we can say there does not appear to be any downside to not feeling pain. Does this make a bit more sense as to what we mean when we reference "cutting the nerve"?

The R in RFA stands for radio frequency and not rhizotomy. Atami is the technical term for damaging a nerve fiber. So radio frequency ablation is done to cause riot to the dorsal branch of the spinal nerve root that goes to the facet joint. I hope that helps. I know the terminology is very awkward.

DVR can be done in the cervical spine, but as a practical matter, it’s usually not necessary. The cervical spine is so much smaller than the lumbar spine. That traditional needle based radio frequency ablation is less invasive and a better option.

@@BestPracticeHealth what about a minimally invasive anterior endoscopic disectomy as an option which will free any nerves that are being impinged or a bone spur or bulging disc material and giving more long term relief . Doing so without doing a full laminectomy which removes the complete back of the bone structure and the patient losing the stability in that region. The ablation can be so temporary 6-12 months and possibly having to do it again and you have to have 2 cervical blocks 2 weeks apart first to even gauge if the patient is a candidate

Sorry, I don't understand what you mean. But you should know that the most common medication in an epidural is steroid, and most steroids last no more than 100 hours. Some people may get some relief for 4 months, but the epidural was actually complete after 100 hours.

I am a medical doctor and I have had radiofrequency ablation done and it works very well. This video is a biased opinion by a surgeon who wants you to have only surgery because that is all he does and knows. No wonder the public is confused. I also had RF done for my knee and I am back to kite boarding. The surgeon told me my only option was a total knee replacement. He was wrong, again.

This may be a good idea to try for anyone who maybe dealing with Chronic Pain BUT NEVER EVER CUT ANY NERVES IF YOU HAVE CRPS/RSDS OR IT WILL ONLY SPREAD LIKE WILDFLOWERS AND CAUSE YOU EVEN MORE CHRONIC PAIN WORSE THAN YOU HAD IT BEFORE SAME WITH AMPUTATION OF A LIMB!! IF YOUR DOCTOR RECOMMENDS THIS RUN AWAY AS QUICKLY AS YOU CAN!! Otherwise it'd be something you might want to check out but always check with a second doctor before preforming anything like this as it could be the wrong thing for you completely so you never know! Great point, TEXASCRPS/RSDSWARRIOR J . The condition we call RSD or CRPS is pain in the distribution of a nerve as a result of an abnormality of the sympathetic nervous system. The conditions result from repeated damage tissue, or in particular a peripheral nerve. Cutting an affected nerve in this condition would be a disaster, and wouldn’t help the pain. You are very kind to point this out. Fortunately, this condition affects peripheral nerves, and does not typically affect the medial branches. That’s why we see foot or leg pain with RSD or CRPS and not typically back pain. That means DVR would not really be a treatment for RSD, CRPS regardless.

Thanks for your comment! CRPS stands for "Complex Regional Pain Syndrome" and is alternately known as RSD, or "Reflex Sympathetic Dystrophy." It is pain in the distribution of a nerve as a result of an abnormality of the sympathetic nervous system. The conditions result from repeated damage tissue, or in particular a peripheral nerve. Symptoms include burning chronic pain, inflammation (possibly leading to skin irregularities), spasms, and trouble sleeping. THANK YOU !!! You probably just saved my life..Since 2014 I've suffered with stage 3 RSD/CRPS that set in after a knee injury left untreated. It scared me so bad because I didn't know what the heck it was!..Purple right foot, and ice cold, but with burning hot pain..then it spread to knee, now it's attacking the area of a shoulder surgery on the left side, and into my neck...BURNING across the trapezius area, down into my left hand, changing it's color. I suffer with cervical stenosis and radiculopathy, and have a bad feeling that i may be rejected for procedures or surgeries/( at my own risk now)..YOU are the first to mention RSD. My doctor,(in practice for 30+ years has seen only 6 or 7 cases of RSD, it's so rare. I burn and burn and burn day and night. Anyway, I found your post...Thanks!

Great question. The nerve that is cut in radio frequency, ablation or direct, visualized risotto is a pain, fiber containing nerve only. It’s not sensory to any part of the body, nor is it motor. So cutting it causes neither numbness nor weakness.

That’s interesting. Steroid, epidural injection is a treatment for sciatica due to a herniated disc or nerve root impingement. RFA is a treatment for swollen and arthritic facet joints. They’re not treatments for the same thing. Which one do you have?

Hi, Elizabeth. I hate to disagree with you but I am not a practicing surgeon, and I have no bias for one procedure over another. Back when I was in practice I did both DVR and RFA procedures. RFA is a great way to relieve facet mediated back pain without much intervention, but it's temporary and often not effective. DVR is more permanent, but it's also more invasive, and typically much more expensive. Everyone is different, I just want to let people know there are options.
