Thank you Dr. Philips- very useful. There is also this big discussion among specialists (although still divided) on test and treat for HBV ( so treat once HBsAg positive) or decreasing the HBV DNA threshold to 2000 from the current recommendations of 20,000 IuML. There are also interesting recent articles about the benefits of treating patients at the immune-tolerant phase and the association of advanced liver disease with higher viral load. The HBV medications (on at least a functional cure) and recommendations on when and whom to treat are going to evolve rapidly and will be a fantastic area to watch out for in the next 5 to 10 years. I look forward to more of your talks.

Last year, during a surgery, I was diagnosed with hepatitis B. Since then, I have been taking Taf 25 mg as prescribed by my doctor. Thanks for the wonderful video doc! I have a query, please do answer me. My husband is Hbsag positive ,since the last 30 years, his viral load last checked was 4000, earlier it used to be more than 10,000. LFT was normal, Ultrasound was fine. Can we go for treatment to completely cure him of the virus? Because all along we have been told it is a sleeping virus and treatment can actually activate it! Waiting for your reply!

Hi Doc, had a question. You have mentioned that hep b doesnt need treatment unless fibroscan gives a score of >=8 and in some other specific high risk scenario. But if left untreated in a normal, otherwise healthy person who doesnt fit into any of the scenarios u mentioned as needing hep b treatment, wont the virus eventually end up impacting the liver ? Why do these ppl not need treatment?

Doc, you have to talk about Subi Suresh, the 41 year old actress who died at your hospital in Aluva. Only the superintendent is talking to the media and he said liver transplant procedure was underway. As a hepatologist there, you weren't in the loop? He said she had liver problem which spread to kidney and then the heart which lead to the death. Subi had also said in her yt channel that she had many problems because of not eating food on time. Hi Dr. thank you so much for great information! Sir a patient has history of rap recurrent acute pancreatitis 6-7 times in last 15+ years aged 53 female ... diabetic hypertension.... gallbladder removed in 2014 ..uterus also in 2012. Patient stable on managing diabetes n hypertension...we did mri in 2017 it showed pancreatic divisum ...then after that few acute pancreatitis attacks occured vd major in 2021 early then few vd no hospital admission required...then we did mri in 2022 it showed pancreatic divisum n fatty liver then Dr advised us eus endospic ultrasound it showed chronic pancreatitis early...patient has no pain at all only gets when when there is an attack ..patient was put on enzymes creon 25k bd then now on pancreoflat 170/80 bd n other medications...patient is stable. There is difference between GASTRO some say stent is required some say it's not..some say it's heridiaty pancreatitis.and not chronic pancreatitis because there is no pain .so kindly guide us what to do ...thanks

so i talked about this on twitter too, my hbsag is positive, viral load and hbeag are negative, but i had HCV in past and got treated. My liver is fatty with fine margins but didn't have fibroscan. My hepatologist asked me to take entacavir and take it for life and he said no harm in the drug even if no viral load.

You should immediately tell him/her, you are risking your partner's life by not telling ; the virus might not be affecting you, but it can affect your partner, as everyone has different levels of immunity.
