There are many conflicting viewpoints on what drives weight gain. Excess calories? High insulin levels? Unwavering hunger? Dhru has been on a quest for answers to learn exactly how to shift body composition to lose fat and build muscle.
Today on The Dhru Purohit Podcast, Dhru welcomes Dr. Ben Bikman on the show to continue his summer weight loss. In this episode, Dhru and Dr. Bikman dive deep into insulin's critical role in weight gain, obesity, and cardiometabolic disease.
Dr. Bikman explains how high fasting insulin creates insulin resistance, grows fat tissue, and makes it more challenging to achieve sustainable weight loss. He also touches on how our genetics influence weight gain, the power of protein and resistance training, and why we should focus on lowering our insulin levels first for weight loss before worrying about calories.
Dr. Ben Bikman is a renowned metabolic research scientist and a popular speaker on human metabolism and nutrition. Dr. Bikman has a Doctor of Philosophy in bioenergetics and completed his postdoctoral fellowship in metabolic disorders with the Duke-National University of Singapore Medical School. He is the author of the book Why We Get Sick, which offers a thought-provoking yet real solution to insulin resistance and reversing prediabetes, improving brain function, shedding fat, and preventing diabetes. Currently, Dr. Bikman is an associate professor at Brigham Young University with the mission to better understand the role of elevated insulin in obesity and diabetes and the relevance of ketones in mitochondrial function.
In this episode, Dhru and Dr. Bikman dive into:
-The perfect formula for creating a sick society
-The most underrated blood test for metabolic health
-Optimal fasting insulin reference ranges
-The two primary factors that contribute to weight gain
-Dhru’s weight-loss journey and its relationship to calories and insulin
-Is it possible to gain weight on low-carb diets?
-Dr. Bikman’s stance on the role of calories in weight loss
-Dr. Bikman’s thoughts on using satiety as a metric for weight loss
-The protein leverage hypothesis
-Protein recommendations for optimal health and weight loss
-The role of skeletal muscle in glycemic control and diabetes prevention
-The link between resistance training, muscle mass, and insulin resistance
-Dr. Bikman’s four pillars for eating a healthy diet

Love how Dr. Bickman knows how to express politeness and true respect of other professionals in a harmonial elegant way. Hes a great example of "you can carry alot of knowledge in your brain, disagree with others, without needing to be pompous and arrogant about your own intelligence." Arrogance does not add to ones knowledge it decreases it.

Here is what I did for weight loss and overcame on health issues I had. 1- removed junk food from my diet 2- I started eating Whole Foods including fruits, nuts and honey. 3- eI eat mostly two times a day. As a result, lost 52 lbs in seven months and right now after two years, I have my weight maintained. Pre diabetes gone and Hba1c dropped to 5.2 which was 5.8 when I was sick. My blood pressure 162/90 but now 113/72. Triglycerides was 136 and now 63 Also my hdl raised to 60 from 35. I eat everything healthy but twice a day only. I also don’t know count calories.

I’m 58/M, a stage2 breast cancer patient, had surgery MRM and underwent chemotherapy. Everything discussed just affirmed what I’ve been doing the past 9 weeks. I’ve lost 15 kg from 85 kg body weight, well within normal Asian BMI for my height now; my waistline dropped from 35inches down to 32inches and I don’t have cravings anymore. I stopped taking antihypertensive and antilipidemic meds a month ago since my BP is now 120/80-110/75 range. Next month, I will have my lipid profile, A1c, FBS, fasting insulin, etc lab works. Here’s hoping I have reversed my insulin resistance. Thank you for this video. Mabuhay!

0:22: 📉 Dr. Ben Bickman discusses the recipe for making society sick, fat, hungry, miserable, and ultimately causing a painful death. 10:20: 📈 Insulin is a key factor in fat cell growth and energy storage, and its absence can lead to weight loss even with high calorie intake. 25:42: 💪 Building skeletal muscle can improve insulin sensitivity and metabolic health, but it may also result in the accumulation of belly fat even with optimal insulin levels. 28:34: 💡 Insulin resistance and weight gain are not directly correlated, as weight gain may indicate insulin sensitivity in fat cells. 37:35: 📊 Insulin resistance in fat cells leads to weight gain and metabolic consequences. 46:30: ✅ Lowering insulin levels is the key to successful weight loss. 1:01:17: 💡 Protein is an important macronutrient that should be prioritized in nutrition, as it is not a fuel and does not contribute to calorie intake. 1:04:25: 🥩 Don't fear fat and prioritize protein with fat for better digestion. 1:12:45: 🤝 The speakers engage in a healthy discussion about the satiety per calorie model and its focus on protein, fiber, and hedonic score. 1:22:06: 💪 Muscle is a metabolic powerhouse that can absorb a significant amount of glucose, making it important for metabolic health. 1:31:21: 💪 Increasing muscle mass and challenging the muscle through exercise can help maintain steady blood glucose levels and improve insulin sensitivity. 1:41:15: 🥦 Ben's four pillars for a healthy lifestyle: control carbohydrates, prioritize animal source protein, don't fear fat, and frequently fast. Additionally, engage in physical activity and establish connections with people who share common values. 1:49:50: 📚 Ben Bickman shares his contact information, his entrepreneurial venture, and his book 'Why We Get Sick' which focuses on insulin resistance. Dr Bikman gave a wonderful insight into fat cells - It was fascinating to hear how some ethnicities can “ multiply “ fat cells and thereby grow fatter without having health consequences whereas other ethnicities cannot multiply fat cells , and their cells become so enlarged that they ultimately start leaking to prevent bursting .. And thereby cause metabolic disaster to the body.
@lisasternenkind6467
@lisasternenkind6467 3 месяца назад
Yes. It's natural that ethnicities differ due to the different nutrition situations past generations have faced. I greatly dislike how woke cultists try to control science by stating all people and ethnicities are the same. They are not! That's why all ethnicities have certain diseases they are prone to have. For example sickel cell disease, which prevented malaria for the forefathers of black Africans. Our DNA is so specialized on our epigenetic values. That's one of the wonders of life. 🌱

I used to manage fitness clubs, and I found that most people did A LOT of "wishing" (they weighed less, were in better shape, etc.) and I used to tell them to take all of that "wishing" and convert it into DOING. Unfortunately, most people who start a fitness regimen, don't realize the discipline, determination and WORK it requires to get into and stay "in shape", and they (sooner than later) either start "tapering off" or just quit all together. I'm 66, and I'm in excellent shape, with no health issues, because I want it bad enough to make it happen; most people "want it" but lack the right attitude and discipline to get it done, as it's easier to eat, drink and watch TV.

Type 1 diabetic, and trying to reset my food -- way too much cheese, because it doesn't raise glucose much. Still fattening, though. "Insulin" is not the enemy -- we'd all be dead without it. "Excess insulin" is not as easy to control as people seem to think, when you have to control it yourself, control your eating, etc -- it's overwhelming sometimes. You don't know how lucky you are if you are worried about "insulin resistance" and are not even diabetic, and can still make your own insulin and amylin. Wish you could do an episode for T1D who are having weight gain issues, because there are too many of us. "Choose: High glucose, or fat storing insulin, or obsessing about not eating and exercising all of the time." Theory goes out the window in the real world. As a lifelong insulin dependent diabetic, I understand the complaint that T2D are being given too much insulin or sulfonylureas to reduce glucose, but some of us need injections of this slow and dangerous medication or we croak. And so, more of a qualifier needs to be said by these doctors, because not all diabetics are in the same situation, and we don't have a choice. It is always going to be to inject engineered insulin, or get sick and expire. I'm glad he mentioned us, but missing an easier solution. Obviously "diabulimia" is self harm and eventual suicide.

I would need more info to be more helpful. See Dr Ken Berrys and Dr. Lustig and Richard Johnstone and Daniel Amen for starters. It's a lot of homework. Slow steady progressive but better than the horrendous suffering that diabetes causes By the way type 1 or type 2 can be mixed up by even endocrinologist.

Ben Bikman: Nobody puts insulin in a corner. I'm sooo glad you pressed him on weight gain while in a low insulin state! Because that's a question that needs answering. Those who are all or primarily about the endocrine model of weight loss constantly try to hand wave caloric intake. Like you, Dhru, I endlessly see anecdotal reports (on Reddit fasting/carnivore/keto subs) of people eating low insulinemic diets who aren't losing any weight, have plateaued with their weight loss, or who are even gaining weight. I lost half my body weight (from 252 to 124) and my last DEXA puts me at 18% bf (I'm female). But all along the way I experienced plateaus that only fasting would bust. I did many a PSMF cycle and the needle wouldn't budge for a week or even more. There is an explanation for that that cannot be explained with just insulin. Or, if it can, I haven't heard the answer yet. Not even from Ben Bikman. And lastly, why is "counting calories" such a dirty word? I did it and I loved it. It provided structure, accountability, and knowledge! How else was I going to know how much protein I was getting? How many calories I was REALLY eating? NO ONE knows how much they're really eating by just guessing and doing mental math. Now, are calories/thermodynamics perfect? No. But in my experience every obese person trying to take the weight off must weigh and log everything they eat, at least for a significant period of time, if they want to maximize their results.

If people are drinking 5 gallons of heavy cream a day they will gain weight because the fat doesn't disappear, it just goes to the adipose tissue first and not to the liver first unlike the fats produced from fructose or alcohol which go straight to the liver. That's why many low carb doctors discourage liquid calories and want people to eat the fat that comes with the protein and don't add more. I think the most powerful combination is a low carb diet that lowers insulin combined with time-restricted eating (which is easier to do when your insulin is lower and your body body is fat adapted) and also do some kind of alternate day fasting. I had struggled with my weight since I was a kid. I finally was able to attain sustained fat loss with a keto diet but I still plateaued after I lost 100 pounds with 70 to go. The next 70 I lost when I incorporated 2 or 3 days of non consecutive fasting days per week. That way I reduce overall calories without having my basal metabolic rate collapse which happens when you do the same caloric restriction everyday. Love the work of Dr Fung regarding this process. I also just eat 2 meals a day during my eating days eliminating all snacking

It’s obvious that the daily loading of sugar and simple carbs are driving this problem. But admittance to it and the subsequent corrections might not gel well with the food and healthcare industry.

I really respected Ivor Cummins videos and his analyses of Covid, the lockdowns, epidemiology and all cause mortality graphs throughout the pandemic and how different countries approaches created better or worse outcomes in a holistic way. He's an engineer, not a virologist so he brought a refreshing and novel viewpoint to the topic.

Question: are we doing this same thing to our animals? Pets and the ones we eat (cows being fed corn & other things)?
@PiujuqInuitArt
@PiujuqInuitArt 3 месяца назад
Yes so cows and pigs can have leaky gut so if they are grain fed we are exposed to the grain proteins by eating the grain fed meat. And that has all kinds of legal implications.

Thanks for having Ben, and I have to second all the above positive comments in reference to him! Wondering about DHA, which is a high priority to keep my brain from shrinking but Alaskan cod, which obviously doesn't have any fat, so wondering about which fat to add, and what about salmon, which has more fattiness, but wondering which fat to add, and how often he would recommend, to have seafood, but not be shortchanged on DHA. It seems I feel less satiated after a fish meal.

Dr. Bikman said something that contradicts most every diet authority (meaning teacher or doctor, influencer or other person's informing the general public) denies, and that is that consumed fat can be stored as body fat (46:54). I have been told for years, that is not possible. I have struggled to understand why I am putting on body fat when I was eating only moderate protein (not enough actually according to most online health authorities), high fat and very, very low carb (averaging 1800-2000 calories per day). I can even gain weight on a high fat carnivore diet! Every other authority seems to claim it is the protein being converted to glucose and that the glucose is then stored as body fat. But Dr. Bikman seems to indicate that the consumed fat itself can be stored as body fat. This is paradigm shifting.

You don’t have to watch your calories if you’re eating real food. It’s really hard to over eat veg and meat.”Keto” snacks and packaged foods are never really good. If you’re snacking, you aren’t eating enough of the fat/ protein or You’re triggering your insulin and creating hunger for yourself..

There's reference to the impact of 'other' hormones but I'm not sure you can have this conversation focused on insulin without dealing with the combined role of cortisol, the HPA axis, and gut health, in changing metabolism.

Dhru: to move forward along the lines of current metabolic debates, a meaningful step would be to consider the work of Dr. D'Adamo on blood type. He has uncovered differences in the amount and type of protein individuals can digest and the types of lectins they react to. You have got to get Dr. D'Adamo on your podcast!!!
