RN TO FNP TRANSITION| The first 6 months. In today’s video I’ll discuss an overall recap of the transition from experienced Registered Nurse (RN) to brand new Family Nurse Practitioner (FNP). I’ll discuss my biggest challenges I faced in the first six months as a new provider in primary care, some of the unexpected emotions I had, some of the things I’m loving so far, and some tips to make your transition to NP a little bit easier. My hope is that if you are feeling some of these things while you start a new role you will feel a little less alone! Hang in there. It will get better!

I’ve been in the medical field for 20 years 16 years as a nurse and I’m Viewing this the night before I start my first NP job and I just can’t sleep due to the high anxiety level. Glad to hear these feelings aren’t alien.

Haha the social aspect you mentioned is so true. Coming from a busy ER with a bunch of nurses to be a provider and being seen as a supervisor, this was def different to overcome! I totally didn’t think it would be this “lonely.” But you get used to it. Now our spouses just get to here more of our issues lol. Congrats to 6 months!

hahaha best of luck to our spouses! Is it hard working at home? I feel like that would be a whole new level of weird. But also awesome. You cant beat that commute! Or proximity to the kitchen haha.

@@NurseLiz it is working hard at home, but i only do it twice a week so its not bad. it isnt as bad as i thought it would be in terms of feeling isolated. If i did this right after being an RN, i would be miserable. As i eased into the NP role, being by myself isnt so bad. The hard part is having to clean all the time hahha, especially with how my son terrorizes the house.

I think every time you switch specialties or roles you can go from being an expert to novice nurse, until you build up your skill set in that specialty, and that can be a difficult adjustment. I believe that if you give it time and have good people and resources it all works out. At least that what I keep telling myself before starting my FNP program next year.

Hi Liz! I can so feel your pain through the screen. All of your reasons and feelings for being on struggle bus are valid. I started to have apprehensions about the NP program because very similar reasons. I finally feel confident and secure that I am on the right path v educator. But, I already miss the nurse culture not working as much because of school. You mentioned having difficulty with asking questions, because you don’t want to seem not knowledgeable, however, one way I look at asking questions is a means of communication and learning :) Thanks for sharing your genuine feelings, I love your videos!!

I am in the first 2 weeks of practice as a primary care NP and omg GAWD what an emotional roller coster!😭 😆🤪 As a nurse with 16 yrs experience coming from being an expert in PICU I can say that I am now just a bundle of nerves and overwhelmed with everything. Drowning in a sea of newness!

ALways love your videos!! So this is not exactly the same but I feel you on a certain level. I started my BSN program 5 weeks ago and its been sooo hard. Like I was worried about all these other things and didnt even think it'd be hard on the levels ive actually been experiencing. Thanks for sharing youve given me a little spark of hope hahah.

I will be transitioning to FNP role next year. I have been practicing as a Nurse Case Manager for the past five years and I am blessed to have had that experience. Many of the struggles you faced during your RN - FNP role transition are similar to my transition from bedside nurse to Case Manager. I have gained valuable knowledge regarding insurance policies and regulations in the state of Alabama that I hope has better prepared me for resource utilization as an NP. Thank you for your video! loved it:)

6 months as a FNP too! I am feeling the same as you. My confidence is slowly building, I love seeing patients at their follow up and they have improved or they followed your preventative suggestions when they never would do them before, or finding a metastatic skin cancer when listening lungs posteriorly...That is what's cool about being a FNP! I am sad that it has taken me so long to get into the job and my career may not be as long as some of you, but I am truly happy that I chose this career path!!!!

It's great that you're feeling better in the NP career. I can see how it would feel stressful or doubtful. Anxiety and stress are the silent killers. I'm going into med lab tech myself starting this fall and I'm worried but that worry also leads way to excitement and a lot of other feelings. I know I'll be fine with classes and practicum later on in the program. Although I know it'll be like a fish out of water moment where I'll doubt my own existence. But we all do that so it can't be bad lol! Anyways, love the cheerful vibes you give and the helpful tips!

I really appreciate your insight...This us my feeling exactly...I have three classes left and I feel I KNOW NOTHING....my preceptor tells me all the time you are so advanced already but i feel otherwise....Im doubting myself and its blocking me i feel if that sounds right lol...I feel all the time that i should have stayed a nurse..because like you said I was the person everyone came to...I am in primary care for my rotations and is def helpful....I hope it gets better🤦‍♀️...thanks for encouragement

Thank you for sharing this! Soon to be new NP and I'm feeling nervous. When you said you step out to look for guidelines is this because you don't have access to them in the room or because you want to look it up in private? I have my Ipad, and books in the room with me and I wonder if I should grab them and step out to look things up?

I'm on the cusp of nursing school and honestly I don't know if I want to do it. I'm terrified. I am not a very good student, I don't like to do busy work which is most of what college is. Oddly with all of the cons this video was encouraging. I want to be an NP, but I've got to get the nursing chunk out of the way first. I have doubts, I'm looking at other options but it's been good to realize that I'm running my own race and if I need a good break before hurdling through (what has been said to be) the hardest thing I'll ever do (super encouraging right?), then that's what I'll do. I guess it'll all work out in the end? Oh! Also I'm terrified of making it through nursing school and then immediately regretting my decision. I don't do well with change and I'm utterly terrified I'm going to work a week as a nurse and want to give up completely - which I won't be able to do because I'll have a mountain of debt to pay off. This industry is really rough.

Baili Wilkinson first semester nursing student here! The first thing I’ll say is that it is definitely not busy work, you are learning SO much material that will be useful to your actually career. Also, you’ll have to change your mindset. It’s pretty challenging but I’m am loving it so far. You’ll have that feeling of being unsure for the next two years of nursing school and for at least a year or two starting off as an RN but from what people have told me, it’s definitely worth it. You won’t know if it’s what you want to do till you try it! I’ve had a few moments of completely doubting myself but I wake up the next day excited to learn something new! Try shadowing a nurse before you start to get a general idea of what your in for, that might help you decide if it’s for you or not.

You could be a direct entry midwife but your scope will be more limited than a CNM.

Baili I'm honestly not the best student either. But you can absolutely do it if its what you want. I think one nice thing about nursing is how wildly broad it is so there's likely an area you will find and like in terms of worrying about finding a job you like. Hang in there! We've all been in that place of "oh gosh is this what I want to do". I think its normal, especially right before school starts

You are so relatable! I am currently studying for my FNP boards! Did you feel like you had to look up a lot of things or ask a lot of questions in your first 6 months? How was it finding a job right out of school? Any tips for interviews? Resume vs. CV? What state do you practice in? Thanks in advance!

I am completing a DNP now but never became an NP due to confidence. I feel confident now and may go for a post DNP certificate for an FNP. Been in the business since the 1980s.

The whole decisions things as an NP does make me nervous because I want to be a cardiac NP. The heart is very sensitive and as an NP my choices affect my patient and their care. I am gonna prepare myself for this as I go forward to be confident and feel competent. As a patient myself I totally get the part about a doctor referring you somewhere or prescribing some meds and as a patient you have to say “I’m sorry, I can’t afford to do that...” I have to push my blood work back sometimes for my warfarin once in a while sadly. It’s life and that’s a small part.

I had a complete melt down regarding this the other day after clinicals even! I graduate in December and I hate being a beginner at things. I want to be fully prepared for what I'm getting into, but I know that is impossible for this exact career.

I literally cried watching this video. It kind of took some of the weight I have been feeling off. I am newly graduated, test in 4 days and I am just overly anxious about taking the leap into being a provider. I haven’t even allowed myself to apply for a job. I know I won’t know everything and I’m ok with that but I’m stuck with so what do I do when I don’t know. I mean yes I will have a collaborating physician but while I am in the room with the patient, what do I do. I don’t want to look incompetent to the patient. It’s very intimidating. Thank you for this video

Ugh starting next week I'm going to be the new kid on the block! I've been doing dialysis for 2 years, and I've been charge nurse for about a year. I've become the go to person for my patient, techs and other nurses and even some of the doctors ask for my insight on patient specific issues. Now I'm going to feel like a walking durp.

Godddddd I'm a new RN (1 year, 6 mo SNF 6 mo hospital) and I'm just thinking of my long term plan. I definitely do not feel like an expert at all, but at least I don't have panic attacks before my shift anymore. Just the thought of eventually going through that again....I'm not sure I have it in me. But bedside forever......:OOOOOOOO

Hi, liz. Firstly, thanks for your video. Since I started to subscribe your channel, I've been getting lots of information about NP or FNP. (So, I'm so grateful to you.) And I have a question for you, which is that "Is it possible for anyone who doesn't have any experience in hospital as a RN to get into Master's or Doctor's degree?". Of course, it would be more helpful having some experience as a RN in hospital, but it seems not that essential part, although there are some differences in each university.) I mean, it would be great to everyone who are ready to go to graduate school, if sharing your process of procedure to going to graduation uni.

Depending on the program some require nursing experience and some don’t. I went to one that did require it, but I don’t think it necessarily had to be in a hospital.

Hi nurse Liz! You're videos are so motivating. I was curious as to your opinion on online fnp vs an on campus program. I applied to both but feel the online path would allow me to continue to work. Do you feel an online program will affect an employer's decision to hire ? I work in a busy ED and have already found 4 potential preceptors for clinical rotations. Unfortunately, all the programs I researched do not help with clinical placement, even the on campus programs.

I think it totally depends on the online program! Some of them scare me a wee bit, but others seem awesome! I’d see if your state board of nursing has any regulations on it. Some only accept certain online schools and accreditation

I'm an ADN student, one semester away from the taking the NCLEX. I'm not really sure where to go from here, can you help me understand what requirements I need to apply for NP school? Also when I should start preparing myself.

I have been an NP in a oncology specialty and still have days where I ask questions. It will get better but it sure was a hard transition for me as well.

Nurse Liz - I would say it took me close to two years. I can clearly remember just 6 months in my collaborating doctor left for a one week vacation and I was left alone for that week. Sure I had a backup doctor off site I could call but I had to learn to sink or swim. I did have 13+ years of oncology experience as an RN but as you know the roles are so totally different.

Hi Liz! Thank you so much for the video. I will be graduating from an entry level Masters in Nursing (Clinical Nurse Specialist) program. I have no bedside experience but my career dream has always been to become an FNP. What are your thoughts on becoming an FNP without any bed side experience?

Do you advise having L&D experience as an RN before becoming a Womens Health/GYN FNP ? I have half way through an FNP program but I've been on an adult acute surgical unit for 5 years.

Having your name on something is the worst responsibility ever. I go home every day thinking did I make the right decision in my treatment plan. Barely a month in, but i know it will get better.

Also....nurses on boards. Find a board in the community where you work and advocate for programs to help those in medical need. We had a program here where people only paid $15 and they could see and doctor for that fee.

I'm really wanting to do the Bachelor of Nursing here in Australia. Do I need to be a genius at maths and science? I'm not stupid, but these subjects aren't my strong points. What's your opinion?

I don’t think they need to be your strongest, as long as you can get by with them. Math isn’t my thing and I was fine!

I am curious to know how much more into depth do you need to know from RN to FNP? I feel in nursing we have to learn a lot and know a lot of things. Would you be able to explain more on depth of knowledge from RN to FNP a person will acquire becoming an FNP

It was quite a bit more in depth for the NP program. A lot more assessment, differentials, and looking at the root of the problem.

Passed boards May 2020. August 2021 found a volunteer position at a clinic as a NP just to gain experience. July 2022 still no NP job. Graduated with honors from Duke University. Anyone else have a rough transition to a new NP?

You might want to connect with your local health department. Nurses there can facilitate linkages for your clients.

You can see any age, who you actually see depends on where you work. My office sees mostly 5 years and older

The more you can pretend like its real life the less nervous I tended to be! And lots and lots of practice at home, going over the steps!

I met a FNP, an older woman, who asked me if I needed to pick up an infant if I needed to feed it a bottle. I was seeing her for a workmans comp issue. I hurt my arm in a fall I had at work in a daycare center. I almost asked her if that was a trick question.

Hey there, kinda random question. I'd love your opinion as a NP about shopping around for NP's as a primary care provider. For perspective, I'm not a nurse. But I am familiar with healthcare and the different practitioner modalities. I majored in medical anthropology, and my fiance is a physician (in Mexico, and we don't have NP's here). I prefer the nursing ethos compared to the medical model. I'm shocked how difficult its been to find a FNP in San Diego with my insurance. Like, when I search for an FNP as primary care provider on my insurance website, barely anyone pops up and if they do they're not accepting new patients. How would you feel, as a practitioner, about prospective patients calling your practice and asking questions about it?

I dont see anything wrong with someone calling and asking some questions to the MA's about the practice! I'd say its probably a good idea to get the vibe of the practice! Best of luck on your search

Hi...I'm glad I found your channel. I've been an RN since 2011 and I've started looking into possibly going back and enrolling in an RN to NP program. Maybe you've answered this in another video....but what school did you go to? I want to do online due to working full time and having 4 kids. Congrats to you for making it through the program...you seem like an awesome NP!

Glad you are here! I went to Michigan state. It was a part time hybrid program so we were on campus once a month, and it was 3 years
