Great, I guess if you're a medical professional with a focus on this topic, and eager to get your continuing education credits, but for those of us looking for the "punch line" of how you reduce already existing coronary artery plaque, it'd be nice to cut right to the chase with the answer/recommendation.

I could not agree more, after all what's the objective here? Yes it appears to be building patient base or some paid program when most people stumble across this looking for a solution to clean up plaque in the arteries and eliminate the production of additional soft plaque. Most people coming here are very serious about action based solutions.

I enjoy the deep dives. The conversation you guys have about the material helps clarify it. There’s a lot to study and learn about oxidative conditions and blood vessel damage. The elephant in the room still appears to be insulin resistance.

Insulin resistance is highly dependent on diet and time of eating. Show me a man who eats a muffin every 3 hours and I’ll show you someone with insulin resistance. ( I know it’s not perfectly correlated but it’s a safe bet)

My father had a triple bypass at 70, my younger brother has a CAC over 500, I had a CAC test at 52 and 57 both were 0 we all ate terribly our whole lives but I did 3 things different from them: 1 I quit drinking in my early 20’s, they are both lifetime social drinkers 2 I worked outside my whole life (vitimin D) they worked in offices 3 I exercised my whole life

So your saying drugs are not needed. How dare you ;) Nitric oxide production is mandatory for blood flowing and clot prevention. Nice summation. I love the family side by sides and differences. I'm always trying to be outside and skin exposed. I think walking is great but covered up isn't. Cheers

@@scoobtoober2975 I like Dr Brewers/PREVMED protocol namely arresting soft plaque, the heart attack causing clots if that means a statin after addressing food and lifestyle so be it. I lived in the northeast US so I was covered up during the cold winter but our eyes also stimulate vitamin D production so yes do get out.

I smoked for many years in my early life... I was a pack a day smoker and at the end of my time smoking, I smoked menthol cigarettes which I found to be highly addictive. I'm almost 70 now and I quit in my 30's, So I'm hoping that smoking will not effect My health now.

Dr Ford, Me taking a look at my heart is not taking me down a path to a bypass or stint if I dont choose one. Its just giving me vital information about the condition of my heart so that I can track its health across time, as well as how it responds to changes I make, and make informed decisions. There is only a downside if you are someone who does whatever ill advised thing you are told.

Please research K2 vitamin. I went into a deep dive on this and you will thank me with your life if you do. Please. There is compelling scientific evidence it can reverse plaque. High dosage only works. 6000 FU and higher.

Agree. I do not like the new format at all. It is way too long, is quite argumentative, and can be confusing. I have watched Dr. Brewer for years and he has saved my life; however, I do not think this new format will reach as many people as Dr. Brewer presenting alone. It is turning me off and I am a provider.

Mayo Clinic divisions is some states are informing patients that they are no longer accepting patients on "Advantage" plans (private insurance), but are accepting those on traditional Medicare administered by CMS (less paperwork and few, if any, pre-approvals required).

Unfortunately these Medicare advantage plans are starting to be exposed for not doing what original Medicare does especially in rural areas. But once you go Medicare advantage you usually can never get supplemental Medicare insurance again if you wanted to switch back to original Medicare. This leaves most stuck indefinitely with these advantage plans.

It's about as bad as the lifetime penalty for those who don't get Medicare part d within 60 days. I mean 60 days really? Plus is a lifetime penalty really fair especially if you don't even take any medicine? 🤷

This is great but why, particularly in relation to Oxidative stress and insulin resistance drivers, are so few CV experts looking at the correlation with the rise in CVD and high carb processed foods containing industrial process seed oils (post WW2) let alone lack of meaningful exercise?

I didn't know Bill Clinton went back and got his Doctor of Medicine degree - that's an impressive and admirable mid/late career change. From what we know about his presidency, of course, he was always very interested and engaged in anatomy, particularly the female anatomy, so it certainly makes sense he'd go the medical route as a career change. All that aside, he certainly looks a lot healthier here than the last time I saw him, so kudos on that!.

Dr Brewer...thanks for your hard work....I just had a total shoulder replacement one week ago...they did a ct scan prior to surgery to look at the joint...the radiologist noted that he also saw calcification in a coronary artery...I am 66 and have been overweight forever...despite being overweight I am in good shape (aside from my arteries) and exercise regularly...I take BP meds though my BP still runs high...I'm at the low end of being pre-diabetic...I have been vegan for the last 3 years...I've been a sugar addict forever...I don't want to do the heart health tests...I'm pretty sure about what they will find...so here is the plan...1. lose substantial weight...2. go low carb avoiding sugars and precessed food...eat lots of vegetables ...3. take K2 supplement and omega 3s... 4. exercise...5. intermittent fast with an eating window from 2pm to 6pm...It would be awesome to know your thoughts on this. thank you

Eat like our ancestors did (100,000 or so years ago). The hunter gatherers ate meat, organ meats, maybe some milk and cheese. (I take organ meat freeze dried supplements). Also, exercise as they did. Lift weights (they lifted stones and trees, etc) two to three times a week., sprints (6 x 45 seconds) twice a week.

@@raymondspagnuolo8222 humans are omnivores. They survived all around the world eating different things. Some ate only plants and some ate only animals. The theory amongst the most prestigious, Paleo anthropologist is that people survived mostly off of starches. Roots Hunting meat is energy, expenditure, and labor-intensive and often unsuccessful. You don’t just walk out your cave, and take down an antelope While All the men would go out hunting the women collected roots and other starches and plants from the forest and that’s what sustained them. Bringing home meat was a rare occurrence and often there was not enough to feed the entire families or tribe. Even when people started to domesticate animals, they did not eat one every day. If they did, they would not have any. Eating your domesticated, animals was for celebratory purposes for the most part, and very rare. Also, eating like our ancestors did, does not mean much in this day and age. If they lived to be 40 years old, they were lucky and rare. There is so much more to nutrition.

​@@debbiec6216I think it's more like what you *don't* take to reverse insulin resistance. Don't eat constantly. Fasting for at least 16 hours a day should help. Avoid eating carb laden foods constantly, that force the body to swim in insulin all day and all night, creating "insulin fatigue" (resistance) in the cells and especially inflammation in artery walls and the brain. I believe the theory is that eventually the body can heal/recover once the cells aren't constantly forced to deal with a chronic overload of insulin. Idk if it actually reverses so much as you adjust the factors that keep bombarding your body with insulin. I.e. stop eating carbs all the time.

Sooo interesting the association of inflamed arteries resembling that of acne on the skin. Because of my belief of “as above, so below”; the external being a reflection of the internal…for years I wondered if this was a thing, and whether or not I should be highly concerned about my facial acne being a reflection of my arteries and intestinal surface. No doctor had ever validated my queries. In 2022 I had necrotic tissue appear on CT scan of my left carotid artery. It was enlarged to the size of an egg over the course of 5 days, then just as quickly reduced down to the size of an almond. My cardiologist didn’t raise and eyebrow at all…had no answers and did not express concern.

@@SET12DSP This is superb information! Heard of Linus Pauling and vitamin C therapy but not wholly familiar with the use of Lysine and Proline. I’m 50+ and have had those skin conditions same as you mentioned. Vitamin C is part of my daily supplement routine. Good to know you’ve healed with these supplement therapies. I will certainly incorporated them as I’m still struggling with the weird scaliness of the skin. At times, my skin looks like a shingled rooftop of a house after a big wind storm passes (😆)lifting off some of the shingles. Skin dry with partial flaking skin just hanging and deliberate exfoliation does very little to smooth the surface. The dermatologists don’t even have a clue the solution other than trial and error prescription potions that does not work long-term. Thanks for sharing, and so good to be connected here to other like-minded individuals on this subject🙂😀

lost 17# but my blood sugar has gone up slightlyfrom 101 to 107. Still overweight but not obese. I am insulin resistant. 143 after 1 hour. a1c is 5.4. Lp-PLA2 is high 154. I am a vegetarian for 45 years. I will not eat meat. I do not eat refined sugar or flour. I limit my grains. 1 piece bread/day, no more pasta. My B/P has normalized from140/84 to 118/74. I will get a BiPap within the month for moderate sleep apnea. I have plaque in my left CA, right is clear. My plaque is hardened. I'm 69, it was at a level for 61 y.o. Tri 113, HDL 66, LDL 130. 2 docs have recommended statins. walking 2 miles day. I will add resistance training. My plan is to retest in 6 months and continue K2, D3 and magnesium, and weight loss.

Gotta do OMAD, 48, 72 and an occasional 5 day fast, lots of electrolytes. Sounds like vegetarian is not helping… you should be able to walk 2 miles fasted, again electrolytes.

@@andrewrivera4029 I disagree about being vegetarian although i will look into electrolytes. I have quite the family history. All of my family ate meat. My brother had a triple bypass at age 51 of which one has recently failed, several stints and has an aortic aneurism at the arch. My father had his first heart attack at 54 and dead at 72 with MI and stroke. My paternal grandfather died in his 50s and my paternal uncle in his 60s. They were smokers. My maternal grandmother lived to 99.5 on a more moderate diet and as a non-smoker. I'm doing well considering and at least I'm making positive steps. I do fast on ocassion and have since I was 22. It is a yogic practice to fast 2x month.

Many health insurance will not approve of expensive lab test - and will only approve of a limited number of blood test. So many primary care doctors are not going to be using these tests.

Yes, very true. So, there are many blood test sites online Ulta blood testing is a good one. INVEST IN YOURSELF. Spend the money if you can. My doctor loves when i bring in all thse test results that he can't order himself. It gives him a greater picture of my health.

Think in hydraulic terms. When the pipe or artery becomes porous due to inflammation sonething must block the leak. LDL enters the gap in the wall to block the hole to stop leakages by blocking the pores. Inflammation is caused by poor diet.. carbs sugar etc.. LDL is merely the canary in the coal mine. After this, the body responds to patch over the damage as it continues. After a while of continuous stress, these bandaid layers build up too much and are either cleaved off due to the higher flow and pressure or get too unstable.

Putting people on statins increases the rate of diabetes by ten percent or so- maybe more over time. At 1:29: the chart with increasing insulin over time might be showing the trend to increased diabetes.

I read that if your LDL is 50 or below, taking statins increases the chance of diabetes by at least 55% and increases strokes by over 150%. And they still want to put me on statins!

If I ate 200g of say, rice a day for three days here is what would happen: my immediate response would be BG of way over 200. Then it would take days of zero carb and or fasting to get back to normal. My liver would be in turmoil trying to turn all that glucose into fat. My lipids would be off the scale. The only person who would pass such a GTT is someone gushing insulin, which in itself means they are metabolically sick. I am as metabolically healthy and insulin sensitive as I can be on a very low carb diet. I do better still on carnivore. A1C 5.2. Fasting insulin 2.5-6.0. My liver does make a little too much glucose in the mornings. I regard eating any glucose as extra, unneeded and harmful to me. Been doing this for 25 years since diagnosed T2D in 1998.

So much knowledge, but I'm a bit confused when you can't interpret the diagram shown on the right about fasting insulin 1:30 (Bale, Doneen, Leimgruber-paper) They're transparent showing the downside that the fasting insulin actually increases over the 8 years this study covers. How can you not see this?

I've read it is more for clearing clots. It is also a blood thinner so one needs to be careful if taking other thinners. Then gradually reduce when stopping. Only a couple of weeks taking should be sufficient. Re Dr Russell Blaylock MD retired neurosurgeon. Excellent newsletter and books.

Hello from India. Ive had high lipids for 20 odd years. Since October ,Ive had 1 tsp soaked fenugreek seeds andc1 tsp chia seeds and 10 mg Rosuvastatin. I've measured 50% reduction in LDL,Vldl,non Hdl chloesterol and Triglycerides. Ive had 2 stents 21months ago and felt slight angina on waking. What is a non invadivecway of looking inside my cardiac arteries?

I have lost 26kgs since Sept 21. I am 70kgs. Next goal is 68kgs. Intermittant fasting from 6pm fasting -2pm and do alternate day fasring with added evening salad for sleep satiety. I have M.A.F.LD liver disease. It was F3/F4 with 12.9klpscals/snd . I have pulled it back to F0/F1 to 6.17klpascals/scd . I am fighting hard but so is the disease. My diett is mostly plant based with some light chicken breast ocassionally when my body needs it. Exercise 6days/week! Power walking 4klms/day 11.000 steps per day. I am 68.5 years old. Seems to me after listening that if inflammation is going in releasing enzymes to eat up or dissolve the plaque yet the process isn't fully successful by not dissolving the plaque thoroughly enough for it to pass through but rather clot and cause a heart attack would the root cause be never have the plaque to begin with? If a person has plaque build up they are definitely at risk of dying from stroke or heart attack. Would it be possible to hear a plan to reverse the plaque and clean the arteries?

Vitamin K2 supplements seem to be increasingly popular as a way to de-calcify the arteries. From what I've heard so far, listening to this discussion, pulling the calcium from the arteries would have the potential of destabilizing the calcified plaque and therefore lead to a higher risk of more plaque breaking loose and causing blood clots. Any ideas on this topic?

That’s a great question. It’s logical & very popular. There is inadequate evidence to say for sure. But in the patients where I saw a decrease in arterial calcium, the patients clearly improved their situation. They were not destabilizing their plaque.

Paul Mason speaks about MMP-9 being responsible for unstable plaque and not the lack of calcification. Calcification in arteries just doesn't sound right. There is something else going on that hasn't been widely recognized yet.

Hi Dr , as a random study being a semi pro athlete, ex boxer and fitness gym maniac. I recently finished a 16 week keto diet, lost 1.5st and managed to get my fat content down to 7% I have my abdominal muscles poking out 8 pack but when I run my routine bloods after the diet to strip down I found my ldl level was very high. 6mmol but my HDL/Triglycerides level was very low 0.5 mmol. I then payed for a CT Calcium Score Test as I wanted to make sure my arteries was ok . The Calcium score test came back as 1.. I'm now back on a normal training/eating plan (not keto) my ldl has dropped back to normal levels, my blood pressure is 110 over 65 (which is good) I'm not diabetic and my fat content is still 7% very low as I'm really lean... is a Calcium score of 1 dangerous and do you think I get back my calcium score back down to Zero? As its only 1 I have to lose? If you reply to this message I appreciate it.. thanks. P.S I live in the U.K (United Kingdom)

RU-vid has videos. Activates in mitochondria. Allows last engine to run more effective, improved ATP production. Improve blood sugar, deals with the low grade Inflammation your body has as it ages. Lights are made commercially. Infrared and near infrared are what you need. Can't believe this works. So much could be improved. Seems to be a catalyst for exercise to help with diabetes. Wish they would do more clinical trials. Just grateful I have 😊

The thing about atherosclerosis, is that the veins, nerves arteries get so filled up with cholesterol that turn into plaque and harden , making the path of the blood very narrow, and any slight harsh movement in the body will cause easy inflammation, but fasting., lots of vegetables and relaxing and walking and hydration will do wonders., avoid all high salt/sugar foods, refined carbs and sugars, fried food specially fast food, and saturated fats ., go for cruciferous vegetables specially raw form less cooked, leafy greens, and definitely not oils added of any kind, and sunlight /vitamin D in moderation dissolve plaque on the inside turn it into cholesterol again and little by little bring it out to the surface cleaning the inside., all those tips help .

Also great Lots of onions, bell peppers, garlic, basil etc, and for last less high starchy fruits such as mango, banana, papaya and more high water and citrus fruits like orange, lemon, watermelon, berries etc.

Veinous thrombosis can occasionally be an issue, but not often. Veinous atherosclerosis is even far more rare.

My 79 year old father just had a mini stroke and Drs at Mayo Clinic, with imaging, discovered he had multiple tiny clots in his brain prior to this He eats a high carb diet but stays relatively in good shape. He has always had excellent blood pressure maybe even low. He just stopped playing hockey last month just saying this is an active person. His total cholesterol numbers were ok except Trigs 150, HDL 35 I can’t remember lDL I think his LDL was a few numbers above standard cut off, barely tho. I’m sure they are going to drug the heck out of him now. He refuses to hear me on the carbs and statins. He only hears Mayo Clinic.

Valtaylor the dirty little secret of the Carnivore, Keto, and even the vegan world is these lifestyles encourage a low insulin response diet. Fats have the lowest response, next are protein and then carbohydrates. But the most effective thing is fasting. Every time you eat or drink something you get an insulin response. And if your snacking that response is CUMULATIVE. It takes 4-6hours for the body to clear the insulin. But if your snacking you'll drag out that clearing process and cumulatively raise your insulin. That's why you should aim for at minimum a 14hr fasting window. Preferably overnight. There's only one way to measure a fasting insulin level and that's through a lab test. A proxy would be your bathroom scale. If your losing weight your lowering your insulin, if not then your either maintaining that insulin level or raising it. Demand a fasting insulin test with your next A1C. You can have a normal A1C with an elevated insulin level because your body is compensating. At least until it can't then you'll have type 2 diabetes. Which is reverseable with the above advise.

How does the ldl get between the endothelium cells ?????.It cannot in the presence of a glycocalyx and that is where this hypothesis falls apart. Strip the glycocalyx and this will cause inflammation in the endothelial cell causing it to rupture and evacuate the contents into the lumen resulting in a micro clot,It repairs by anti fibrinolytic agent breaking down the clot as endothelial stem cells in the circulation repair the damage and restore the glycocalyx.If the originator persists the damage will persist,overwhelming the repair mechanism with gradual increase in plaque in the vessel wall.ApOB has zero to do with it,anything that prevents the micro clot and anything that increases fibrinolytic activity are helpful.

Niacin improves lipid markers, but it does *not* reduce either the risk of all cause mortality or cvd mortality in almost all studies. Niacin is a red herring, in that it tricks you with improvements on lab tests, but it does not appear to help out with the desired end result. Compare that to statins. Statins appear to significantly reduce the risk of CVD and all cause mortality.
