doing a particular mission for Neoloth , you have to track down and find the assailant who's trying to kill him. eventually you find out it was one of his former students who had a heart stone implanted into her chest! well what do you do? Rip it out like a boss.

Nope it’s in the vanilla game of Skyrim . It’s only possible in a single mission called “old friends”

I’m telling you can only do this finisher to the one enemy in the mission old friends. Unless you mod the game. I’m not talking about loot. Neloth even requests you rip her heart out. Lmao
